Potyazhenko","doi":"10.24061/1727-4338.xx.2.76.2021.9","DOIUrl":"https://doi.org/10.24061/1727-4338.xx.2.76.2021.9","url":null,"abstract":"The issues of clinical significance and possibilities of using the assessment of spectralanalysis indicators of a short recording of heart rate variability for assessing thefunctional state of patients in accordance with the stages of the cardiovascularcontinuum in non-communicable diseases are described in the article.The aim of the study – to carry out a comparative assessment of the indicators of ashort recording of heart rate variability in respondents with different stages of thecardiovascular continuum in order to increase the effectiveness of measures for theprevention and treatment of non-communicable diseases in Ukraine by improvingtheir diagnosis and prevention through the introduction of modern science-intensivetechnologies into medical practice.Material and methods. 307 respondents were surveyed using the Poly-Spectrumhardware-software complex (Neurosoft, Russia). All of them underwent registrationand analysis of a short recording of heart rate variability. 200 comorbid patients withnon-communicable diseases were divided into four subgroups depending on the stage ofthe cardiovascular continuum and the degree of comorbidity. 107 functionally healthyindividuals (control group) were divided into two subgroups depending on the level ofsports training.Results. A decrease in the electromagnetic power (total and all spectral components)was found in patients with non-communicable diseases, depending on the stage of thecardiovascular continuum. The results of personalized analysis of spectral parametersin all study groups were presented and their medical-clinical interpretation was written.Conclusion. Non-communicable diseases are accompanied by a gradual decreasein the electromagnetic power (total and all spectral components) of cardiac activityin accordance with an increase in the degree of comorbidity and progression of thecardiovascular continuum. The study involved 90 patients with IBS (30 men and 60women aged 22 to 56 years). The polymorphic variant of the CD 14 gene (C-159T)was analyzed by polymerase chain reaction in 90 patients with IBS without and withconcomitant obesity and 30 people in the comparison group. Blood levels of C-reactiveprotein (CRP), tumor necrosis factor α (TNFα), transforming growth factor β1(TGFβ1), interleukin-10 (IL-10), 8-isoprostane, ceruloplasmin, medium molecules andcalprotectin levels in feces were determined.Results. In patients with IBS and obesity, the frequency of TT genotype (36.7%) washigher compared to healthy subjects (TT genotype – 13.3%). Significantly higherserum levels of CRP (3.5 times and 26.7%), TNFα (1.7 times and 19.5%), TGFβ1(29.8% and 19.2%), 8-isoprostane (54.1% and 31.9%), ceruloplasmin (56.7% and33.0%), medium molecules (7, 5% and 12.9%) and calprotectin (55.7% and 37.4%) inthe feces compared to the CC and CT genotype have been determined in patients withIBS, combined with obesity, TT genotype with a predominance of diarrhea.Conclusions. The association of a polymorphic variant of the CD14 (C-159T) gene withthe risk of IBS development in obese patients has been established. Material and methods. 104 male patients with neurological manifestations ofosteochondrosis of the lumbar spine at the age of 25-45 years (mean age 34.05 ± 5.7)without obesity, somatic and vascular pathology and 25 practically healthy persons,who did not differ in age from groups of the patients under study, have been examined.Among the examined patients, there were 50 patients with radiculopathies against abackground of herniated intervertebral discs, verified by means of neuroimaging, and54 patients with reflex manifestations of osteochondrosis of the lumbar part of the spine.The study of the clinical picture of osteochondrosis neurological manifestations of thelumbar spine was carried out by the method of standard neurological examinationusing a visual- analogue pain scale.The state of the autonomic nervous system was assessed using standard tests forautonomic reactivity and autonomic provision of activity, as well as using theQuestionnaire to identify signs of vegetative changes and the Study Scheme to identifysigns of autonomic disorders (O.M. Wein, 1993).The study of cerebral circulation was carried out by the method of extra-intracranialDoppler ultrasonography according to the standard techniques on the Sonomed-350apparatus using the Overshut test for cerebrovascular reactivity.Concentration of fibronectin in the blood plasma, tumor necrosis factor alpha andsoluble fms- similar tyrosine kinase was investigated by enzyme immunoassay accordingto the manufacturer's procedure under laboratory conditions.The paired Student's test and Fisher's angular Phi-transformation are used.Results. Statistically significant decrease of the pain syndrome has been revealedaccording to the VAS scale in the group of patients, who took Glutargin in the complextreatment. Significant changes in autonomic homeostasis have not been revealed whenusing Glutargin. Surgical treatment of stratified aortic aneurysms of type Aremains one of the most difficult problems in cardiac surgery. The article presentsall experience (947 stratifying aneurysms of type A) of surgical treatment of suchaneurysms by one team. Factors that could have caused the formation of an aorticaneurysm (aortic dissection) have been identified.Results. The total hospital mortality during surgical treatment of a stratified aorticaneurysm type A was 9.7%, in addition to that – 10.1% in the acute stage and 8.4% inthe chronic stage.Conclusions. The analysis of surgical treatment of 508 patients with IVH from2009 to 2020 was conducted. According to the Europenian Herniology Association(EGA) classification (Ghent, Belgium, 2008) IVH was distributed as follows: MW1-2R0 was diagnosed in 217 (42,7%), MW3R0 – in 291 (57,3%) patients. Diastasis of therectus abdominis muscles up to 5 cm was present in 217 (42,7%) patients, diastasis5-10 cm – in 127 (25%), diastasis greater than 10 cm – in 164 (32.3%) patients.Depending on the size of the hernia and the width of diastasis of the rectus abdominis,patients were divided into 3 groups.In group I, laparoscopic allohernioplasty was performed in 109 (21,5%) patients withsmall and medium-sized IVH with diastasis of up to 5 cm, in particular the developedlaparoscopic preperitoneal in 63 patiens and laparoscopuc retromuscular alloplastiesin 46 patients. The comparison group IIa consisted of 108 (15,1%) patients whounderwent open retromuscular allohernioplasty.In group II, 64 (12,6%) patients with large IVH and diastasis of the rectus abdominis5-10 cm underwent open allohernioplasty by «sublay» technique. The comparison groupIIa consisted of 63 (12,4%) patients who were performed the open method «onlay».In group III, in 82 (16,1%) patients with giant IVH and diastasis more than 10 cm ananterior component separation technique of the abdominal wall in combination withalloplasty with intra-abdominal placement of a mesh implant with anti-adhesive coatingwas performed according to the developed method. Comparison group IIIa consistedof 82 (16,1%) patients who underwent anterior component separation technique of theabdominal wall in combination with alloplasty «onlay».Results. Shorikov","doi":"10.24061/1727-4338.xx.2.76.2021.7","DOIUrl":"https://doi.org/10.24061/1727-4338.xx.2.76.2021.7","url":null,"abstract":"The purpose of the study – to assess the disorders’ degree of endothelium dysfunction inpatients with nonspecific ulcerative colitis (NUC) and Crohn's disease (CD), dependingon the severity of inflammation.Material and methods. 34 patients with NUC and 18 people with CD with continuousrecurrent courses, 15 patients with NUC and 15 patients with CD in remission wereobserved. The control group included 30 healthy volunteers.Results. It has been established that patients with active NUC had a significantly higherlevel of markers characterizing changes of proliferative (VEGF-165) and adhesive(VCAM-1) endothelial functions vs. control (in 4.62 and 2.01 times) and patients whohad an inactive disease (at 3.95 and 4.62) (p<0.05). In the active course of NUC, VEGF-165 level increased by 3.27 and 5.03 times, the level of VCAM by 1.83 and 2.09 times vs.inactive and control group (p<0.05).In patients with severe and moderate degrees of NUC, we have set an increase in theconcentration of VEGF-165 compared with mild degree, p<0.05. It has been proven thatin severe degree of CD, the concentration of VEGF-165 increased 21.5% vs. moderateactivity and 63.1% vs. mild activity, p<0.05.It has been established that adhesive phenotype of endothelial dysfunction with severeactivity of NUC was associated with an elevated content of adhesion molecule, dependingof activity, p<0.05. The adhesive phenotype of endothelial dysfunction is proved forpatients with severe activity of CD compared with moderate and mild activity, p<0.05.We have revealed a reliable direct regression relationship between the concentration ofthe vascular endothelial growth factor and the activity of UC according to the TrueloveWitts scale (R=0.29, p<0.05), between the CDAI index level in CD and the content ofVCAM-1 (R=0.29, p<0.05).Conclusions. Changes of excretory, water- and ion-regulating functions of thekidneys following a single and long trenthal and xanthinol nicotinate administration atdoses of 3mg/kg under conditions of salt loading with NaCl solution 0,45% were studiedin the experiments on non-linear white rats.Results. Both the rapid response of the kidneys after a single injection of trenthaland xanthinol nicotinate and more significant changes of the renal function followingrepeated (7 days) drugs’ administration under conditions of salt hydration with NaCl0.45% solution were observed. Comparative estimation of the renal action at a singleadministration has shown that xanthinol nicotinate activates water-, ion-regulating andexcretory function of kidneys more significantly than trenthal. After xanthinol nicotinateadministration the urine output in rats increased 74%, natriuresis - 66%, creatinineexcretion – 52% in comparison with the control, while following trenthal administrationthe indices under study increased only 62%, 33% and 34% correspondingly.Under conditions of salt loading water-regulating function of kidneys activated moresignificantly following durable (7 days) administration than after a single introduction ofpreparations. Under trenthal influence diuresis increased 1.8 times, xanthinol nicotinate– 1.9 times in comparison with the control. Trenthal increased endogenous creatinineexcretion – 1.4 times and xanthinol nicotinate – 1.6 times.Natriuresis increased almost 2 times, trenthal - 1.4 times under xanthinol nicotinateinfluence. The natriuretic effect of xanthinol nicotinate is 55% higher than that oftrenthal. Positive correlative relation has been established between increased urinationand natriuresis only after durable xanthinol nicotinate administration (r=0,784, p<0,05).Trenthal appeared to be more pronounced kaliurretic.Conclusions. Excretory, water- and ion-regulating renal function is activated following asingle and protracted (7 days) introduction of methylxanthine derivatives – trenthal andxanthinol nicotinate at a dose of 3mg/kg under hydration conditions with NaCl solution0,45%. Renal effects of the preparations under study is characterized by an increaseof endogenous creatinine excretion. Fifteen experimental wound canals were reconstructedby means of an alginate imprinting mass with a rubber-like effect \"Hydrogum 5\"(Zhermack, Italy), which hardens quickly, remains elastic after polymerization, allowsto obtain imprints with an extremely smooth surface andmost accurately preserves andreproduces the properties of the immersed in it blade of the knife under study. Astabcutting tool - a knife with one-sided sharpening of the blade, blade length - 9.6 cm,width in the place of the greatest blade thickening - 2.6 cm, butt thickness - 0.24 cmwas used to mark experimental damage. Given dimensions of the stab- cutting toolwere obtained by means of sliding caliper with an error of ± 0.03-0.15 cm. The formedwound canal was divided into fragments with a step of about 3.5 cm. Each fragmentof the wound canal was contrasted with dye (1% alcohol solution of diamond green).These fragments of the wound canal were opened parallel to its length and placed on arotating table, which was located in a light cube to provide sufficient illumination andphotography was performed. A SONY RX 10 II digital camera was used for taking aphotograph. The index number and a fragment of a scale ruler 1.0 cm long were placedon the object for further calibration of the scale and control of the dimensions of theobject under study in computer programs. The photos obtained in JPEG format wereloaded to the computer program \"Agisoft Photoscan\", where 3D textured models of thewound canal fragment were created. The obtained model and texture were exportedin OBJ format. Then the obtained 3D models were moved to the graphic space of theprogram \"3ds max\" to calibrate the scale of the model and in the graphic editor thereconstruction of the wound canal was carried by means of 3D models of its fragments.Results. Measurements of the linear dimentions of damages were carried out at firstby a classical method (by means of a ruler). In addition to, that the depth of the woundcanal, which consists of three fragments, was obtained according to the method ofmeasuring the depth of the wound canal in the body of the corpse by putting togetherits seperate parts in accordance with immersion and permeability of the blade in thevictim's body (skin, subcutaneous muscle tissue, cavity wall and in it itself, the internalorgan and other anatomical formations). The study was carried out on the basis of the maternitydepartment for pregnants and parturient women with obstetric pathology of the StateInstitution “Institute of Pediatrics of Obstetrics and Gynecology named after V.I. acad.EAT. Lukyanova National Academy of Medical Sciences of Ukraine \". The first groupincluded 45 pregnant women who underwent a course of psychophysical training withpositive motivation for partner childbirth. The second group consisted of 45 pregnantwomen, also underwent a course of psychophysical training with positive motivation forpartner labor, but had a desire not to carry out pain relief during childbirth, therefore,they chose training using the method of transcranial electrical stimulation (TES) of brainstructures. All pregnant women underwent a determination of their psychoemotional stateto define the level of situational anxiety according to the method of Ch.D. Spielberger andYu.L. Khanin. The second group of pregnant women was tested after the fourth EFTprocedure and again the day before delivery. The threshold of pain sensitivity of apregnant woman in each group was also determined. Subjective assessment was carriedout for women in labor on the first day after delivery and using a verbal descriptivescale for assessing pain (Verbal Descriptor Scale, 1990). To assess the condition of thefetus during childbirth, cardiotocographic studies were carried out, and after birth – anassessment on the Apgar scale at the first and fifth minutes.Results. In women who underwent a course of psychophysical training with positivemotivation for partner childbirth together with the method of transcranial electricalstimulation, physiological childbirth proceeded 1.2 times more often than in womenpatients who did not undergo prenatal training (88.9 % versus 75.6 %, respectively).Allchildbirth, occurred in the second group, began on their own and did not require laborinduction, but in the first group this indicator was 6.3% of cases.Conclusions. The study is based on the clinical observations of 18 patientswith non-alcoholic steatohepatitis accompanied by hyperferritinemia and concomitantobesity. Each patient was undergoing the basic therapy as well as the sessions of discreteplasmapheresis for 10-14 days, which led to the removal of 17-20% of the volume ofcirculating plasma during each procedure, the total of which amounts to about 1-1.5 ofthe circulating plasma volume.Results. As a result of the two-week treatment, the patients' well-being improvedsignificantly, their efficiency increased, and previously elevated levels of ferritin (1.29times in men and 1.21 in women) and serum iron (2.7 times) decreased. The initiallyelevated level of total cholesterol also decreased significantly (2.34 times), and due toVLDL (7.86 times) and triacylglycerols – 9.78 times. The atherogenic factor decreasedfrom 3.8 to 1.5. (2.5 times). The sizeable decrease was observed in the uric acid content– 1.77 times, ALT activity – 1.55 times, LDH – 1.77 times, LF – 1.42 times, GGTP – 4.0times (p <0.05).Conclusions.
