Consequently, understanding the mechanisms by which trauma/hemorrhagic shock (T/HS) leads to acute (non-infectious) MODS is of major health importance in the USA. One of the major hypotheses being studied to explain the development of sepsis and MODS after trauma is the gut hypothesis of MODS. Additionally, there is recent experimental and clinical information that the response to injury and sepsis may differ between males and females. Thus, the overall global hypothesis of this review is that T/HS-induced early distant organ injury and cellular dysfunction is secondary to gut injury and is primarily mediated by factors exiting the gut via the mesenteric lymphatics. Among these enzymes, type IIA secreted PLA2 (sPLA2-IIA) is secreted by various inflammatory cells, including macrophages and neutrophils, and has been suggested to play a role in the pathogenesis of a number of inflammatory diseases. sPLA2-IIA is involved in the release of arachidonic acid, the precursor of proinflammatory eicosanoids, and has been found to bind to specific receptors located on cell surface membranes. However, the most pertinent roles of this enzyme are its ability to kill bacteria and to hydrolyze pulmonary surfactant phospholipids. Inflammatory pulmonary edema and alveolar collapse are central to ARDS. Numerous redundant mechanisms are involved which include oxidants neutrophils products and other inflammatory mediators. Surfactant is damaged or destroyed. Supportive treatment with low tidal volume ventilation and positive end expiratory pressure is the standard of care. No specific pharmacologic agent has been shown to alter the course of disease. Survivors may have varying degrees of pulmonary insufficiency, or may be normal. An additional method is based upon dead space analysis. Tomographic ventilation/perfusion scintigraphy, V/PSPECT is superior to planar technique. Important is interpretation criteria based upon pattern recognition and clinical information. With optimal strategy, the rate of non-diagnostic findings is only about 1%. The sensitivity of CT is too low to exclude subsegmental PE,. The radiation dose is for CT is several times higher than for V/PSPECT. Quantification of PE, only offered by V/PSPECT has impact on therapy and is vital for follow up. Limited availability of V/PSPECT makes CT an essential element in a strategy for diagnosis of PE. The epithelium is capable of directly interacting with intestinal pathogens such as Salmonella species, resulting in a release of cytokines beneath the epithelium that likely play a role in sepsis. More directly, the epithelium expresses Toll-like receptors so that it can directly respond to luminal lipopolysaccharide. Breaching of the epithelial barrier is likely the principal cause of sepsis in patients with compromised intestinal circulation. However, a number of reparative mechanisms have developed to rapidly repair epithelial defects, including epithelial migration (restitution) and closure of injured tight junctions. However, a variety of diseases, such as acute pancreatitis (AP), may disturb this balance. A limited number of studies on exocrine function in the acute phase of pancreatitis have been performed, most of them in animals. In the convalescent phase of AP, a substantial number of patients will have abnormal results of pancreatic function tests. Many patients with AP develop temporary hyperglycemia requiring insulin treatment and some will end up requiring long-term insulin therapy. The aim of this review is to summarize current knowledge of exocrine and endocrine dysfunction of the pancreas during both the acute and recovery phases of AP. Although no effective pharmacologic interventions have reached the clinic, use of a low-ventilation strategy is of clear benefit. Surfactant replacement continues to be a topic of study, while use of nitric oxide, prone positioning, extracorporeal gas exchange, and high-frequency oscillatory ventilation have not been shown, in prospective randomized studies, to provide added benefit. Measures such as rigorous control of blood glucose levels and glucocorticoid replacement have a well-established role in certain settings. So-called “goblet (mucous) cell metaplasia/hyperplasia” is the pathologic feature in these diseased airways, in which the normal mucociliary epithelium is replaced by goblet/mucous cells. The nature of the goblet/mucous cell population which arises in these diseased airways is unknown. Our recent studies have shown that trans-differentiation of surface epithelial cells occurs so that they express the submucosal gland-type mucin gene, MUC5B, in addition to a general elevation of all mucin gene products in surface epithelial cells as well as in the submucosal area.
