Both diseases have substantial public health consequences in terms of associated morbidity and mortality. In obese individuals with diabetes, weight loss can bring several physical and psychological benefits: decreased glycosylated hemoglobin and blood pressure, improved lipid levels, and elevated sense of well-being. A combination of diet, exercise, and behavioral therapy, which are considered components of lifestyle modification, is recommended as the first strategy. If an individual is unresponsive to these approaches or has comorbidities that are currently compromising his or her health, pharmacotherapy and/or surgery may be advised. These include the classic cardiovascular risk factors, type 2 diabetes, menstrual abnormalities, sleep-disordered breathing, and psychosocial effects. Although stigmatization of obese children is convincingly documented, the evidence regarding the psychological consequences of child and adolescent obesity is more equivocal. Although data from long-term studies are lacking, short-term investigations indicate that consumption of low-GI carbohydrates may delay the return of hunger and reduce subsequent energy intake relative to consumption of higher-GI carbohydrates. In spite of the rising numbers of people who are overweight and obese, medical treatment of obesity is currently where medical treatment of hypertension was in the 1950s with only two Food and Drug Administration-approved medications for chronic weight loss and maintenance, sibutramine and orlistat. This article reviews these medications and others currently being used for weight loss and weight maintenance. Available medications for obesity include drugs that affect caloric intake and appetite and alter energy expenditure and nutrient absorption, as well as some with an unclear mechanism of action at present.
