Escobar Giraldo","doi":"10.1016/j.oftal.2024.10.003","DOIUrl":"10.1016/j.oftal.2024.10.003","url":null,"abstract":"<div><h3>Purpose</h3><div>To determine the effectiveness of intense pulsed light (IPL) therapy in improving symptoms in patients with a diagnosis of evaporative dry eye disease secondary to Meibomian gland dysfunction (MGD) refractory to conventional treatment.</div></div><div><h3>Methods</h3><div>Pseudo-experimental, analytical, prospective, longitudinal, interventional study. Patients aged 18-74<!--> <!-->years with a diagnosis of dry eye disease refractory to conventional dry eye treatment with the presence of DGM were included. A total of 4 IPL sessions were performed at days 0, 15, 45 and 75. At each visit, ocular surface disease index (OSDI) data and 1-10 analog scale were taken as objective evidence of symptomatic status.</div></div><div><h3>Results</h3><div>Sixty three patients were included, median age was 43<!--> <!-->years, 61.5% were female, 42.9% of patients used one conventional treatment for dry eye prior to IPL therapy and 57.1% of patients used two or more treatments. Sixty three patients completed at least one IPL session, 45 had two sessions, 32 had three sessions and 18 had four sessions. While rare, they have been associated with some uncommon adverse effects. We present a case of retinal vasculitis associated with bevacizumab in a72-year-old woman who presented to our clinic with sudden visual acuity loss in her left eye due to macular edema following central vein occlusion. She was treated with bevacizumab, with a minor inflammatory response that resolved with topical steroids. After 6 weeks, the macular edema recurred and a 2<sup>nd</sup> dose of bevacizumab was indicated, with a severe inflammatory reaction that resolved with periocular steroids and topical NSAIDs. Blanch","doi":"10.1016/j.oftal.2024.10.002","DOIUrl":"10.1016/j.oftal.2024.10.002","url":null,"abstract":"<div><h3>Background and purposes</h3><div>Age-related macular degeneration (AMD) is one of the leading causes of visual acuity (VA) impairment in Europe. The assessment of the patients’ vision-related quality of life (VRQoL) is critical to disease management. The Impact of Visual Impairment (IVI) questionnaire is indicated to assess VRQoL in AMD and its most severe form, neovascular AMD (nAMD). Translation and cultural adaptation of 28-item IVI questionnaire (IVI-28) to Castilian and three other co-official languages in Spain is presented.</div></div><div><h3>Material and methods</h3><div>Observational, cross-sectional, multicentric study with two focus groups for each language: Castilian, Catalan, Galician and Basque. Translated versions of IVI-28 were administered to patients, with a subsequent comprehension test to assess the clarity and understanding of the instructions, items and response formats.</div></div><div><h3>Results</h3><div>The Castilian version of the IVI-28 (IVI-Es) was administered to 11 patients, the Catalan version (IVI-Ca) to 12 patients, the Galician version (IVI-Ga) to 9 patients and the Basque version (IVI-Eu) to 8 patients. Druchkiv","doi":"10.1016/j.oftal.2024.07.007","DOIUrl":"10.1016/j.oftal.2024.07.007","url":null,"abstract":"<div><h3>Purpose</h3><div>To analyze the incidence, causes, risk factors and treatment of acute postoperative endophthalmitis (POE) after phacoemulsification in a private ophthalmological group.</div></div><div><h3>Design</h3><div>Uncontrolled retrospective observational study of all cases of POE over 22 years.</div></div><div><h3>Material and methods</h3><div>369,476 eyes were included after phacoemulsification in 41 surgical centers of the Clínica Baviera-AIER EYE Group from 2002 to 2023. POE cases were reviewed.</div></div><div><h3>Results</h3><div>The general group was divided into two: group A (2002 to 2007), with vancomycin in the intracameral irrigation flow (27,705 eyes); Group B (2008 to 2023), with intracameral cefuroxime (341,771 eyes). 31 cases of POE were found (incidence, 0.0084%); 5 in group A (0.018%) and 26 in group B (0.0076%) (p<!--> <!-->=<!--> <!-->0.314), with a mean age of 67.3 years (14 men and 17 women). Cultures were positive in 14 cases in Groups A and B. Treatment was based on intravitreal antibiotics and vitrectomy. The mean time to symptom onset was 6.76 days and the mean time to resolution was 120.7 days. 12 eyes achieved corrected distance visual acuity ≥20/40.</div></div><div><h3>Conclusions</h3><div>The incidence of POEwas 0.0084% (0.0181% in group A and 0.0076% in group B). The probability of having POE in group A was 2.37 times greater than in group B (OR: 2.37; 95% CI: 0.71-6.2; p<!--> <!-->=<!--> <!-->0.079). We present three clinical cases of CSHRPE located in the macular region observed casually and diagnosed thanks to autofluorescence and especially optical coherence tomography, non-invasive tests. We categorized our patients in 3 groups; pre-COVID-19: patients from January 2019 through January 28<sup>th</sup> 2020, COVID-19: patients from January 29<sup>th</sup> 2020 through July 2021 and late COVID-19: patients from August 2021 through July 24<sup>th</sup> 2023.</div></div><div><h3>Results</h3><div>No differences across groups were reported; the date of first consultation (DFC) and MMS (<em>P</em> <!-->=<!--> <!-->.06), the date of the referral request (DRR) and MMS (p<!--> <!-->=<!--> <!-->0,054), the MMS date to reconstruction (<em>P</em> <!-->=<!--> <!-->.19), and the time of lesion onset (<em>P</em> <!-->=<!--> <!-->.054). The type of reconstruction was not impacted by the number of days between the clinic date and reconstruction (<em>P</em> <!-->=<!--> <!-->.60), the DFC and MMS (<em>P</em> <!-->=<!--> <!-->.60), and the DRR and MMS (<em>P</em> <!-->=<!--> <!-->.27). A total of 29% were treated as combined procedures with cataract surgery. A total of 71% were categorized as moderate/advanced glaucoma. Success criteria A was defined as an intraocular pressure (IOP) reduction<!--> <!-->&gt; 20%, 5 &lt;<!--> <!-->IOP &lt;<!--> <!-->21<!--> <!-->mmHg, no reoperation, no severe complications. More restrictive definitions of success were also applied for further analysis: B (5<!--> <!-->mmHg &lt;<!--> <!-->IOP ≤ 18<!--> <!-->mmHg, no reoperation, IOP reduction<!--> <!-->&gt;20%, no severe complications); C (5<!--> <!-->mmHg &lt;<!--> <!-->IOP ≤ 15<!--> <!-->mm Hg, no reoperation, IOP reduction ≥ 40%, no severe complications).</div></div><div><h3>Results</h3><div>Mean IOP dropped from 22.2<!--> <!-->±<!--> <!-->8.0 down to 15.2<!--> <!-->±<!--> <!-->3.7<!--> <!-->mmHg (20.5<!--> <!-->±<!--> <!-->12.7-months follow up, 30% reduction) (<em>P</em>&lt;.0001). Success rates in the overall group were 71% (A), 62% (B), and 19% (C). Complications included intraoperative arterial hemorrhages in 2 cases (4.8%) and late blood reflux in another 2 (4.8%). Six eyes (14%) required reoperation due to treatment failure. A total of 21 eyes (50%) had baseline IOP levels ≥ 22<!--> <!-->mmHg.
