The etiology of both cecal bascule and dehiscence is discussed. Contributing factors in this case include increased intra-abdominal pressure from the cecal bascule, and a running catgut closure of the anterior rectus sheath. When the diagnosis of cecal bascule is made, cecostomy with cecopexy is the treatment of choice. {"title":"Uterine, placental, renal biopsies, and fibrin deposits in toxemia of pregnancy.","authors":"S C Sommers","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Histopathologic lesions of toxemia of pregnancy found in 46 first trimester-induced abortions and 82 women with kidney biopsies are reviewed and discussed. Decidual arteriolar vasospasm and degeneration, syncytial trophoblastic degeneration, and chorionic villous fibrin deposits characterize the early toxemic changes of the uterus and placenta. Pure toxemic glomerular, arteriolar, tubular, and juxtaglomerular alterations were present in about 40% of the kidney biopsies. Others had arteriolar nephrosclerosis, chronic glomerulonephritis or both. Two women had renovascular hypertension. Excessive vascular permeability predisposed to fibrinogen leakage and fibrin deposits. {"title":"Müllerian adenofibroma of the endometrium. The tumor arose in the lower uterine segment and was composed of a prominent fibroblastic growth of club-shaped papillae projecting into cystic spaces and into the endometrial canal. The lining epithelium was both mucinous endocervical and endometrial in type. A feature not previously described was smooth muscle within the stromal component. Histology revealed a proliferating endometrioid cystadenofibroma. These are rare neoplasms that generally occur in older women. {"title":"Chemosensitivity testing with CIS-platinum (2) diamminedichloride in squamous cell carcinoma of the cervix uteri.","authors":"K Yannopoulos, C J Cohen, G Deppe, S B Gusberg","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The prophylactic Papanicolaou Test contributes in diagnosing dysplasias and carcinoma in situ of the uterine cervix with a realistic 100% cure. Surgery and radiotherapy improved considerably quantity and quality of life in stages 1 and 2 lesions. However, stages 3 and 4 have shown little improvement in results. Systemic treatment with chemotherapy offers hope for control. Most of the drugs are ineffective against squamous cell carcinoma of the cervix. Experimental and clinical trials with CIS-platinum (2) diamminedichloride (DDP) show effectiveness against cervical carcinoma. In an effort to individualize the treatment, a chemosensitivity testing (CST) is applied analogous to the radiosensitivity testing (RST). Basically, a control biopsy and posttest dose of DDP biopsies are studied to evaluate the response using objective morphological criteria. This safe, relatively simple, clinically applicable approach offers an objective dimension in selecting the patients and in monitoring the treatment. An important observation that gives this study an original authenticity is the fact that radiation and chemotherapy affects the cells in an identical way. Two hundred fifty-six were biopsied and 27 patients were not. Forty-five patients had cone biopsies done during the postpartum period. Effect of delayed assay on pH and lactate.","authors":"S J Wilson, F Silverman, B K Young","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Fetal umbilical artery blood was analyzed for pH and lactate at intervals over a 3-hour period. No significant change occurred in pH over the first 30 minutes, despite a progressive rise in plasma lactate. The pH changes paralleled the lactate in an inverse relationship, but remained within the error of the method for the first hour. Lactate analysis should be considered for clinical use, for confirmation of pH observations. Also pH assays may be used in any hospital with a blood-gas capability, since a 30-minute delay within the assay does not materially alter the results. {"title":"Papillary carcinoma arising from ectopic breast tissue in an axillary lymph node.","authors":"A N Walker, R E Fechner","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A cystic carcinoma was located in an axillary lymph node of a 90-year-old woman. The lymph node also contained cytologically benign mammary ducts and a few small glands that had epithelium cytologically intermediate between the normal epithelium and the neoplastic cells. In a subsequent mastectomy specimen, a small intraductal papillary carcinoma was found, differing cytologically and architectonically from the nodal carcinoma. The nodal carcinoma is considered primary because of its morphologic differences from the mammary lesion, the in situ nature of the mammary tumor, and the transition of benign to atypical epithelium in the nodal inclusions.
