Depuis le debut de notre travail clinique dans l'Unite Infantile, nous nous sommes inquietes de l'evaluation individuelle et des questions sur le role que jouent les parents tout autant que les bebes dans la pathologie et l'intervention. Dans cet article, nous commencons par une description du chemin qui nous a menes a la selection de DC 0-3 en tant que systeme de classification diagnostique et comment cela nous a aides de maniere cruciale a mieux definir la psychopathologie infantile et a nous guider vers des orientations de traitement. Ensuite, nous presentons les resultats de l'application des axes I et II de DC:0-3 dans notre population clinique dans les annees 1997, 1998, et 1999. Les objectifs de cette etude etaient d'approfondir les connaissances sur la distribution des troubles mentaux chez une population clinique âgee de moins de quatre ans. Nous avons essaye de separer les enfants a risques de developper des troubles psychiques de ceux presentant une psychopathologie actuelle ainsi que l'influence possible de traits demographiques sur cette distribution, de facon a definir une population cible et a concevoir des mesures therapeutiques adaptees. L'utilisation clinique des diagnostics DC: 0-3 est decrite dans le contexte de notre processus routinier d'evaluation et de notre approche theorique de base. Les raisons pour lesquelles les enfants ont ete envoyes a l'Unite et la distribution des diagnostics DC 0-3 de 414 bebes et leurs parents, evalues dans l'Unite entre 1996 et 2000, sont donnees. 25% n'avaient aucun diagnostic sur aucun des 4 axes, 29,5% des bebes avaient a la fois un diagnostic primaire et un diagnostic relationnel, et seuls 5,6% avaient un diagnostic sur chacun des 4 axes. Moins de la moitie (45%) des bebes recurent une diagnostic primaire, et 52% avaient un diagnostic relationnel. Des explications possibles sont offertes, et les problemes auxquels nous avons fait face avec l'utilisation de la classification sont decrits. L'utilisation de la classification DC 0-3 en matiere de recherche est illustree a travers un d'une etude sur les diagnostics et les interactions lors de seances de jeu et de prise de nourriture de bebes qui nous avaient ete envoyes et de bebes qui ne nous avaient pas ete recommandes. Huet, Frédérique Jacquemain","doi":"10.1002/IMHJ.10059","DOIUrl":"https://doi.org/10.1002/IMHJ.10059","url":null,"abstract":"The goal of this article is to contribute to the validity of the Zero-to-Three diagnostic clas- sification (DC: 0- 3) by studying the range, the indices of consistency, the interjudge reliability, and the issue of comorbidity on Axis I. Eighty-five consecutive children under three years of age and their families were assessed in six mental health centers with clinical interviews and developmental testing. Diagnoses on the five axes of the classification system were made after clinical consensus. The Task Force data record DC: 0- 3 was completed for each subject. The use of the classification on a sample of 85 children showed good consistency between the different axes, moderate interjudge reliability, and a high per- centage of associated diagnoses on Axis I. The DC: 0- 3 appears to help the clinician catch the complexity of the clinical situation in planning the therapeutic strategy. Significant gaps remain in the validation of disorders for this age group. The DC: 0–3 represents an alternative nosology for disorders of infancy that attempts to complement existing approaches and assist clinicians and researchers. In this commentary we review research on two disorders of early childhood—Posttraumatic Stress Disorder (PTSD), and Reactive Attachment Disorder (RAD)—to illustrate the promises and problems of the DSM-IV and DC: 0–3 approach to definition and conceptualization of these disorders. In addition we review what is known about RAD and PTSD with regard to key areas of controversy in early childhood psychopathology: (1) continuous versus categorical conceptualization, (2) developmental considerations, and (3) the relationship context. Deux groupes de 15 sujets, âges de 20 a 30 mois, diagnostiques avec RD ou MSDD, selon le critere DC: 0-3 (1994), ont ete compares sur les profils relationnels, linguistiques et comportementaux en utilisant des instruments specifiques. Les resultats ont montre que les enfants RD et MSDD etaient affectes sur toutes les mesures linguistiques avec un degre d'affectation MSDD plus eleve pour les gestes de communication, la parole spontanee et la comprehension verbale. La Checklist de Comportement de l'Enfant d'Achenbach (Child Behavior Checklist, en anglais) a fait etat de differences importantes pour les problemes isoles et somatiques. Seul le groupe MSDD a atteint le score clinique limite dans les problemes globaux, les comportements d'internalisation et d'isolations. Une analyse de discrimination a montre que le nombre de gestes communicatifs et les scores sur les problemes isoles et somatiques predisait correctement 86% des cas. La conclusion est que les enfants RD et MSDD font preuves de differents quantitatives et qualitatives sur bien des dimensions linguistiques, comportementales et relationnelles qui ont ete explorees. Using data obtained from clinical in-depth evaluations of 167 consecutive referred infants, the characteristics and clinical features of this sample of children and parents are examined. The principal issues are children and families' socioeconomic status, number and types of stressors, as well as the symptoms or difficulties exhibited by the infants themselves. A significant proportion of infants come from a deprived socioeconomic background, and many are exposed to violence in the home. The diagnoses given to infants and to the kind of relationship they have with the primary caregiver (Axes I and II) are presented, as well as data as to their physical status (Axis III) and stressors (Axis IV). Many children are diagnosed as having regulatory disturbances, predominantly of the hypersensitive and motorically disorganized type. A significant proportion of mothers exhibited depression. About a third of the babies and primary caregivers had a relationship disorder, the predominant category being the underinvolved type. The implications of these findings are discussed, emphasizing the usefulness and frequency of some categories, while questioning the usefulness of other diagnostic categories.
