Methods: This nonrandomized retrospective observational study analyzed data of two medical check-up of 728 office workers, over 40 years old: intervention group (376) and control (352) at baseline and after 2 years of a global health promotion program (based on nutrition, physical activity, and mental health) of mixed approach (individual, group and organizational) in the workplace of the intervention group. Results: Systolic and diastolic blood pressure experienced a statistically significant decrease (p ˂ 0.05) at 2 years of -0.94 mm Hg and -0.79 mm Hg in the intervention group versus the control group whose changes were + 0.02 mm Hg and -0.38 mm Hg. Glucose also decreased 1.59 mg/dl more in the intervention group. Lipids, smoking, body mass index or waist circumference did not undergo relevant modifications. Conclusion: This program improved blood pressure and glucose of office workers after two year follow-up, even among employees with low cardiovascular risk. However, according to the findings, this intervention was not as effective as expected in improving weight-related outcomes and cardiovascular risk in an office worker population. Previous reports have suggested various radiographic and histological features typical for these tumors, but the diagnosis can be challenging. We present a unique case of astroblastoma diagnosed after 13 years of treatment as a CNS embryonal neoplasm. Histologically, this tumor lacked previously identified astroblastic features such as pseudorosettes, trabeculated patterns, and hyalinized vessels. The tumor was synaptophysin positive which further confounded the diagnosis in this case. Methylation classification was performed with a high confidence match to a high-grade neuroepithelial neoplasm with a CXXC5-MN1 fusion. Molecular characterization confirmed a CXX5-MN1 fusion transcript which has been seen in at least one other instance. Though known to be involved in tumorigenesis, the roles of CXXC5 and MN1, in this case, remain unclear. We discuss the unusual histopathological features of this tumor and the value of recent updates to the WHO molecular diagnosis scheme for central nervous system tumors. We also briefly review the literature related to astroblastoma. Testicular tumors (TT) comprise 1 to 2% of all pediatric solid tumors. Although rare, TTs are often misdiagnosed. Screening improves clinical practice and decision-making for early diagnosis. Objective: To analyze the screening methods for detection of pediatric testicular cancer and contribute to the management of suspected cases. Methods: We conducted an integrative systematic review of the PubMed, EMBASE, and LILACS databases limited to records published between January/2013 and August/2018. Results: Sixty articles were initially selected based on the presence of specific search terms on the article body, title, and abstract. Of those, nineteen articles were selected for a full review. Clinical signs were not evident in most studies, but 12 (63%) reported the presence of scrotal masses. Cryptorchidism, family history, and the presence of contralateral TT were identified as risk factors. Alpha-fetoprotein (AFP) was elevated in 73% of studies, but normal levels do not rule out a suspected diagnosis. Scrotal ultrasound (US) was instrumental in 73% of studies and Doppler US (32%) was able to detect nearly 100% of cases at diagnosis. Computed tomography (abdomen/chest) was required in 12 studies (63%) for confirmation of metastasis of testicular germ cell tumors. In 73% of studies, stages were subdivided based on the levels of serum tumor markers and the presence of metastasis. The inguinal-scrotal approach is suggested as the preferred method as it does not affect prognosis or require adjuvant therapy. There are minimum requirements for therapeutic retroperitoneal lymphadenectomy. In 14 studies (73%), advanced stages showed a good prognosis with surgery in combination with chemotherapy. TT histology and treatment are different for prepubertal and postpubertal patients. In the same 14 studies (73%), relapsed patients received chemotherapy. In eight studies (42%) the overall survival increased by five years and, in three of them, it increased 2 by 100% post-combination therapy. Conclusion: Few studies have evaluated the prognosis, disease recurrence, and survival rates of children with testicular cancer. Studies abound on the independent effects on relationships of two of adult ADHD’s most common comorbidities - mood and substance use disorders. However, adults with ADHD sans comorbidities often experience interpersonal relationship difficulties due to the disorder’s signature symptoms alone - hyperactivity, impulsivity, and inattentiveness - as well as due to adult ADHD’s oft-observed dimension of emotional lability. The primary purpose of this study is to review the literature regarding the impact on intimate relationships of adult ADHD, as well as look at the potential compounding effects of comorbid mood or substance use disorder. The secondary purpose of this paper is to examine the literature on integrative therapy to treat couples wherein at least one partner struggles with adult ADHD, with or without comorbidities. Objectives: To determine incidence rates (IR) and risk factors of COVID-19 breakthrough infections in vaccinated people with vaccine booster (VB). Methodology: An observational, longitudinal, and prospective study of patients with COVID-19 breakthrough infections in vaccinated people with VB in a general practice setting in Toledo, Spain, for the period December 2021 to February 2022, during the wave of infections by omicron variant. Results: Forty-six cases of COVID-19 breakthrough infections with booster shot were included. The IR was 3.1 cases per 100 people with booster. The IR was higher in < 45 years (4.7%) vs. > 65 years (4.2%), and in women (3.6%) vs. man (2.6%). The only statistically significant risk/prevention factors were the presence of diseases of the skin [RR = 2.74 (95% CI: 1.3, 5.79)], Genitourinary chronic diseases [RR = 1.87 (95% CI: 1.19, 2.95)], complex family [RR = 0.22 (CI 95%: 0.58, 0.08)] and chronic diseases of the mental group [RR = 0.4 (95% CI: 0.82, 0.2)]. Conclusion: The IR of COVID-19 breakthrough infections with a booster shot, at the peak of omicron infections (December 2021-February 2022), in the general medicine clinic, Toledo, Spain, was high, suggesting modest VB protection effectiveness against symptomatic infection. Statistically significant risk and protective factors show mixed results; so, it is hypothesized that they are related to other main variables such as gender and age, and/or with risk/preventive behaviors. Socially this stage is considered the total of one lived experience. Materials and methods: The sample size has been 200 which is 100 in each group. Individuals beyond 60 years of age and who will be able to cooperate with the study were included in the study. World Health Organization oral health assessment form (2013) was used to record the oral health status of the subjects. The data were entered into an excel sheet and transferred to SPSS V. 21. for statistical analysis and Chi-square and ANOVA tests were done. Results: On assessment, 83 percent of old-age home residents and 68 percent from the family group did not have a periodontal pocket. Nonetheless, 12 percent of the individuals in a care home and 31% of the subjects in the family setting had pocket depths of 4-5 mm, while 1% of both groups had pocket depths of 6 mm. In comparison among elderly within the family set up and old age, homes reported a statistically significant difference between the 2 groups (p < 0.001) Conclusion: The findings of this study revealed that the institutionalized geriatric population suffers from a wide range of dental illnesses, the most frequent of which is dental caries.
