{"title":"Evaluation of the surgical treatment results of chronic otitis media with tympanic membrane perforation","authors":"Long Luong Si, Nhat Vo Doan Minh, Thai Le Thanh","doi":"10.34071/jmp.2023.1.18","DOIUrl":"https://doi.org/10.34071/jmp.2023.1.18","url":null,"abstract":"Objectives: To describe the clinical and paraclinical characteristics of chronic otitis media with tympanic membrane perforation and evaluate the results of surgical treatment of chronic otitis media with perforation.\u0000\u0000Materials and methods: 75 patients were diagnosed with chronic otitis media with perforation and underwent surgery at the Hue University of Medicine and Pharmacy Hospital and Hue Central Hospital from May 2020 to September 2021.\u0000\u0000Results: The disease was more common in women, mainly in the age group of 16-30 years old (34.7%). Common preoperative symptoms are hearing loss and tinnitus. The main reason for surgery was a history of prolonged otorrhea (50.7%). The primary location of the tympanic membrane perforation is central (74.7%). The symptoms of hearing loss and tinnitus in patients after myringoplasty improved significantly compared to before surgery. The average hearing after myringoplasty and attico-antrostomy surgery improved statistically significantly compared to before surgery. The overall anatomical outcome after surgery was good, accounting for the majority with the rate of 86.7%.\u0000\u0000Conclusion: Chronic otitis media is a common disease with a long duration and affects life quality. Parotid gland tumors show a high morphological heterogeneity and presents with diagnostic and surgical challenges. Therefore, investigation of clinical, para-clinical features of parotid gland tumor and the morphology of the facial nerve is very important for surgery and preventing the facial palsy.\u0000\u0000Objective: Evaluate clinical, para-clinical features and the morphology of the facial nerve in parotid salivary tumor patients indicated for surgery.\u0000\u0000Materials and Methods: 31 patients with parotid gland tumor who were examined at Hue University of Medicine and Pharmacy from May 2020 to March 2021 were recorded about clinical, paraclinical and the facial nerve branching patterns in surgical treatments of parotid tumors.\u0000\u0000Results: Male cases were in the majority (58.1%), the mean age of patients was 51.5 ± 12.5. Slow progressively swelling was the common presenting complaint. Majority of the tumor size in CT scanner were between 2 and 4 cm (61.3%) and in the superficial lobe (80.0%).All are benign tumors and Whartin tumor is the most common type (38.7%). In all 27 patients were exposed the VII nerve, the facial nerve consisted of one trunk separated into two divisions. In 12 patients with identified type of facial nerve branching, type IV (41.7%) were the most common type.\u0000\u0000Conclusion: Clinical and para-clinical were valuable for diagnosis. Although there has been a slight increase in overall survival due to the inclusion of targeted medications in the standard chemotherapeutic treatment paradigm, the prognosis remains poor. New evidence suggests that certain molecular and histopathologic tumor characteristics, as well as staging, may also influence patients’ prognosis. Indeed, two detailed genomic classifications of gastric cancer have recently been developed by The Cancer Genome Atlas (TCGA) and the Asian Cancer Research Group (ACRG), showing that microsatellite instability (MSI) is a distinct subgroup related to gastric cancer patients’ prognosis. Interestingly, the MSI-H subgroup was identified as a separate entity of GC in both of these classifications that are relevant to the prognosis of gastric cancer. Microsatellite instability (MSI) is caused most frequently by deficiency in the mismatch repair proterin (MMR) (including MLH1, MSH2, PMS2, MSH6) and can be defined by immunohistochemistry (IHC). Microsatellite instability offers a good prognostic marker associated with different cancer types. Coronary artery disease is an important cause of chest pain that should be evaluated and managed early. The HEART score is designed to stratify the risk of emergency patients with undifferentiated chest pain, to identify high-risk patients who require more resources or need early emergency treatment, and low-risk patients to avoid unnecessary hospitalization. In my country, there have not been many studies on the HEART score yet.\u0000\u0000Objective: To evaluate the value of the HEART sore in risk stratification among patients with chest pain in the emergency department.\u0000\u0000Methods: Data used from 70 patients presented with chest pain to Emergency-Stroke Center of Hue university hospital. The HEART score was calculated. Outcome was occurrence of MACE (mortality, AMI, PCI, CABG) within 30-days of initial presentation.\u0000\u0000Results: Patient demographics include an average age of 63.80, 67.10% female, and 41.40% with history of hypertension, 21.40% with history PCI. 14.30% of patients met the outcome. The percent of patients with 30-day MACE with HEART score between 0 and 3, 4 - 6, and 7 - 10 was 4.30%, 10.50%, and 55.60%, respectively. Currently, there are many imaging methods to use for the early detection, and diagnosis of pancreatic tumors. Endoscopic ultrasound (EUS) with the strength of a high-frequency probe, approaching directly the pancreas which is improving the diagnosis of the tumor, especially small lesions < 2 cm. This study was aimed at: (1) To describe some clinical, biological, and endoscopic ultrasound characteristics of pancreatic tumors. (2) To evaluate the relationship between clinical and biological characteristics and pancreatic tumor characteristics on endoscopic ultrasound.\u0000\u0000Subject and methods: Cross-sectional study in 41 patients diagnosed with pancreatic tumor on endoscopic ultrasound.\u0000\u0000Results: Common history was smoking, high intake of alcohol and diabetes. Almost half of the patients 48.5% had a CA 19.9 ≤ 37 U/ml. Lesions in the pancreatic head accounted for 80.5%, hypoechoic lesions 97.6%, solid tumor 80.5%. Common extra-tumoral findings were abdominal lymph nodes accounting for 51.2%, common bile duct dilatation 46.3% and pancreatic duct dilatation 39.0%. There was no relationship between levels of CA 19.9 and endoscopic ultrasound features such as vascular invasion, and lymph node metastasis (p > 0.05). There was a statistically significant relationship of tumors in the head of the pancreas and jaundice, and dilation of the common bile duct (p < 0.05).\u0000\u0000Conclusion: Smoking, high intake of alcohol and diabetes were the most common history. Common findings of tumor were hypoechoic, solid tumor and pancreatic head position. This study aims to accomplish the objective: classify the prevalent histopathological patterns of gastrointestinal lymphoma according to WHO 2019 classification based on histopathology and immunohistochemistry.\u0000\u0000Subjects and methods: A cross-sectional study sampled on 76 gastrointestinal lymphoma patients to be examined and treated at The Hue University of Medicine and Pharmacy Hospital and Viet Nam National Cancer Hospital from January 2021 to June 2022.\u0000\u0000Results: B cell and T cell Lymphoma made up 97.4% and 2.6%, respectively; among them, the most common pattern was diffuse large B-cell lymphoma (59.2%,), Mucosa-associated lymphoid tissue lymphoma (17.1%), Mantle cell lymphoma (15.8%), Follicular lymphoma (2.6%), High-grade B-cell lymphoma (1.3%), and Small lymphocytic lymphoma (1.3%). The most frequent position was the stomach (60.5%), the rectum and colon, small intestine, and ileocecal region at 19.7%, 10.5%, and 9.3%, respectively.\u0000\u0000Conclusions: Using histopathology and immunohistochemistry can classify the majority of gastrointestinal Non - Hodgkin lymphoma according to the WHO 2019 classification. Hoang Thi, Huy Nguyen Quoc, Duy Ho Khanh, Dung Pham Minh","doi":"10.34071/jmp.2023.1.5","DOIUrl":"https://doi.org/10.34071/jmp.2023.1.5","url":null,"abstract":"Purpose: The purpose of this study was to evaluate the role of MDCT in detecting and predicting blunt bowel and mesenteric injuries.\u0000\u0000Materials and methods: This is a retrospective observational study of 66 patients (54 males, 12 females, average age of 45.5 years) who underwent laparotomy at Nghe An Friendship General Hospital between April 2020 and August 2021 with a diagnosis of bowel and/or mesenteric injuries in blunt abdominal trauma. All of these patients had a contrast-enhanced MDCT prior to surgery. All CT findings were documented and calculated for their diagnostic performance compared to surgical outcomes.\u0000\u0000Results: The most common finding was mesenteric infiltration (75.8%), followed by bowel wall thickening (63.6%). Bowel wall discontinuity had the highest specificity and positive predictive value of 100%. The most sensitive signs were free air 81.6%, mesenteric infiltration 79.7%. The specificity of bowel hypoenhancement and mesenteric vessel abnormalities were highest at 92.3% and 98.1%, respectively. Objective: To survey the clinical characteristics and features of color and shape of scleral blood vessels at 12 o’clock position in patients with neck and shoulder pain.\u0000\u0000Materials and methods: A cross-sectional descriptive study was carried out in 147 patients who were diagnosed with neck and shoulder pain at Thua Thien Hue Traditional Medicine Hospital.\u0000\u0000Results: The symptoms of the tongue and pulse: pale red tongue accounted for 42.2%, enlarged tongue 44.9%, white fur 83.0%, thin fur 63.3%, slippery fur 44.2%; sunken pulse 93.9%, moderate pulse 72.1%, replete pulse 73.5%. About scleral blood vessels: bright red 47.6% at the right eye (RE) and 44.9% at the left eye (LE); winding shape 61.9% (RE) and 57.1% (LE); branched shape 66.7% (RE) and 60.5% (LE).\u0000\u0000Conclusions: The symptoms of high rate were pale red tongue, enlarged tongue, white fur, thin fur, slippery fur, sunken pulse, moderate pulse and replete pulse. Đánh giá kết quả điều trị mộng thịt bằng phương pháp phẫu thuật xoay hai vạt kết mạc.\u0000\u0000Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả, tiến cứu có can thiệp lâm sàng trên 41 mắt của 36 bệnh nhân mộng thịt nguyên phát điều trị bằng phẫu thuật xoay hai vạt kết mạc tại khoa Tai Mũi Họng - Mắt - Răng Hàm Mặt, Bệnh viện Trường Đại học Y - Dược Huế. Bệnh nhân được theo dõi và đánh giá kết quả khi ra viện, sau phẫu thuật 1 tuần, 1 tháng và 3 tháng.\u0000\u0000Kết quả: Mộng thịt độ II và III chiếm đa số, hình thái lâm sàng trung gian chiếm 56,1%. Thị lực trung bình sau phẫu thuật là 0,66 ± 0,23 (trước phẫu thuật là 0,54 ± 0,28). Độ loạn thị giảm từ 1,23 ± 0,83D xuống 0,98 ± 0,59D sau 1 tháng và 0,71 ± 0,54D sau 3 tháng phẫu thuật. Chỉ số Kf của mắt mộng thịt độ III và IV sau phẫu thuật tăng có ý nghĩa thống kê với p < 0,05. 40/41 mắt trong nghiên cứu (97,56%) đạt kết quả thẩm mỹ tốt. Không ghi nhận trường hợp nào tái phát sau 3 tháng phẫu thuật.\u0000\u0000Kết luận: Nghiên cứu bước đầu của phương pháp xoay hai vạt kết mạc đã đạt hiệu quả tốt về thẩm mỹ, chức năng, ít biến chứng, tỷ lệ tái phát thấp.\u0000\u0000Từ khóa:mộng thịt, phương pháp xoay hai vạt kết mạc, loạn thị.\u0000Abstract:\u0000Background: The rotational conjuctival flap technique in primary pterygium surgery has been studied before with positive outcomes in terms of aesthetics and recurrence rate. Based on this, we have studied and made improvements in the double conjunctival flap rotation technique in order to simplify the surgical process as well as increase the effectiveness of treatment for patients with pterygium.\u0000\u0000Objective: 1. To investigate clinical features of pterygium. 2. To evaluate the results of pterygium treatment by rotating double conjunctival flaps.\u0000\u0000Methods: Descriptive, prospective, clinical trial on 41 eyes of 36 primary pterygium patients who were treated with double - rotating conjunctival flaps surgery at the ENT - Eye - Odontology Department, Hue University of Medicine and Pharmacy Hospital. Patients were followed up at postoperative day 1, 7; 1st month and 3rd month.\u0000\u0000Results: Grade II and III pterygium accounted for the majority, the intermediate clinical morphology comprised 56.1% of cases.
