Studies have shown that transcranial direct current stimulation (tDCS) changes motor cortex excitability according to the stimulation polarity. Based on these effects, it is conceivable to hypothesize that tDCS, which can modulate brain activity, may induce pain relief in patients with fibromyalgia. Fifty older women with fibromyalgia were included in this randomized, doubleblind, single-center placebo controlled trial study. Patients received sham stimulation or real tDCS with the anode centered over the primary motor cortex (M1) and the cathode over the contralateral supraorbital area (2 mA for 20 minutes for 10 sessions). Pain intensity was evaluated using the visual analog scale for pain. Assessments were done before treatment and 30 days after the last session of stimulations. The mean age of participants was 58.20 years (SD = 7.80) with an age range from 55 to 74 years.Results showed no statistically significant baseline difference among patients in demographics and clinical characteristics. Comparing visual pain analogue between the sham and treatment groups revealed a statistically significant difference (p value < 0.001) for VAS immediately after intervention and 1 month post intervention between the sham and treatment groups. Analysis of data also showed a significant reduction in pain immediately after intervention and one month post-intervention in the treatment group compared to the sham group. Anodal tDCS is an effective non-invasive technique for pain reduction in elderly women with FM. Most researchers and pharmaceutical companies are constantly trying to develop new drugs with more specific measures based on the pathophysiology of the disease. Finding ideal approach in practice guidelines with minimum adverse drug effects is a utopian. Post-drug effects deal with the biological effects of a medicine long lasted more than expected from pharmacological half-life. This could be more evident for many drugs including Hydroxychloroquine, methotrexate, leflunomide, dexamethasone, fluoxetine and lipid lowering. This effect not only maintains the therapeutic effect of the drugs, but also reduces the side effects. Leflunomide is an immunomodulatory drug of antirheumatic and diseases modifying class that is used to treat rheumatoid arthritis and some other inflammatory arthritis (namely psoriatic arthritis) by inhibiting the synthesis of pyrimidine. Due to the metabolite of leflunomide in the body and the long half-life of this drug, it is possible to prescribe this drug on a weekly basis. The results of studies on weekly administration versus daily administration of leflunomide indicate that weekly administration of leflunomide has advantages such as the same therapeutic effect as daily administration, ease of administration, lower cost of treatment, less side effects of medication, and more patient compliance. Interferon-regulatory factors (IRFs) and signal transducers and activators of transcription (STATs) are mainlyconsidered as transcriptional modulators of IFN-signature genes and type I interferon and play a major role in the regulation ofnumerous aspects of an immune response. The current study aimed to assess the transcriptional levels of IRF7 (interferonregulatory factor 7) and STAT1 (signal transducers and activators of transcription 1) mRNAs in PBMCs of scleroderma patientsand compare them with those of healthy subjects.In this study, PBMCs were obtained from 50 scleroderma patients and 30 healthy individuals. Subsequently, total RNA wasextracted from isolated PBMCs and cDNA synthesis was carried out. IRF7 and STAT1 mRNA expressions were assessed byapplying quantitative real-time PCR, SYBR Green method, and specific primers for IRF7 and STAT1.Relative expression of IRF7 was significantly increased in the patient group compared with the control group. Moreover, relativeexpression of IRF7 in limited SSc (lSSc) and diffuse SSc (dSSc) was significantly increased compared with healthy subjects (p< 0.05). The relative expression of STAT1 transcripts in PBMCs was not statistically significantly different between the patientgroup and the control group. Reactor-producedβ-particle emitting radionuclides is a new therapeutic strategy in the management of RA. This study was conducted in 2019and analyzed the toxicity and feasibility of 188Re-tin colloid injection, Three-Phase Positive Bone Scan, and Refractory KneePain. Ten patients with RA were administered radiosynovectomy with 188Re-tin colloid. The main complications after theintervention were assessed and compared with patients’ pre-intervention condition. Patients showed alleviation of pain,tenderness, and morning stiffness after the administration of radiosynovectomy. Only one RA patient received a corticosteroidinjection; the other 6 patients did not need a corticosteroid injection after radiosynovectomy. Intra-articular 188Re-tin colloidinjection seems to be an effective treatment modality for refractory knee joint pain in rheumatoid arthritis patients. In this study, we aimed to determine the prevalence and clinical characteristics of TMJ involvement in RA patients.Materials and Methods: The present study consisted of 60 individuals with RA. The patients were assigned to two groups, based on TMJ involvement: patients with TMJ involvement and patients without TMJ involvement. We defined TMJ involvement based on the findings of the questionnaires and clinical examination by a rheumatologist. The symptoms and signs were recorded by a single trained dentist.Results: The majority of the participants were women (n=49, 81.66%), with the mean age of 43.9±15.52 years (range: 21-78 years). Thirty-five patients (58.3%) had clinical evidence of TMJ involvement. The levels of C-reactive protein (CRP) and rheumatoid factor (RF) were found to be correlated with TMJ involvement. There was no significant correlation between TMJ involvement and erythrocyte sedimentation rate (ESR), age, gender, treatment duration, and disease duration.Conclusion: The high level of CRP in RA patients with TMJ involvement was reported. TMJ pain was reported in almost half of the patients. We report a patient who initially exhibited the symptoms of sarcoidosis and then limited GPA, predominantly affecting the nose and paranasal sinus, which was histologically proven. Imaging, pathological, and laboratory findings were described.
