{"title":"Prevention of VZV infection in immunosuppressed patients using antiviral agents.","authors":"Michael Boeckh","doi":"","DOIUrl":"","url":null,"abstract":"

Antiviral agents play a key role in the prevention and treatment of varicella zoster virus (VZV) disease in immunosuppressed patients. Randomized trials show that aciclovir is effective in preventing VZV reactivation disease; however, no consensus exists on dose, duration and patient population for its use. The recent shortage of VZV-specific immunoglobulin has generated renewed interest in the use of antiviral agents as post-exposure prophylaxis. The use of antiviral agents for post-exposure prophylaxis is not supported by randomized trials, but uncontrolled experience suggests that it might be a reasonable alternative if varicella-specific immunoglobulin is not available. Current evidence on the use of antiviral agents in the prevention of reactivation disease and management of exposure to VZV is discussed.
","PeriodicalId":83725,"journal":{"name":"Herpes : the journal of the IHMF","volume":"13 3","pages":"60-5"},"PeriodicalIF":0.0,"publicationDate":"2006-11-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"26425104","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The correlation between the two events is discussed. It is speculated that, during the early phase of the infection, the HHV-6 spread to the central nervous system and triggered a pathogenic process that initially developed without symptoms. When the neurological disorders appeared, HHV-6 had already established a latent state: only the virus carried by infected blood cells was detected in the cerebrospinal fluid.
","PeriodicalId":83725,"journal":{"name":"Herpes : the journal of the IHMF","volume":"13 3","pages":"72-4"},"PeriodicalIF":0.0,"publicationDate":"2006-11-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"26425107","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

{"title":"Varicella zoster virus: out of Africa and into the research laboratory.","authors":"C Grose","doi":"","DOIUrl":"","url":null,"abstract":"

This review updates on numerous topics relating to the evolutionary origins of varicella zoster virus (VZV), the replication cycle, virion assembly and the recent genomic analyses. VZV is one of eight human herpesviruses that have existed for at least 400 million years. It has co-evolved with humankind and is present in all nationalities globally. The pathogenesis of varicella (chickenpox) is dependent on viral replication and dispersion through the body in T-lymphocytes. VZV replication is similar to that of herpes simplex virus. A complete analysis of VZV transcripts has identified their relative abundance, with transcripts for the regulatory proteins (open reading frame) ORF62 and ORF63 among the greatest. Studies of virion assembly have shown that endocytosis pathways are involved in the envelopment process by the viral glycoproteins. The complete sequencing of five VZV strains has identified numerous single nucleotide polymorphisms, and in turn, VZV strains have been segregated into European/North American and Asian clades. Furthermore, a small number of mutant VZV strains have been identified. These results suggest more diversity between VZV strains than previously recognized.
","PeriodicalId":83725,"journal":{"name":"Herpes : the journal of the IHMF","volume":"13 2","pages":"32-6"},"PeriodicalIF":0.0,"publicationDate":"2006-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"26190141","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

{"title":"Toll-like receptors, innate immunity and HSV pathogenesis.","authors":"M Herbst-Kralovetz, R Pyles","doi":"","DOIUrl":"","url":null,"abstract":"

In the last decade, substantial progress has been made in understanding the molecular mechanisms involved in initial host responses to viral infections, and how viral recognition leads to the innate responses that ultimately shape the adaptive immune response. Viruses, including herpes simplex virus (HSV) types 1 and 2, trigger toll-like receptors (TLRs) that elicit cytokine and chemokine production. In turn, this can create local resistance and modulate T- and B-cell-mediated immunity. TLR activation by HSV-produced molecules (or other synthetic agonists) leads to the remodelling of draining lymph nodes. This enhances the screening of naive T-cells, from which antigen-specific lymphocytes can be selected and expanded. The innate response thereby serves to direct a timely and effective acquired immune response, through the initial TLR recognition of viral pathogen-associated molecular patterns that can limit or possibly exacerbate viral pathogenesis. Recently, these findings have been exploited by strategies that utilize synthetic TLR agonists as prophylactic or therapeutic devices. Such devices prime innate immune responses, enhancing host resistance to viral infections, including experimental HSV infections.
","PeriodicalId":83725,"journal":{"name":"Herpes : the journal of the IHMF","volume":"13 2","pages":"37-41"},"PeriodicalIF":0.0,"publicationDate":"2006-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"26190143","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

{"title":"New approaches to the therapy of HSV infections.","authors":"R Whitley","doi":"","DOIUrl":"","url":null,"abstract":"

Two recent studies have demonstrated single-day regimens of high-dose antiviral therapy (famciclovir 1000 mg twice daily for genital herpes, and famciclovir 1500 mg single dose for herpes labialis) to be effective episodic treatment strategies. Both have the potential to improve patient compliance and therapeutic satisfaction. Patient-initiated episodic therapy (PIE) regimens such as these may improve the time to treatment initiation, which can halt lesion development: PIE allows antiviral drugs to be administered in the narrow therapeutic window that occurs early in the course of a herpes simplex virus (HSV) infection episode. This review article discusses short-course antiviral therapy, which may offer an effective alternative to traditional episodic or suppressive antiviral regimens for HSV-related disease outbreaks.
","PeriodicalId":83725,"journal":{"name":"Herpes : the journal of the IHMF","volume":"13 2","pages":"53-5"},"PeriodicalIF":0.0,"publicationDate":"2006-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"26190146","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

While highly active antiretroviral therapy has led to impressive declines in the incidence of Kaposi's sarcoma, HHV-8- associated infections remain common and morbid across the globe. Although comprehensive clinical trials of therapy for HHV-8-associated infections are lacking, numerous studies have found current treatment modalities to be ineffective for many patients. A growing body of basic scientific and clinical work suggests that antiviral drugs may have a role in the treatment and (more significantly) in the prevention of HHV-8-associated infections. This review discusses current evidence for the efficacy of antivirals against HHV-8 and proposes novel uses for such medications in HHV-8 treatment.
","PeriodicalId":83725,"journal":{"name":"Herpes : the journal of the IHMF","volume":"13 2","pages":"42-7"},"PeriodicalIF":0.0,"publicationDate":"2006-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"26190144","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

{"title":"Adolescents anticipated experience of screening for genital herpes.","authors":"S Ramos, S Cohen, J Caskey, S Rosenthal","doi":"","DOIUrl":"","url":null,"abstract":"

Screening asymptomatic adolescents for genital herpes will require sensitivity to adolescents' developmental needs. Twenty-four adolescents (age range 1619 years) were interviewed to explore their perceptions of screening. In general, adolescents thought screening for genital herpes would be viewed as taking care of themselves, although there were concerns that their peers might view getting screened as implying that the adolescent was infected or sexually promiscuous. Most adolescents expected their parents to respond positively, but typically thought that younger adolescents should obtain parental consent for screening, and that adolescents should inform their parents of positive test results. Adolescents wanted to be screened in settings that provided confidentiality and by non-judgemental care providers, but they differed on the setting which they believed would accomplish this. Some recommended mass screening (for example, screening all of those in a certain year at school), presumably as a way to reduce embarrassment and/or stigma. Screening programmes that are adolescent-friendly and accessible, and address adolescents' specific concerns regarding managing the information, can be created.
","PeriodicalId":83725,"journal":{"name":"Herpes : the journal of the IHMF","volume":"13 2","pages":"49-52; discussion 48"},"PeriodicalIF":0.0,"publicationDate":"2006-08-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"26190145","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

{"title":"Re-evaluating the diagnostic methods in herpes simplex encephalitis.","authors":"Amer Al-Shekhlee, Naira Kocharian, Jose J Suarez","doi":"","DOIUrl":"","url":null,"abstract":"

Several methods are used in clinical practice to investigate herpes simplex encephalitis (HSE), including electroencephalography (EEG) and polymerase chain reaction (PCR) of the viral genome in cerebrospinal fluid. PCR is the most sensitive and specific of the diagnostic methods currently employed. We retrospectively examined the diagnostic utility of EEG and cranial imaging within the first 24-48 h of symptom onset in patients with suspected HSE. Patients with herpes simplex-positive PCR results were compared with those with herpes simplex-negative PCR results. Periodic lateralized epileptiform discharges and/or focal temporal slowing were present in 90% of the PCR-positive patients at symptom onset compared with only 30% of the PCR-negative group. The sensitivity of EEG recordings decreased after 48 h. Although no patients had computed tomography findings suggestive of HSE, magnetic resonance imaging results were consistent with HSE in 86% of those with herpes simplex-positive PCR results obtained after 48 h from symptom onset.
","PeriodicalId":83725,"journal":{"name":"Herpes : the journal of the IHMF","volume":"13 1","pages":"17-9"},"PeriodicalIF":0.0,"publicationDate":"2006-05-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"26052953","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
