Marin","doi":"10.1016/j.arbres.2024.07.003","DOIUrl":"10.1016/j.arbres.2024.07.003","url":null,"abstract":"<div><h3>Introduction</h3><div>In patients with obstructive sleep apnea (OSA), novel metrics such as hypoxic burden (HB) and sleep apnea-specific pulse-rate response (ΔHR) may better correlate with cardiovascular diseases (CVD) than the apnea–hypopnea index (AHI). This manuscript aims to assess the correlation between ΔHR and HB with subclinical atherosclerosis in patients with OSA, testing the hypothesis that elevated ΔHR and HB are associated with subclinical atherosclerosis development.</div></div><div><h3>Methods</h3><div>In a prospective study, individuals aged 20–65 years with suspected OSA without known comorbidities were consecutively recruited and defined as OSA (AHI<!--> <!-->≥<!--> <!-->5<!--> <!-->events/h) or healthy controls. Using bilateral carotid ultrasonography, common carotid intima-media thickness (CIMT) was assessed and the identification of at least one atheromatous plaque defined the presence of subclinical atherosclerosis. ΔHR, and HB were derived from pulse-oximetry.</div></div><div><h3>Results</h3><div>We studied 296 patients of age 45<!--> <!-->±<!--> <!-->10 years old, of whom 28% were women, and with a BMI of 30.3<!--> <!-->±<!--> <!-->5.3<!--> <!-->kg/m<sup>2</sup>. Overall, 245 had OSA and 51 were healthy controls. After controlling for confounding variables higher ΔHR but not HB, was associated with higher CIMT (<em>p</em> <!-->=<!--> <!-->0.006) and higher time spent with oxygen saturation below 90% (T90) was associated with an increase in carotid atheroma plaques (<em>p</em> <!-->=<!--> <!-->0.032). When stratifying OSA based on HB tertiles, we observed that within tertile 2 of HB, an increase in ΔHR was associated with larger CIMT (<em>p</em> <!-->=<!--> <!-->0.017).</div></div><div><h3>Conclusion</h3><div>A higher ΔHR is associated with an increase in CIMT among adult patients with OSA. {"title":"Five-Repetition Sit-to-Stand Test as Predictor of Mortality in High Risk COPD Patients","authors":"Roberto Bernabeu-Mora , Elisa Valera-Novella , Elodia Teresa Bernabeu-Serrano , Juan José Soler-Cataluña , Myriam Calle-Rubio , Francesc Medina-Mirapeix","doi":"10.1016/j.arbres.2024.07.026","DOIUrl":"10.1016/j.arbres.2024.07.026","url":null,"abstract":"<div><h3>Objective</h3><div>To determine if adding performance on the five-repetition sit-to-stand test (5-STS) to chronic obstructive pulmonary disease (COPD) high-risk criteria, proposed by the Spanish COPD guidelines (GesEPOC), affects mortality prognosis.</div></div><div><h3>Methods</h3><div>Observational study of COPD outpatients involved prospective follow-up for 5 years. Patients were classified based on 5-STS performance and risk criteria proposed by GesEPOC version 2021. Outcome measures were 5-year mortality timing and rate. Kaplan–Meier curves and univariate and multivariate Cox proportional-hazard analyses, analysis of variance, and univariate and multivariate linear and logistic regression models were used.</div></div><div><h3>Results</h3><div>One hundred and thirty-seven patients were included. Mean age was 66<!--> <!-->±<!--> <!-->8.3 years, and 87.6% were men. Of them, 115 (83.9%) were classified as high risk, 43 (34.4%) of whom had poor performance on the 5-STS. Overall mortality at 5 years was 27% and was significantly higher in the high-risk (29.6%) compared with the low-risk (13.6%) group. Among high-risk patients, mortality at 5 years was significantly worse with poor 5-STS performance (60.5%) compared with non-poor performance (11.1%).
