Reproducibility is good. Toxicity, evaluated by cell cultures, was detected in only one chromatographic fraction. The previous adage of not treating this entity for fear of precipitating renal failure has now been replaced by one of aggressive medical therapy as a means to prevent further renal damage. Even when treatment appears to cause renal insufficiency, continued aggressive therapy is indicated as the majority of these people will either stabilize or even improve their renal function. The association of renal failure and benign hypertension is less clear. We feel that if long-term controlled studies could be performed, benefit would be evident. In five diabetics on CAPD, insulin was added to the dialysate in an attempt to maintain continuous control of blood glucose throughout the day. Glycosylated hemoglobin was elevated in seven diabetics prior to the initiation of CAPD (12.0 +/- 2.3%) as it was in six non-diabetics after several months on CAPD. (9.6 +/- 0.90%). With the use of intraperitoneal insulin, satisfactory control of blood glucose could be achieved and the rate of peritonitis was not increased. The occurrence of a steal phenomenon as a complication of a PTFE (Gortex) fistula is often serious enough to necessitate taking down the fistula. This measure of personality was correlated to psychological and physiologic measures of adaptation to hemodialysis. Patients with high external locus of control had poorer overall compliance with higher interdialytic weight gain, but did not show more psychological distress. We evaluated three currently available follow fiber adult dialyzers in vitro and in vivo during treatment of adult patients. All three dialyzers, Gambro GF-120H, Gambro GF-120L, and Travenol CF-1211 were well suited for the dialysis of adult patients.
