Other secondary objectives were evaluate safety and whether the characteristics of smoking, adherence, and the intensity of craving and withdrawal syndrome at 4th week were associated with effectiveness.</p><p><strong>Methods: </strong>Observational, prospective, multicenter study that includes smokers motivated to quit evaluated in twelve Smoking Cessation Services in Spain.</p><p><strong>Results: </strong>A total of 337 smokers were studied. Effectiveness of cytisinicline was 68% at the end of treatment, but was reduced to 48.7%, 40.9% and 28.2%, at 3rd, 6th and 12th month of follow up respectively. The measurement of adherence and the intensity of craving by SRC showed statistically significant association with effectiveness at 24th week, RRadj=2.2 (p<0.001) and RRadj=0.87 (p=0.011) respectively. Common adverse effects (occurring in more than 10% of patients) were: sleep disorders, headaches, dizziness and digestive disorders.</p><p><strong>Conclusions: </strong>The effectiveness of protocol was 68% at the end of treatment, but was reduced to 48.7%, 40.9% and 28.2% at 3rd, 6th and 12th months respectively. Subjects who met more than 80% of treatment adherence doubled their chances of success at 6th month. For each point of craving intensity, measured by the SRC, at the end of the pharmacological treatment the chances of success at 6th month were reduced by 13%. Pulmonary rehabilitation<span> programmes, including exercise with non-invasive ventilation (NIV) or high-flow nasal therapy (HFT), aims to improve quality of life and exercise tolerance. This study investigates the relationship between thoracoabdominal </span></span></span></span>asynchrony<span> (TAA) during supported (NIV and HFT) and unsupported (conventional oxygen therapy – COT) exercise and clinical and functional parameters in severe COPD patients awaiting lung transplantation.</span></div></div><div><h3>Methods</h3><div><span>This experimental, longitudinal, prospective, controlled study included 20 severe COPD patients on the lung transplant waiting list. Patients underwent three constant load exercise tests under COT, NIV, and HFT conditions. TAA was measured using respiratory inductance plethysmography, and neurorrespiratory drive (NRD) was assessed via parasternal </span>electromyography.</div></div><div><h3>Results</h3><div>Patients exhibited distinct TAA patterns during exercise. Clockwise rotation (thorax ahead) was associated with worse baseline lung function, higher peak exercise dyspnoea and higher peak exercise NRD compared to counterclockwise rotation (abdomen ahead). No significant differences in TAA were observed between the three exercise conditions (COT, NIV, HFT). However, patients with clockwise TAA were more likely to have reduction in breathlessness with NIV compared to COT than those with counterclockwise rotation.</div></div><div><h3>Conclusions</h3><div>TAA patterns during exercise in severe COPD patients can indicate the severity of lung function impairment and predict severity of exercise induced dyspnoea. Advances in imaging devices have improved diagnostic capabilities, potentially affecting clinical characteristics and overall survival (OS); however, details remain unclear. This study conducted a pooled analysis of case reports and series to analyse the clinical characteristics and OS of PPAS in the era of advanced </span>medical devices.</div></div><div><h3>Methods</h3><div>Data were sourced from PubMed and CINAHL, focusing on studies published between 1 January 2014 and 31 December 2023. The study included patients diagnosed with PPAS, with extracted data covering demographics, diagnosis, treatments, and survival.</div></div><div><h3>Results</h3><div>Overall, 643 patients were included (mean age: 52.6<!--> <!-->±<!--> <span>13.1 years; 50.4% were female). Initially, 70.6% were diagnosed with pulmonary thromboembolism<span><span> (PTE), and 15.4% were suspected of having PPAS. Among these, 93.9% and 55.2% showed suggestive findings on computed tomography (CT)-integrated </span>positron emission tomography with 2-deoxy-2-18F-fluoro-</span></span><span>d</span>-glucose (18F-FDG PET/CT) and CT, respectively, with 98.2% confirmed before death. The right main pulmonary artery was the most affected site on CT (72.3%). Surgery and chemotherapy were performed in 81.4% and 66.4% of patients, respectively. The median OS was 31 months, with surgery extending OS across all stages and chemotherapy benefiting stages III–IV. Longer OS was achieved in patients who underwent complete surgical resection.</div></div><div><h3>Conclusions</h3><div>18F-FDG PET/CT and multi-detector-row CT can differentiate PTE from PPAS. Despite their clinical relevance, current COPD guidelines provide limited direction regarding nutritional assessment and intervention. This consensus document, developed jointly by the Spanish Society of Pulmonology and Thoracic Surgery (SEPAR) and the Spanish Society of Endocrinology and Nutrition (SEEN), offers evidence-based recommendations for the nutritional assessment and management of patients with COPD. A multidisciplinary panel of experts identified three key clinical issues relating to the prevalence and impact of malnutrition, appropriate assessment strategies, and the effectiveness of nutritional interventions. Systematic reviews were conducted in accordance with PRISMA guidelines, and the quality of evidence was assessed using SIGN and GRADE methodologies.
