To be successful, we wanted it to be multidisciplinary and not to be limited to IMANA members as has been with other IMANA conferences. To achieve these two goals we agreed that it would be best to hold the conference jointly with an established medical institution and for it to be held in a metropolitan area. Dr. Faroque Khan, the vice chairman of the Ethics Committee and a resident of Long Island, New York, suggested the local Hofstra University to be the venue for the conference. The next major decision was to decide the theme of the conference. At the time there was a lot of heated debate between politicians and in the media about health-care reform. Part of these discussions involved end-of-life care. This is a very significant issue, more so for the immigrant community with their different cultural backgrounds and specifically with Muslims because of their religious beliefs. We felt a great need to explore these issues in depth and decided to have the theme End-of-Life Issues: Ethical and Religious Perspectives. Dr. Khan has good connections with leaders of other faith communities in the area and specifically with several faculty members of the newly established medical school that is a part of the well-recognized Hofstra University. We agreed with the suggestion. We contacted the university’s Muslim chaplain, Professor Mamdouh Farid, professor emeritus of management, entrepreneurship, and general business,. He was very enthusiastic about the project and assumed the responsibility of providing the space and the local arrangements. I contacted Professor Daniel Varisco, of Hofstra’s Anthropology Department, who has been working in Yemen. He showed great interest in the project and offered to recruit Hofstra faculty members for the conference. I asked him to co-chair the program, and he graciously agreed. The next step was to select the faculty. Several members of IMANA Ethics Committee – Drs. Malika Haque, Shahid Athar, Faroque Khan, and myself –agreed to participate. Other IMANA members with great interest in medical ethics – Drs. Hasan Shanawani and Aasim Padela – were asked to and agreed to participate. Other IMANA members who work in disciplines that involve end-of-life issues were asked to participate: Dr. Tanveer Mir in palliative care, and Dr. Ali Nadroo in neonatology. We then looked for religious leaders. I selected Dr. Gamal Badawi to represent the Islamic faith. Dr. Khan recruited Rabbi Jerome Davidson and Dr. Fredrick Smith to represent the Jewish and Christian faiths, respectively. I wanted to share a brief snapshot of the conference from my perspective. The conference started with a discussion on brain death. We found a lot of commonalities among the Abrahamic faiths, and the consensus was that brain death is an acceptable criterion for the vast majority of scholars of Jewish, Christian, and Islamic faiths.1 Dr. Aasim Padela presented a dissenting viewpoint.2 During the presentations and discussions all the presenters emphasized the importance of evaluating the quality of life as we deliberate and decide upon the many options available at the end of life. We heard about distributive justice and proportionality, which are important in decision making, particularly at the community level. We heard about deep-brain stimulation and how we are learning that some things that we took for granted are not really true. Dr. Tanveer Mir enlightened us regarding brain recovery after serious trauma, from complete recovery to minimal recovery.3 We heard about the applications of hospice care and its usage within the community.4 I particularly enjoyed Professor Varisco’s presentation on the anthropological view regarding grieving.5 This is one of the few meetings I have attended where we have nonmedical speakers, and that broadens and enriches the conversation. Professor Varisco shared with us information dealing with the manner in which animals grieve for their dead. To learn about how chimpanzees and monkeys and elephants grieve for their deceased was informative and enlightening. \u0000 \u0000On the second day, the morning session on compassionate care was very enjoyable. I wish I had exposure to that session when I was a medical student. Dr. Stephan Post’s presentation and the subsequent discussions by Professors Julie Byrne and Warren Frisina were really enlightening. We were presented with some hard evidence about the relation between emotion and health. When we are under stress, the lymphocytes go down which can be a set up for frequent colds etc. We also learned from Professor Post’s presentation that there is scientific evidence regarding the beneficial effect on the recovery process of a good upbeat mood.6 \u0000 \u0000The afternoon session on day two was an eye-opener for me. All the challenges which our colleagues in obstetrics and pediatrics are facing as the result of rapid advances of technologic and surgical techniques are mind-boggling. Dr. Fadel presented the variety of procedures to diagnose and, in some cases, repair fetal anomalies or defects while the fetus is still in utero. He also discussed the ethical implications of these procedures. Who would have thought just a few decades ago that one could operate and fix anomalies in-utero?7 Certainly the challenges that Dr. Ali Nadroo presented are worth pondering. Dr. Mohamed Sharawy was one of the handful of Muslims in Augusta, Georgia, in 1970, when he moved there. He was one of two Egyptian Americans and one of three foreign faculty members at the Medical College of Georgia at that time. Dr. Sharawy had a great influence on my decision to move to Augusta with my wife, Dr. Skina Fadel, and our two sons in 1975. He is the first dentist that we profile in JIMA, and I hope this will encourage other dentists to join our organization. Dr. Sharawy is professor and director of oral biology/anatomy and professor of oral and maxillofacial surgery at the College of Dental Medicine and professor of anatomy and cell biology at the Medical College of Georgia (MCG) and the College of Graduate Studies at the Georgia Health Sciences University (GHSU). Dr. Sharawy was born in Cairo, Egypt. He received his bachelor of dental surgery with honors (BDS) in 1962 and followed it by three years of oral and maxillofacial training. He received his diploma and science medallion in 1963 from Egyptian President Nasser for the highest achievement among graduating dentists. Dr. Sharawy received a Fulbright Scholarship to study for a doctor of philosophy in the United States. In 1970, he earned his degree in anatomical sciences from the School of Medicine and Dentistry, University of Rochester, Rochester, New York. Dr. Sharawy joined the faculty of the School of Dentistry at MCG in Augusta in 1970 and became one of the initiators and developers of the new school. During the next 40 years, in addition to teaching undergraduate students and consistently working on extramurally funded research, he became deeply interested in international advanced education. Dr. Sharawy was one of the developers of joint supervision programs for dentists in Egypt and other Middle Eastern countries. In this program, a promising dentist would register for a doctor of philosophy in his country and then choose two advisors, one from his home country and the other from the United States. After passing graduate courses and qualifying examinations in his native country, the candidate would join a research laboratory for two years at MCG, now renamed GHSU. Then, the doctorial candidate would write a thesis and go back to his home country to defend it in the presence of the foreign advisor. Dr. Sharawy served as a mentor and major advisor for more than 37 dentists from Egypt. The research projects often addressed questions related to bone, local growth factors, implants, and the temporomandibular joint (TMJ). The research papers were published in dental and medical literature with the student names often listed first. These graduated dentists are now professors at their respective universities in Egypt and are serving as doctorial advisors. {"title":"Nutritional guidelines for school lunch programs: a survey of islamic schools and recommendations for creating a culture of healthful eating.","authors":"Sumiya Khan, Ziena Saeed, Hanifa Hameed Diwan, Iqra Hussain, Sarah Amer, Mohamed M Haq","doi":"10.5915/43-2-8279","DOIUrl":"10.5915/43-2-8279","url":null,"abstract":"<p><strong>Objective: </strong>To determine the status of lunch programs in Islamic schools in the United States and develop recommendations for improving them.</p><p><strong>Study design: </strong>The Islamic Medical Association of North America (IMANA) conducted a survey of lunch programs by mailing questionnaires to 100 Islamic schools in the United States. Muslims in Dietetics and Nutrition (MIDAN) developed lunch menus using American and ethnic foods conforming to nationally recommended guidelines.</p><p><strong>Results: </strong>Forty-eight Islamic schools responded to the survey, revealing that 20 schools follow guidelines and only six have dietitians advising on menu planning. Based on this survey, IMANA, with the assistance of MIDAN, has developed a summary of guidelines that schools can follow. These guidelines include sample menus of American and ethnic foods, recommendations for creating a n environment for healthful eating, and sources for funding school lunch programs.</p><p><strong>Conclusions: </strong>IMANA and MIDAN, recognizing the scientific significance and religious relevance of a nutritious diet, have developed these recommendations. The clinical specimens were collected from April 2007 to June 2008 and included sputum (247), bronchial wash (80), and blood (300) samples. The identification process employed direct examination, culture, conversion test, and serological study. Among 218 fungal isolates only six were categorized as true pathogenic fungi; two Histoplasma capsulatum, and four Blastomyces dermatitidis. A total of 162 houses and 670 subjects were observed during the study period. One hundred forty-two febrile patients were eligible for the study. After obtaining informed consent from all febrile patients, 140 blood samples were collected to diagnose malaria, dengue, and chikungunya. Larval samples were collected by the standard protocol that follows. Correlation of data was performed by Pearson correlation test.</p><p><strong>Results: </strong>Forty-seven blood samples were found positive: 33 for chikungunya, 3 for dengue, and 11 for malaria. Fifty-one out of 224 larval samples were found positive. It responds favorably to radiotherapy with or without chemotherapy depending on the stage of disease. Recurrence is local or locoregional. In her 2010 book, The Immortal Life of Henrietta Lacks, Rebecca Skloot documents the histories of both the cell line—called the HeLa cell line after the first two letters of her first and last names to protect her identity—and the Lacks family. Henrietta's husband , David Lacks, was told little following her death. Suspicions fueled by racial issues prevalent in the South at the time were compounded by issues of class and education. Members of the Lacks family were kept in the dark about the existence of the tissue line, and when its existence was revealed in a 1976 Rolling Stone article by Michael Rogers, family members were confused about how Henrietta's cells could have been taken without consent and how they could still be alive 25 years after her death. Skloot's book takes the reader on an incredible journey from the \" colored \" ward of Johns Hopkins Hospital in the 1950s to the research laboratories with freezers full of HeLa cells, to Henrietta's small, dying town of Clover, Virginia, to east Baltimore, where Henrietta's children and grandchildren live. The basic facts about the story of Henrietta Lacks are well documented. On February 1, 1951, Ms. Lacks visited Johns Hopkins because of a painful \" knot \" in her cervix and bloody vaginal discharge. After a biopsy, she was diagnosed with cervical cancer. The appearance of the tumor was unlike anything the examining gynecologist, Dr. Howard Jones, had seen. Prior to the treatment for the carcinoma, cells from the tumor were removed for research purposes without her knowledge or permission, which was standard procedure at that time. During her second visit eight days later, Dr. George Otto Gey obtained another sample of her tumor. These cells would eventually become the HeLa immortal cell line. In significant pain and without improvement, Lacks returned to Johns Hopkins Hospital on August 8 demanding admission and remained there until her death on October 4 at the age of 31. A subsequent partial autopsy showed that the cancer had metastasized throughout her body.
