The tumor was associated with hirsutism and androgenic excess which normalized following surgical removal. On light-microscopic study, the tumor resembled a well-differentiated endometrioid tumor with luteinized stroma. With further study, the interstitial stromal cell were recognized as Leydig cells, containing scattered Reinke crystals. Tubules and solid cords reminiscent of sex cord elements were also recognized. The dilated glandular elements showed scant apical and luminal positivity with the PAS stain following diastase digestion. Electron-microscopic examination confirmed a dual population of cells, featuring Sertoli-Leydig cell tumor and endometrioid cells. The tumor occurred in the right labium minus of a 26-year-old nonpregnant female. Optic microscopy revealed a clear cell neoplasm with a marked relationship to arterial vessels from which it may originate. Six patients (55%) had extrauterine involvement at the time of presentation; five of these six died secondary to malignancy and one died of other causes. The remaining five patients presented with tumor confined to the uterine corpus and cervix. One of these developed a vaginal recurrence, one died of other causes without evidence of tumor, and three are alive without disease at 15 months, 18 months, and 13 years. The majority of cells in this reaction showed ground glass, eosinophilic nuclear \"inclusions\" simulating herpetic Cowdry type B nuclear inclusions. This was coupled with a history of herpetic stomatitis of the patient and her husband 2 weeks prior to the curettage, raising the question of viremia and transplacental herpetic endometrial infection. Immunoperoxidase staining using the PAP technique directed against Herpes simplex virus types 1 and 2 was negative. Ultrastructural analysis elucidated the true nature of the nuclear \"inclusions\" as pseudonuclear inclusions consisting of invaginations of cytoplasm within the nuclei. Viral particles were not found. The revision of the histology of the biopsies and cones showed in 56 cases the association of CIN with viral cytopathic effects (VCE) attributable to human pappiloma virus (HPV) and in 52 the coexistence of a predominantly flat condyloma. The comparison of the two groups of CIN, with and without VCE, showed that in the first the association had favored in 20% of the cases the histologic overestimation of the severity of the lesion. Of the patients with CIN III, 46% showed additional changes due to VCE. The mean age of the patients with CIN and VCE was 39.8 years and that of the patients with CIN was 48.6 (p less than 0.0001). The exocervix was significantly more often involved by CIN + VCE than by CIN alone (p less than 0.00001). Follow-up studies revealed in both groups the same percentage of residual disease and, preliminarily, a trend to a better control of CIN with VCE. New disease developed more often in the group of patients with CIN without VCE. Cytologic sensitivity for VCE in cervical smears was high (95%) in the cases of CIN II and somewhat lower (81%) in those with CIN III. Cytologic follow-up showed the persistence of VCE in 17% of the patients treated surgically for CIN and VCE. Fifty-nine patients with cytomegalovirus pneumonia diagnosed by open lung biopsy were studied. The sensitivity of frozen section was compared with that of paraffin section, cytology of imprints prepared from the tissue sample, and viral culture. Fifteen percent of the cases were diagnosable at frozen section alone, and 44% by cytology alone. This confirms our previous study and indicates that frozen section unaided by more specific indentification methods provides low sensitivity even in cytomegalovirus-infected cells, which probably show the most easily identified cytopathic effects of any virus. The simultaneous recording of the flowmetry and of bladder and urethral pressures continues to be the most efficient method for the functional demonstration of urinary incontinence. According to the authors, however, it is necessary to evaluate, before the operation, the suitability of the bladder for surgical intervention to avoid any voiding disorders which might occur following surgery.
