It plays an \u0000important role in signaling initiation, activation T-cell receptor, phosphorylation of multiple \u0000downstream targets. So that believed as T. lymphocyte specific .Several studies is gathering that the \u0000pattern of expression is not lineage specific. We aimed to study the role of ZAP-70 /CD38 as new \u0000prognostic marker in CLL and its prognostic value in ALL patients. Approach: Our study included 49 \u0000ALL ranged from 2-42 years they were 27 males and 22 females. One hundred fifty six patients with \u0000CLL. They were 87 males and 69 females, their ages ranged from 50-77 yearsImmunophenotyping. by \u0000flow cytometry (EPICS XL-2and 4 color Bechman Coulter USA), intracellular ZAP70 protein was \u0000assessed by flow cytometry using (Caltag laboratories, Beckman Coulter USA). Results: \u0000Immunophenotyping of 49 ALL patients ZAP-70 was found at significantly higher levels in T-ALL \u0000cases compared with B-lineage (p<0.001), however, a proportion of B- lineage ALL showed high \u0000levels of ZAP-70. Moreover we observed that ZAP-70 is higher in pre-B ALL than pro B ALL and \u0000common ALL with more decline in mature B-ALL. Follow up patient by zap, pretreatment versus post \u0000treatment p = 0.008, correlation of zap with hematological and clinical parameter in ALL, no significant \u0000correlation except positively correlated with leucocytic count while in CLL highly significant correlation \u0000with splenomegally but not with lymphadenopathy, strong association between zap-70 and binnet staging \u0000but not with CD38. Overall survival of zap-70 in ALL/CLL show that short free survival associated with \u0000positive zap-70. ZAP-70 has raised great interest in CLL patients because it represents a powerful \u0000prognostic marker. Conclusion: Our results provide preliminary indication on the potential use of this \u0000protein as a prognostic marker also in ALL. These results clearly show that ZAP-70 may indeed be a \u0000therapeutic target. Approach: 42 infertile couple underwent standard evaluation for infertility, woman given medication to stimulate development of mature follicles, washed semen specimen placed in uterine cavity using sterile flexible catheter, coincide with ovulation time. Results: Of 42 infertile couples pregnancy occur in 26(61.9%), (19.1%) in primary and (42.9%) in secondary infertility, (21.3%) of them was male infertility and (40.6%) of unexplained infertility. Pregnancy was less with prolong duration of infertility (35.7%) with 3-5 years, (21.3%) with 5-7 years and (4.8%) with 8 and more years of infertility. There is a wide array of good \u0000options for their treatment and controversy over the best methods for many situations. Appropriate \u0000nonoperative and operative treatment of patients with humeral shaft fractures, however, requires an \u0000understanding of humeral anatomy, the fracture pattern and the patient’s activity level and expectations. Although good techniques of osteosynthesis are available, the aim of this article is to emphasize on the benefit and good outcome of conservative treatment for properly selected cases to decrease the cost and avoid the complications of surgery. Approach: During the period from Jan 2008 to Jun. 2009 seventy-eight fractures of humeral shaft were treated at Orthopaedic Department in the Tikrit Teaching hospital. 20 fractures considered suitable for the study. The patients treated conservatively by using the‘U’ shaped coaptation slab and the patients evaluated both clinically and radiologically every two weeks. If there is much pain or any degree of malalignment, we shift to POP cast. Then we follow the patient clinically and radiologically every 2-4 weeks and until the fracture had united and the limb functions were restored. The outcome of treatment was assessed by specific parameters which include alignment, rate of union and limb functions. Results: This study showed that the initial deformities of angulation were considerably reduced by the use of U slab and the POP cast which act as a dynamic rather than a static splint, correcting angulation to less than 30° in coronal plane and less than 20° in sagital plane. Manipulation of the fracture was not required and did affect neither the rate of union nor the final position, as the cast appeared to be capable of correcting angulation deformities. Perfect anatomical reduction was found not to be essential for satisfactory limb function, which was present with virus angulation and posterior bowing. The incidence of delayed union compares favorably with other reported series, although the definition of delayed union is variable. For this purpose, individual data of the involved patient should be analyzed and modeled in a micro sense for the physician to notice whether the treatment was helping the particular patient as demonstrated in this article. Understandably, a medical treatment would work in some patients but not in all patients. The physician would be more helped to know whether the treatment worked in a patient. Otherwise, the physician might switch to another treatment for the patient. No appropriate methodology existed in the literature to perform such a profile analysis. Hence, this article introduced a new statistical methodology and demonstrated the methodology using epileptic data. Approach: A probabilistic approach was necessary, as the number of epilepsy seizure in a patient happened to involve a degree of uncertainty. In some patient, the chance for a large number of seizures might be more depending on his/her proneness. The proneness would be a latent and non-measurable factor and hence, it could be captured only as a parameter. The traditional Poisson distribution was not suitable as it assumed homogeneous patients with respect to the proneness. The probability model should match the reality. A generalized Poisson model with an additional parameter to describe individual patient’s proneness was necessary as the article demonstrated. The author introduced such a model and investigated several statistical properties before in another article A new methodology with that probability was devised in this article for assessing the efficacy of a treatment for a chosen patient in epilepsy study. Results: Physicians pondered over whether epilepsy seizure incidences data support their hunch that their treatment was successful for a patient. This kind of case-by-case profiling was necessary to exercise the option of switching to another treatment for the patient. Aggregated medical data analysis of all patients did not help in making decision for a particular patient. The results of this article demonstrated about how the new methodology worked in epilepsy data to confirm when the treatment was successful. Patients, nurses and physicians were eager to develop an early warning system about how successful the treatment was in a patient. Such an early warning system was feasible, after finding the probability pattern of the data, because of the new methodology in this article. The discussions in this article could be emulated for other medical data analysis to address patient’s profile. Conclusions/Recommendations: As demonstrated with an example using epilepsy data, other medical data could be fit, analyzed and interpreted using the incidence rate restricted Poisson model. Many sources are associated with areas where human impacts are well \u0000documented. Approach: Nonetheless, the largest and most active sources are located in truly remote \u0000areas where there is little or no human activity. Dust activity is extremely sensitive to many \u0000environmental parameters. Results: The persistent sources are located in the Northern Hemisphere, \u0000mainly in a broad “dust belt” that extends from the Meddle coast of North Africa over the Middle East. In this study we identify local environments enables us to identify those characteristics that are important for dust generation. However, if dust has an impact on climate, then the role of humans in the destabilization of soil surfaces takes on an added dimension. Conclusion/Recommendations: Threshold effects may be at work. Approach: The aim of this study is to assess the presence possibility of mutation in thrombophilic g enes (Factor V (FV) gene, Prothrombin (PT) gene and Methylenetetrahydrofolate Reductase (MTHFR) gene) in women with repeated IVF-embryo transfer failure with unknown causes. The study was performed on patients with two or more previously failed IVF-embryo transfer cycles with u nknown causes. Women who conceived spontaneously with no previous history of miscarria ge and women who have had successful pregnancy after their first IVF-embryo transfer cycle are inc luded as control groups. Results: There are increase in allelic frequencies of thrombophilic genes (fact or V, prothrombin and methylenetetrahydrofolate reductase) mutation, in addition to genotype freque ncies (both homozygote and heterozygote) and increase in frequency of multiple gene mutations am ong women with IVF- embryo transfer failure. Conclusion: Thrombophilia has a significant role in IVF- embry o transfer implantation failure. Majority of anti-HIV drugs target reverse transcriptase and protease enzymes. Toxicity and development of multidrug resistant HIV-1 virus strains are the reasons for studying new targets in HIV-1 replication process for identifying novel inhibitor with low toxicity and high activity. Approach: In this study, ROCS software was used to identify the novel HIV-1 Integrase (HIV-1 IN) inhibitor by shape-based virtual screening. The currently used drug raltegravir was used as query molecule. Results: Here five novel molecules were identified, among them Rank 5 molecule was shown to have higher structural and electrostatic similarity to query molecule and this molecule was considered as good inhibitor of HIV-1 IN enzyme. Approach: DNA extracted from AH of 12 TRC patients and 12 controls (patients with ocular inflammation of non-Toxoplasma origin) were subjected to all the 4 nPCRs. Results: Toxoplasma gondii genome was detected by atleast one of the 4 nPCRs in 8 (66%) of 12 TRC patients and in none of the 12 controls. nPCR targeting B1 gene (primer set 1) was positive in 6, B1 gene (primer set 2) in 2 and both 3'end of SAG2 and 5' end of SAG2 in 4 respectively. The sensitivity of B1 gene (primer I) was higher compared to the other 3 nPCRs (Yates correction-Chi square test; p = 0.028). Approach: In 2007, we carried out a cross-sectional study through convenience sampling of 131 TG aged > 18years, residing in Chennai at least prior to six months of the study. Sexual practices and perception towards STIs and HIV were identified through structured interviews. HIV and STIs were diagnosed from blood and urine samples. Associations among sexual practices, HIV/STI and alcohol use were analyzed using contingency tables. Results: The study participants had a mean age of sexual debut of 12.86 years. From the sample, 91% reported involvement with casual, paid and/or long-term sex partners at the time of the study and reported having anal sex in similar proportions across all three types of partners. More individuals failed to use condoms with long-term partners (44%) than with casual (17%) or paid (9%) partners. Alcohol use was more strongly associated with multiple casual and paid partners than with exclusive long-term partners (p<0.05). About 29% reported sex under the influence of alcohol. 72% were diagnosed with least one STI, with viral STIs ranging from 8-48 and 18% diagnosed with HIV. HIV-positive individuals could not be statistically distinguished from HIV-negative individuals on the basis of anal and oral sex, presence/absence of STI, inconsistent condom use or sex under the influence of alcohol. Conclusion/Recommendations: Given the extensive literature on associations between inconsistent condom use, unprotected anal sex, alcohol and HIV risk, the lack of difference between HIV-positive and negative individuals with respect to behaviors suggests that the entire sample represents a group needing primary or secondary prevention intervention.
