Głuszek","doi":"10.5114/ms.2022.117656","DOIUrl":"https://doi.org/10.5114/ms.2022.117656","url":null,"abstract":"Introduction: Laparoscopic surgery of inguinal hernias in children using the PIRS method is a recognized method of treat- ment that has been used for several years. Aim of the research: To assess the effects of inguinal hernia repair performed with the percutaneous inguinal ring suturing (PIRS) method on the volume of the testicles. Material and methods: It was a prospective study conducted between January 2016 and December 2019 in the Department of Paediatric Surgery. Twenty-nine male patients aged 0–8 years were included in the study. Ultrasound evaluation of both testicles, comparing the volume before and after inguinal hernia repair, was performed. Results and conclusions: The ultrasound measurements of the testicular volume after the surgical procedure showed no inhibition of growth with the preoperative sonographic examination and concerning the testis of the opposite, non-operated side. The increase in the median of the volume of both testis’ was observed in cases before and after the elective surgeries, although there was no statistically significant difference. Emergency operation due to the incarcerated hernia on either side did not statistically significantly affect testicular volume in the tested group of patients. None of the laparoscopically operated patients showed atrophy of the testis. CKD is a consequence of common civilization diseases, such as arterial hypertension and diabetes, as well as primary kid- ney and urinary tract diseases of various aetiologies. The pathogenesis of CKD is complex, and the ongoing inflammation and increased oxidative stress (OS) in kidney tissues also play a significant role in the CKD pathophysiological description. Hence, attempts are being made to pharmacologically modify these important pathophysiological pathways. Golusiński","doi":"10.5114/ms.2022.122386","DOIUrl":"https://doi.org/10.5114/ms.2022.122386","url":null,"abstract":"Introduction: In laryngeal cancer, accurate assessment of the laryngeal cartilage is essential to ensure correct staging, prognosis, and treatment. This is especially important given organ preservation in cases with cartilage involvement. Aim of the research: To assess the sensitivity, specificity, and positive and negative predictive values (positive predictive value (PPV), negative predictive value (NPV)) of computed tomography (CT) in detecting cartilage involvement. A second aim was to determine whether the time from the CT scan to surgery influences CT reliability. Material and methods: We retrospectively reviewed data from 233 patients who underwent total laryngectomy from 2007 to 2018 at our institution. We compared the CT findings to the histopathological results to determine the CT reliability in terms of identifying the presence of cartilage invasion and in staging. Results: In the full cohort, CT performance was as follows: sensitivity, 68.8%; specificity, 60.3%; PPV, 65.6%; and NPV, 63.8%. In the subgroup of patients ( n = 68) who underwent surgery 14 days or fewer after the CT scan, the corresponding values were as follows: sensitivity, 82.1%; specificity, 57.5%; PPV, 57.5%; and NPV, 82.1%. CT staging was accurate in 53.2% of cases; postoperative histopathologic evaluation led to upstaging in 73 (31.3%) cases and downstaging in 34 (14.6%). Conclusions: The novel findings of this study show that CT imaging is most accurate when performed no more than 14 days prior to surgery, suggesting that surgery should be performed ≤ 14 days after imaging to maintain the best accuracy of CT. Our patient had undergone a bariatric surgery followed by numerous local complications associated with further surgical procedures and multiple antibiotic treatment. Six months later, she was admitted to our centre with signs of septicaemia. In echocardiography a massive tumour on the aortic valve was revealed. An urgent cardiosurgery operation was performed – the tumour was resected and an artificial valve was implanted. Microbiological analysis of the tumour and blood samples confirmed fungal infection (Candida albicans). Intensive antifungal therapy was continued for 4 weeks. However, during the hospitalization serious embolic complications were observed. Finally, the patient responded well on implemented treatment and was discharged in a good clinical condition. Our case presents an exceptionally rare form of cardiac infection, unexpected in immunocompe- tent patients. Głuszek","doi":"10.5114/ms.2022.122392","DOIUrl":"https://doi.org/10.5114/ms.2022.122392","url":null,"abstract":"Introduction: The PTEN (phosphatase and tensin homologue deleted on chromosome 10) gene is one of the most common- ly mutated suppressor genes in malignant tumours. PTEN protein counteracts the process of carcinogenesis by inhibition of many kinases promoting cell proliferation. Aim of the research: A review and analysis of detected PTEN gene mutations in malignant tumours and identification of the most commonly mutated sites in the PTEN gene. Material and methods: The systematic review included 70 publications. Results: Mutations in the PTEN gene were found in 18% of cases. The largest number of mutations were observed in endo- metrial cancer – 49% of examined patients. Mutations most frequently occurred in exon 5 (49% of examined cases), followed by exon 8 (15% of cases) and exon 7 (13% of cases). Conclusions: The identification of mutations in the PTEN gene in malignant tumours is of great clinical importance – tar- geted treatment with temsirolimus and everolimus is available for patients. Precise and routine method of measurement enables comparison of the scores obtained over time and possible early intervention in order to avoid complications.The aim of the work is to present the pattern for monitoring changes in the shape of the spine in children with postural deformities.MethodsThe study group (n = 211) consisted of the patients with diagnosed shape of the spine deformity, who underwent a four-month therapy, supervised by a physiotherapist. The control group (n = 101) were the children with no shape of spine deformity.The children in the study group underwent a three-dimensional computer analysis of the shape of the spine. The DIERS test was performed in both groups (study and control). In the study group it was performed four times. It involved the measurement of seven parameters enabling a complete assessment of body posture.ResultsStatistically significant scores of the Friedman test for imbalance, pelvic tilt, kyphosis angle, lordosis angle, and lateral deviation were observed. Therefore, a series of post-hoc analyzes were performed using Dunn-Bonferroni tests. It was observed that changes in individual parameters analyzed in the authors' study come up at a different pace.ConclusionsDetailed monitoring of the parameters describing the position of the spine makes it possible to control the course of the treatment process of patients with disorders of the position of the spine.
