A new model of cardiac arrhythmia].","authors":"A I Undrovinas, C January, I Iuriavichus, J Riddle, D Zablotskaĭte","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The effects of calcium current agonist BAY K 8644 on transmembrane potential of guinea pig papillary muscle (PM) and sheep Purkinje fibers (PF) were studied. The action potentials (AP) and contractions (C) were recorded. C both of PM and PF increased (two-four times) in response to BAY K 8644 (0.5-5 microM) addition. The AP duration of PM increased slightly (20%) while PF AP duration increased progressively with the time even at low BAY K 8644 dose (0.5 microM). At 5th min, early afterdepolarizations (EAD) occurred. After 10 min drug application PF became inexcitable at high plateau level. TTX and ethmozin (1 microM) restored AP. Thus, changes in ionic currents balance in PF towards the increase in inward calcium current by BAY K 8644 resulted in additional steady state potential at plateau level. The latter might induce EAD and re-entry around functionally inexcitable PF. {"title":"[The ECG in dilated cardiomyopathy with different degrees of the manifestation of cardiosclerosis].","authors":"F U Gadzhaeva, A Iu Ibragimov, Z Z Dorofeeva, T A Sakhnova, V G Naumov","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>ECG of 46 patients who had died from dilated cardiomyopathy were retrospectively examined in order to evaluate characteristic ECG changes in relation to myocardial sclerotic severity according to autopsy findings. It was found that 1) one of ECG features in this disease is QRS complex morphology in left chest leads in the form of \"rS\", \"RS\" as being independent of cardiosclerotic severity. The functional and structural state of mammary arteries was estimated by the noninvasive Duplex scanning before surgical operation to evaluate the possibility of using them as mammary-coronary bypasses (MCB). Direct blood velocity measurements in the bypasses during aortocoronary bypass (ACB) surgery and MCB were made by ultrasound pulsed Doppler system with a 10 MHz transducer. After 3 months of treatment total cholesterol (TC) level was reduced by 36% (p less than 0.001), LDL cholesterol level by 48% (p less than 0.001), triglycerides level by 19% (p less than 0.001), VLDL cholesterol by 24% (p less than 0.01), whereas the HDL-cholesterol level was increased by 36% (p less than 0.001). Besides, concentration of apolipoprotein A-I increased by 19% (p less than 0.05), apolipoprotein B decreased by 22% (p less than 0.05) and the ratios of LDL cholesterol/HDL cholesterol and TC/HDL cholesterol decreased by 64% and 56%, respectively (p less than 0.001). The side effects of lovastatin were negligible. Investigators are keenly interested in their possible exact registration. The article describes the experience of realization of late potentials registration method based on the method of Simson et al, with the use of a USSR-produced electronic apparatus. The first experience of clinical application of the late potentials registration method in patients with IHD is analysed. Presence of late potentials is correlated with spontaneous ventricular arrhythmias and possible disturbances of left ventricular contractility.
