Light- and electron-microscopic studies of tumor morphology were combined with biochemical measurements of testosterone, dihydrotestosterone, estrone, estradiol, and progesterone. Electron microscopy demonstrated cells with the typical ultrastructural features of steroidogenesis. Analysis of extracts from tumor tissue showed high concentrations of progesterone, testosterone, dihydrotestosterone, estrone, and estradiol. These extracts from the ovarian lipoid cell tumor contained the following quantities of steroids: testosterone 450 ng/g tissue; dihydrotestosterone 56 ng/g tissue; estrone 233 pg/g tissue; estradiol 10896 pg/g tissue; and progesterone 512 ng/g tissue. Ovarian vein blood of the ovary with the lipoid cell tumor showed these results: testosterone 3276 ng/dl; dihydrotestosterone 311 ng/dl; estrone 69 pg/ml; estradiol 258 pg/ml; and progesterone 8.1 ng/ml. {"title":"The preoperative diagnosis of adenomyosis.","authors":"A C Weseley","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The charts of 46 patients between the ages 30-50 years having a hysterectomy for a fibroid uterus were reviewed. The diagnosis of adenomyosis was correctly made preoperatively in four patients. The diagnosis was not made preoperatively in 13 patients in whom adenomyosis was demonstrated on pathologic examination of the surgical specimen. The preoperative diagnosis of adenomyosis is difficult. Pain and abnormal bleeding are frequent symptoms. There is uterine enlargement which is demonstrated on pelvic ultrasound. Occasional characteristic findings may be noted on hysterogram. Adenomyosis and fibroids often coexist. The bicornuate uterus (with either one or two cervices) and a single vagina are the most common genital variations, whereas an ectopic pelvic kidney and unilateral renal agenesis are the most common associated renal malformations. The tumor showed a variety of müllerian type epithelia and a spectrum of stromal patterns including nonspecific fibroblastic, and endometrial type stroma. The latter ranged from normal-appearing to low-grade sarcoma. This study emphasizes the relationship of ovarian adenosarcoma to cystadenofibroma, on the one hand, and to malignant mixed müllerian (mesodermal) tumor, on the other. Radiological studies in 350 patients with adenocarcinoma of the uterus treated at the University of Kentucky Medical Center were reviewed. All patients studied in Stage 1A, Stage 1BG1, and Stage G2 were negative. Bone scans performed since 1973 were reviewed. One out of 11 patients studied with G3 lesions had a positive scan. Four out of 41 patients with Stage 2 lesions had positive scans, seven out of 18 in Stage 3, and two out of nine in Stage 4 disease. Four out of 71 patients with recurrent tumor had positive scans. In general, all positive scans were associated with significant symptoms in the patients studied. {"title":"Uterine leiomyomas, serum cholesterol, and oral contraceptives. A preliminary study of epidemiologic differences in Los Angeles, California and Albany, New York.","authors":"H Ratech, M E Stewart","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In order to determine the feasibility of a large-scale study to investigate the relationship of serum cholesterol levels to the presence or absence of uterine leiomyomas, the medical records of 67 women aged from 30 to 39 who underwent hysterectomies at UCLA Hospital, Los Angeles, and Albany Medical Center, Albany, New York were reviewed. The women at UCLA were more likely to have a history of oral contraceptive use than were the women in Albany; they also had higher serum cholesterol values. In both hospitals, increasing age was associated with the presence of leiomyomas, and oral contraceptive use was associated with absence of pathology in the resected uterus. In Albany, but not in Los Angeles, oral contraceptive uterus. In Albany, but not in Los Angeles, oral contraceptive use was associated with elevated serum cholesterol, As expected, uterine weight and presence of leiomyomas were strongly associated. It was concluded that serum cholesterol levels were apparently unrelated to presence of uterine leiomyomas and to uterine weight in this group of women.
