The thyroid gland modifies its output in order to provide enough hormones to the mother and foetus. Thyroid autoimmunity and thyroid dysfunction are prevalent in women of childbearing age and may affect the course of gestation and having maternal and foetal clinical consequences. The purpose of the present study was to establish the relationship between euthyroid pregnant women with thyrotropin (TSH) at two levels of the reference range (< 1.2 and between 1.2 and 2.5 mIU/L), and positive or negative anti-thyroid peroxidase autoantibodies (TPO Ab) with the frequency of pregnancy complications and the development of thyroid dysfunction.

Methods

A retrospective study of euthyroid women in their first trimester of pregnancy was performed. TSH, free thyroxine (FT4), total thyroxine (T4), and TPOAb values were analysed. A total of 580 women had positive TPOAb (EP group), and 533 women had negative TPOAb (EN group). The EP and EN groups were subdivided according to TSH levels into EP1: positive TPOAb and TSH<1.2, and EP2: positive TPOAb and TSH between 1.2 and 2.5 mIU/L. Maternal and foetal complications, such as miscarriage, intrauterine death, preterm delivery, and thyroid dysfunction during pregnancy and postpartum were taken into account.

Results

TSH values were higher in EP group vs EN group (<math><mrow><mover><mtext>X</mtext><mo>¯</mo></mover><mo>±</mo><mtext>SD</mtext></mrow></math>; 1.57±0.82 vs 1.16±0.54 mIU/L, P=.01). Its prevalence increases with age and with obesity, and is strongly associated with the presence of metabolic syndrome and increased cardiovascular and malignant diseases. It is caused by an accumulation of triglycerides in liver hepatocytes and muscles, and related to insulin resistance. Its presence is associated with the increase of alanine aminotransferase (alt), although it may not be elevated in advanced disease. There are 5 indexes used for the diagnosis of hepatic steatosis: SteatoTest, fatty liver index, NAFLD liver fat score, lipid accumulation product and hepatic steatosis index, whereas for esteatohepatitis the NASH test, NASH diagnostics, as well as the non-alcoholic steatohepatitis (NASH) score and HAIR (hypertension, ALT, and insulin resistance). In stages of fibrosis AST-ALT index increases as well as ferritin in blood and the NAFLD fibrosis score, which has a high specificity for diagnosis. Abdominal ultrasound is widely available, but its diagnostic sensitivity is lower when there are mild degrees of hepatic fatty infiltration. Computed tomography has a specificity of 100% when fat content is greater than 30%, but the radiation emitted prevents frequent use. Magnetic resonance spectroscopy is the method of choice for the detection and quantification of liver fat content. Liver biopsy is an invasive method that enables appropriate prognostic classification of the disease, but has some complications, and should only be performed in selected individuals: high risk of steatohepatitis or fibrosis risk of laboratory or clinical or other co-existing liver disease. Early identification of NAFLD allows early measures to be implemented in order to reduce morbidity and mortality associated with this condition.

","PeriodicalId":78598,"journal":{"name":"Revista argentina de endocrinologia y metabolismo","volume":"54 1","pages":"Pages 37-46"},"PeriodicalIF":0.0,"publicationDate":"2017-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1016/j.raem.2016.12.001","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"54996429","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Its complications have been studied, but there is little about its effect on hearing. Although there are some hypotheses to try to explain this, the relationship between diabetes mellitus and hearing loss is still controversial.

Objectives

To determine the frequency of hearing loss and the audiometric characteristics of diabetic patients in a Level II Hospital of Chiclayo City during 2015.

Materials and methods

A descriptive cross-sectional study in which audiometry was performed on 185 patients with diabetes treated in the Endocrinology Department. The patients included where between 18 and 70 years, diagnosed with diabetes mellitus type 1 and 2, and lived in Chiclayo city. Simple random probability sampling was used. Those patients with sudden deafness, acute or chronic otitis media, a history or presence of tympanic perforation detected during ear examination, using hearing aid materials, on cytotoxic drugs, family history of hearing loss, and occupational exposure to noise, were excluded. Data were recorded in a conventional audiometry table in which other variables of interest (age, gender, and duration of the disease) were added. Measures of central tendency and dispersion were described.

Results

Almost half (49%) of diabetic patients had hearing loss, mainly slight (35%), bilateral (41%), sensorineural (45%), and with a tendency towards higher-pitched tones (42%). However, recommendations for children with DM1 are based on narrative reviews of the available evidence. Our objective was to evaluate the effectiveness of carbohydrate counting on metabolic control of children with DM1. This study was a systematic review. The search was conducted in PubMed, Google Scholar, and controlled trials of the Cochrane collaboration (CENTRAL), published from January 2000 to September 2015. Five eligible trials out of 261 potentially relevant studies were identified, which include two trials. The quality of the studies was generally poor. The two studies included agreed on the positive impact of carbohydrate counting in reducing HbA1c levels as a measure of metabolic control. In general, the other parameters measured showed conflicting results with no significant differences, except for the high density lipoprotein in one study, higher in the intervention group. Although carbohydrate counting is a promising technique for metabolic control in children with type 1 diabetes, the evidence found in this review is not enough to recommend it.

","PeriodicalId":78598,"journal":{"name":"Revista argentina de endocrinologia y metabolismo","volume":"53 4","pages":"Pages 142-148"},"PeriodicalIF":0.0,"publicationDate":"2016-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1016/j.raem.2016.11.003","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"54996397","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
