{"title":"Sighting Dominance, Biometric Parameters, and Refractive Status Analyzing the Role of Ocular Dominance.","authors":"Roshni Robert,&nbsp;Mahesh Babu,&nbsp;Krishnendu Unnikannan","doi":"10.14744/bej.2023.46504","DOIUrl":"https://doi.org/10.14744/bej.2023.46504","url":null,"abstract":"<p><strong>Objectives: </strong>The purpose was to study the association between ocular dominance, refractive status, and biometric parameters.</p><p><strong>Methods: </strong>Ocular dominance was assessed on consenting participants with non-pathological eyes using \"hole-in-the-card test.\" The participants were then examined for visual acuity, biometric measurements, and refraction. Data were analyzed using IBM SPSS software.</p><p><strong>Results: </strong>Among a total of 660 participants in our study, right eye dominance was found in 508 (76.97%) participants. We found that horizontal keratometry readings (K1) were greater in the dominant eye compared to the non-dominant eye, showing a statistically significant difference in emmetropes (p<0.001) and hyperopes (p<0.001). The axial length was found to be longer but not significantly greater in dominant eye among while it was significant among myopes (p<0.001) and hyperopes (p<0.001). In myopic anisometropes, the axial length was significantly longer and more myopic in the dominant eye (24.0±0.7 mm) than non-dominant eye (23.9±0.4 mm) while the non-dominant eye was more hyperopic in anisometropic hyperopes.</p><p><strong>Conclusion: </strong>Right eye was dominant in majority of participants. The dominant eye was more myopic and had greater axial length in anisometropes. The dominant eye was more astigmatic than the non-dominant eye. Visual acuity was not affected by ocular dominance. The mean difference in biometric measurements was significantly greater in hyperopic eyes. The assessment of ocular dominance could improve patient satisfaction in refractive surgeries and monovision treatments. Treatment protocols could be fine-tuned based on ocular dominance. {"title":"Overview of Epiphora Referred to Oculoplastic Surgery Clinic in Adults.","authors":"Kubra Serbest Ceylanoglu,&nbsp;Atakan Acar,&nbsp;Emine Sen","doi":"10.14744/bej.2023.38980","DOIUrl":"https://doi.org/10.14744/bej.2023.38980","url":null,"abstract":"<p><strong>Objectives: </strong>The aim of the study was to evaluate etiological and demographic characteristics of the adult patients referred to the oculoplastic surgery clinic of the tertiary care center with the complaint of epiphora.</p><p><strong>Methods: </strong>The medical records of the patients who applied to the oculoplastic surgery clinic with a complaint of epiphora between January 2014 and July 2021 were reviewed retrospectively. Etiology of epiphora, age, gender, duration of symptom, and follow-up period were evaluated. According to the etiological factors, nasolacrimal system disorders such as punctal stenosis, canalicular stenosis, canaliculitis, and acquired nasolacrimal system obstruction, respectively; the causes of epiphora were grouped as eyelid abnormalities such as entropion and ectropion, and hypersecretory tear secretion due to causes such as dry eye, allergy, and inflammation. The patients with epiphora over the age of 18 with at least 6 months of follow-up were included in the study. Patients with congenital or tumor-related nasolacrimal duct obstruction (NLDO) and epiphora due to trauma-related eyelid or canaliculi injury were not included.</p><p><strong>Results: </strong>A total of 595 medical fields were evaluated. Epiphora was present in 747 eyes of 595 patients. Of the patients, 221 (37%) were male and 376 (63%) were female. According to etiological evaluation of frequency, 372 (62.5%, 432 eyes) patients with NLDO, 63 (10.5%, 123 eyes) patients with punctal stenosis, 44 (7.3%) patients with ectropion, 38 (6.3%) patients with entropion, 37 (6.2%, 69 eyes) patients hypersecretory causes (dry eye, allergy, inflammation, etc.), 24 (4%) patients had primary canaliculitis, and 17 (2.8%) patients had epiphora due to canalicular occlusion.</p><p><strong>Conclusion: </strong>Epiphora is an important complaint that may occur due to different etiologies. A 50-year-old male patient reported a 1-week history of painless vision loss in the right eye in additon to complaining of blurred vision in the left eye 4 weeks earlier. He had tested COVID-19 positive 4 weeks before the onset of symptoms in his left eye. Further investigations revealed that the most possible cause of vision loss was NA-AION associated with COVID-19. COVID-19 infection may be responsible for NA-AION. Age, existence of amblyopia, pre-operative and postoperatively fusion ability, stereoacuity, and deviation angle were recorded. Patients were divided into two groups according to final stereoacuity; 200 sn/arc and lower: Good stereopsis (Group 1), upper 200 sn/arc: Poor stereopsis (Group 2). Characteristics were compared between groups.</p><p><strong>Results: </strong>A total of 49 patients, who were 16-56 years of age, were included in the study. The mean follow-up time was 37.8 months (range 12-72 months). Of patients, 26 had improvement in stereopsis scores after surgery (53.0%). Group 1 includes 200 sn/arc and lower (n=18, 36.7%) and Group 2 includes higher than 200 sn/arc (n=31, 63.3%). The presence of amblyopia and higher refraction error was frequent significantly in Group 2 (p=0.01 and p=0.02, respectively). The existence of fusion postoperatively was significantly frequent in Group 1 (p=0.02). Type of strabismus and the amount of deviation angle were not found in a relationship with good stereopsis.</p><p><strong>Conclusion: </strong>In adults, surgical correction of horizontal deviation improves stereoacuity. The patients underwent a full ophthalmologic examination including the best corrected visual acuity, IOP measured by Goldmann applanation tonometry, biomicroscopy, and dilated fundus examination before PRP. Aqueous flare values were measured by the laser flare meter. Aqueous flare and IOP values were repeated in both eyes at the 1<sup>st</sup> and 24<sup>th</sup> h after PRP. The eyes of the patients who underwent PRP were included in the study as the study group, and the other eyes as the control group.</p><p><strong>Results: </strong>In eyes treated with PRP, 1<sup>st</sup> h (19.44 pc/ms) and 24<sup>th</sup> h (18.53 pc/ms) aqueous flare values were statistically higher than before PRP (16.66 pc/ms) (p<0.05). In the study eyes which were similar to the control eyes before PRP, the aqueous flare was higher at the 1<sup>st</sup> and 24<sup>th</sup> h after PRP compared to control eyes (p<0.05). The mean IOP at the 1<sup>st</sup> h (18.69 mmHg) after PRP in study eyes was higher than both pre-PRP (16.25 mmHg) and post-PRP 24<sup>th</sup> h (16.12 mmHg) IOP values (p<0.001). At the same time, the IOP value at the 1<sup>st</sup> h after PRP was higher than the control eyes (p=0.001). No correlation was observed between aqueous flare and IOP values.</p><p><strong>Conclusion: </strong>An increase in aqueous flare and IOP values was observed after PRP. Besides, the increase in both values starts even in the 1<sup>st</sup> h, and the values at 1<sup>st</sup> h are the highest values. At the 24<sup>th</sup> h, while IOP values return to baseline, aqueous flare values are still high. In patients who may develop severe intraocular inflammation or cannot tolerate increased IOP (such as previous uveitis, neovascular glaucoma, or severe glaucoma), control should be performed at the 1<sup>st</sup> h after PRP to prevent irreversible complications. Anemic retinopathy (AR) was present if hemorrhages in different layers of the retina, Roth's spots, exudates, cotton wool spots, retinal edema, and optic disc changes were noted. Patients with secondary anemia were excluded. Two groups were identified: Group 1: Patients with AR; Group 2: Patients with no AR with normal fundus. Demographic features, fundus findings, and hematological parameters were noted.</p><p><strong>Results: </strong>38 (30%) eyes of 23 patients had AR; 90 (70%) eyes of 47 patients had no AR. The mean age in Group 1 and Group 2 was 54.7±18.9 and 58.6±15.3 years, respectively. Low hemoglobin (Hb) [mean = 7.82±1.64 gm/dl, p=<0.001] and hematocrit [mean = 26.93±5.22 %, p=0.002] values were noted in the AR group. AUC values for Hb (0.737) and hematocrit (0.719) were higher for the AR group. Hb < 8.95g/dl could predict AR with 85.8% sensitivity and 68.9% specificity, and hematocrit < 30.5% could predict AR with 80% sensitivity and 53.2% specificity. On regression analysis, Hb (p<0.001) and hematocrit (p=0.000) were associated with AR.</p><p><strong>Conclusion: </strong>Retinopathy is frequently seen in severe anemic patients. The difference between preoperative and postoperative 3rd month values of manually measured margin reflex distance 1 (MRD1), digitally measured eyelid and ocular surface parameters (MRD1, MRD 2 [MRD2], upper eyelid crease height [ECH], pretarsal show height [PTH], eyebrow line-height [EBH] and OSA), and VF parameters were evaluated. The correlation between preoperative and postoperative values of manually and digitally measured MRD1 and also preoperative and postoperative values of OSA and VF parameters were analyzed.</p><p><strong>Results: </strong>Thirty-six eyes from 36 patients were included in this study and the mean age of patients was 57.93±7.64 years. There were statistically significant changes between preoperative and postoperative values in means of the manually measured MRD1 and the digitally measured MRD1, PTH, OSA, and ECH (p<0.001). However, the postoperative changes in the mean MRD2 and EBH were not statistically significant (p=0.664 and p=0.983). There were moderate positive correlations between pre- and post-operative OSA values and pre- and postoperative values of manual and digital MRD1. A statistically significant agreement was observed between the change in OSA and the change in all VF parameters (Bland-Altman analysis test).</p><p><strong>Conclusion: </strong>Digital measurements can be used to evaluate the changes in eyelid and ocular surface parameters in patients who underwent upper eyelid blepharoplasty surgery. All scans were taken between 9.30 am and 11.30 am to avoid the diurnal variation of CT and CVI. To calculate CVI, macular SD-OCT scans were binarized using the publicly available software ImageJ and luminal area and total choroidal area (TCA) were measured. CVI was calculated as the proportion of LA to TCA. Furthermore, the relation between CVI and axial length, gender, and age was evaluated.</p><p><strong>Results: </strong>This study included 78 individuals with a mean age of 51.4±7.3 years. Group 1 consisted of 44 patients with inactive stage TAO, and Group 2 consisted of 34 healthy controls. Subfoveal CT was 338.92±73.93 µm in Group 1 and 303.97±40.35 µm in Group 2 (p=0.174). Medical records of patients who were treated with intravitreal RAN or DEX implant were evaluated before and at the 1<sup>st</sup>, 3<sup>rd</sup>, and 6<sup>th</sup> months after the injection. Primary outcome measures were the change in best-corrected visual acuity (BCVA) and central retinal thickness. The level of statistical significance was set at 0.05/3=0.016, according to the Bonferroni correction.</p><p><strong>Results: </strong>Thirty-nine eyes of 39 patients were included in the study. The mean age of the study population was 53.82±5.08 years. Median BCVA in the DEX group (n=23) at baseline, 1<sup>st</sup>, 3<sup>rd</sup>, and 6<sup>th</sup> month was 1.1, 0.80 (p=0.002), 0.70 (p=0.003), and 1 (p=0.018) logarithm of the minimum angle of resolution (log-MAR), respectively (p<0.05). Median BCVA in the RAN group (n=16) at baseline, 1<sup>st</sup>, 3<sup>rd</sup>, and 6<sup>th</sup> months was 0.90, 0.61, 0.52, and 0.46 logMAR, respectively (p<0.016 for all comparisons). Median central macular thickness (CMT) in the DEX group at baseline, 1<sup>st</sup>, 3rd, and 6th months was 515, 260, 248, and 367 μm, respectively (p<0.016 for all comparisons). Median CMT in the RAN group at baseline, 1<sup>st</sup>, 3<sup>rd</sup>, and 6<sup>th</sup> months was 432.5 (p<0.016), 275 (p<0.016), 246 (p<0.016), and 338 (p=0.148) μm.</p><p><strong>Conclusion: </strong>There is no significant difference in treatment efficacies in both visual and anatomical outcomes at the end of the 6th month. Infectious keratitis, persistent epithelial defects, corneal edema, or severe melt have been reported after CXL (1). Most of the complications can be managed easily; some may lead to severe visual loss, such as central toxic keratopathy (CTK). Herein, we present six eyes of four patients that underwent CXL for progressive KCN elsewhere and, then, were referred to Dokuz Eylül University, Department of Ophthalmology with findings consistent with CTK (Table 1). Patients underwent detailed ophthalmological examination including corrected distance visual acuity (CDVA) on Snellen chart, slit-lamp examination, Scheimpflug corneal tomography (Pentacam, Oculus®, Germany), and specular microscopy (CellCheck, Konan®, United States). The study adhered to the tenets of Declaration of Helsinki. All patients complained of decreased vision after CXL with significant hyperopic shift. Slit-lamp examination revealed central corneal haze or scarring, vertical striae with indistinct margins, thinning and flattening, and clear peripheral cornea. No epithelial defect, epithelial laxity, staining patterns, stromal, or epithelial edema was detected. In Case 1, confocal microscopy showed acellular stroma, striations, and thick collagen bundles (Figs. 1a-c), and specular microscopy revealed endothelial cell count of 1602 cells/mm2 OD, 2181 cells/mm2 OS. In Case 2, significant inferior flattening at the tomography (Figs. 2a and b) was seen, whereas endothelial cell count was 2890 cells/mm2 OS on specular microscopy. Confocal or specular microscopy could not be performed in Cases 3 and 4; however, central corneal haze and associated significant flattening in the tomographies were evident (Figs. 3a and b, 4a and b). On consultation with their surgical-centers, patients’ operative histories were clarified. All cases had pre-operative thinnest de-epithelialized pachymetry of >400 μ and underwent accelerated de-epithelialized CXL protocol which included saturation of the stroma by instillation of 0.1% riboflavin solution (Riboflavin, Ricrolin, Peschke Meditrade, Germany) for a total 20 min and 365 nm ultraviolet-A (UVA) light application for 10 min at an irradiance of 9 mW/cm2, delivering a total dose of energy of 5.4 J/cm2. None of the cases developed infectious keratitis or diffuse lamellar keratitis (DLK) in the early postoperative period, and corneal healing was uneventful. Duration of the findings was at least 6 months in each case, with no resolution of the symptoms. Topical loteprednol bid, cyclosporine 0.05% bid, and carboxymethylcellulose qid treatments yielded no significant benefits in any of the cases.
