Data from 56 consecutive patients presenting complete edentulous jaw, aged 18 years or older, treated between January 2008 and December 2013, were evaluated. The outcomes were implant and prosthetic survival and success rates, any complications, and marginal bone loss (MBL). Two-hundred twenty-four implants were placed in 56 patients. During the entire follow-up, 1 maxillary implant but no prosthesis failed during the healing process. Fourteen patients experienced 1 complication each (10 technical, 4 biologic). The overall implant and prosthetic success rate was 98.2% and 82.1%, respectively. All complications were considered as minor and successfully resolved chairside. A mean MBL of 1.30 ± 0.63 mm was observed at the last follow-up. Statistically significant difference was found for postextractive implants (0.79 ± 0.26) vs implants placed in healed sites (1.03 ± 0.46; P = 0.024). Within the limits of the present study, the All-on-4 concept may be a valuable surgical and prosthetic option for the treatment of complete edentulous jaws. However, minor technical and biologic complications can occur. Further long-term prospective data with primary outcomes focused on success rates are needed.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"27 1","pages":"265-71"},"PeriodicalIF":0.0,"publicationDate":"2016-06-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"88355283","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In 2003, Marx identified bisphosphonate treatment as a possible contributor to osteonecrosis, and many studies demonstrating a correlation between chronic biphosphonate consumption and osteonecrosis onset have appeared in the literature since then. The risk of BRONJ rises if the patient is taking other drugs such as chemotherapeutic agents, steroids, and antiangiogenic therapies, and it has been hypothesized that rheumatoid arthritis can be a risk factor on its own. In 2014, the term medicationrelated osteonecrosis of the jaw (MRONJ) was introduced, replacing the common expression BRONJ, as this condition can also result from treatment with other antiresorptive (denosumab) and antiangiogenic therapies. The American Association of Oral and Maxillofacial Surgeons (AAOMS) defines MRONJ as a condition of exposed or probable bone in the maxillofacial region without resolution for greater than 8 weeks in patients treated with an antiresorptive and/or an antiangiogenic agent who have not received radiation therapy to the jaws. For patients who have taken an oral bisphosphonate for fewer than 4 years and have also taken corticosteroids or antiangiogenic medications concomitantly or for those who have taken an oral bisphosphonate for more than 4 years with or without associated steroid therapy, the AAOMS guidelines recommend considering discontinuation of the oral bisphosphonate (drug holiday) for at least 2 months before oral surgery, if systemic conditions permit. The antiresorptive should not be restarted until osseous healing has occurred. Marx et al, in 2007, found a direct exponential relationship between the size of the exposed bone and the duration of oral bisphosphonate use, which was also correlated to the morning fasting serum C-terminal cross-linking telopeptide (CTX) test results; CTX is a bone turnover biomarker, and a stratification of relative risk was seen, with CTX values of less than 100 pg/mL representing high risk, CTX values between 100 and 150 pg/mL representing moderate risk, and CTX values greater than 150 pg/mL representing minimal risk. In addition, drug holidays associated with CTX values rising above the 150-pg/mL threshold were observed to correlate either with spontaneous bone healing or a complete healing response after an officebased debridement procedure. Several clinical protocols have been suggested to treat ONJ, and good results have been shown for the use of blood derivates, such as plasma rich in growth factors (PRGF). PRGF seems to permit achievement of better and faster healing, due to the high concentration of platelets and growth factors in the PRGF. This is why its use has been proposed in many different fields of surgery. Eighteen patients requiring extraction of a tooth followed by a single immediate implant were enrolled. Small volume preoperative CBCT scans were used to plan the position of the implant. A taper screwed-type implant was immediately placed into a fresh socket using only the final 1 or 2 drills for osteotomy. Postoperative CBCTs were used for the analysis of actual implant placement positioning. Measurements of the planned and the actual implant position were made with respect to their position relative to the adjacent teeth. Mesio-distal displacements and the facial-lingual deviation of the implant from the planned position were determined. Changes in the angulation of the planned and actual implant position in relation to the clinical crown were also measured. To statistically summarize the results, box plots and 95% CIs for means of paired differences were used. The analysis showed no statistical difference between the planned position and final implant placement position in any measurement. The CBCT scans coupled with the computer-aided implant planning program along with a final 1-to-2 drill protocol may improve the accuracy of single immediate implant placement for taper screwed-type implants.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"17 1","pages":"233-9"},"PeriodicalIF":0.0,"publicationDate":"2016-06-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"87336597","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This may occur as a result colonization by various anaerobic and facultative bacterial species. If an implant is placed in a site where vegetative bacteria are residing, the implant may fail to integrate if a bacterial colonization proceeds coronally. If the implant apical cortical bone is thin or if there is an apical fenestration, the colonization may proceed through the thin or nonexistent bone through the covering mucosa, relieving inflammatory pressure to create an apical (retrograde) peri-implantitis. Enterococcus faecalis may be the prime culprit in these types of implant failures. After thorough debridement, the implant may be immediately placed after extraction of an endodontically failed tooth, and the patient treated with an appropriate antibiotic. Alternatively waiting for postextraction healing and subsequent implant placement can be done. Nevertheless, either way may allow for the formation of bacterial vegetative forms or biofilms. The implant surface may be colonized when the surface is exposed to the bacteria. Thorough debridement is crucial. Nonetheless, organisms may persist. Randomized controlled trials are needed to elucidate this issue.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"26 1","pages":"224-30"},"PeriodicalIF":0.0,"publicationDate":"2016-04-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"74040200","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Two different 3D maxillary posterior bone segments were modeled. Group 1 was composed of a bone segment consisting of cortical bone and type IV cancellous bone with 100% bone-to-implant contact. Group 2 was composed of a bone segment consisting of cortical bone and type IV cancellous bone including spherical bone design and homogenous tubular hollow spaced structures with 30% spherical porosities and 70% bone-to-implant contact ratio. Four-millimeter-diameter and 5-mm-height dental implants were assumed to be osseointegrated and placed at the center of the segments. Lateral occlusal bite force (300 N) was applied at a 25° inclination to the implants long axis. The maximum von Mises stresses in cortical and cancellous bones and implant-abutment complex were calculated. The von Mises stress values on the implants and the cancellous bone around the implants of the 70% bone-to-implant contact group were almost 3 times higher compared with the values of the 100% bone-to-implant contact group. For clinical reality, use of the 70% model for finite element analysis simulation of the posterior maxilla region better represents real alveolar bone and the increased stress and strain distributions evaluated on the cortical and cancellous bone around the dental implants.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"37 1","pages":"26-33"},"PeriodicalIF":0.0,"publicationDate":"2016-02-11","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"89702646","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Fourteen healthy subjects (7 free-end missing cases in the first and second molars and 7 intermediate missing cases in the first molar region) were included. Six months after the implant prosthesis was placed, an occlusal evaluation was performed with or without the implant superstructure by using Dental Prescale® film and an occluder device. In free-end missing cases, the total occlusal force and area, implant-side occlusal force and area, and implant-side occlusal force and area of the residual natural teeth were significantly affected by the implant prostheses. In intermediate missing cases, the implant-side occlusal force of the residual natural teeth was significantly affected by the implant prostheses. In free-end missing cases, the proportion of implant-side occlusal force, non-implant-side occlusal force, and implant-side occlusal force of the residual natural teeth relative to the total occlusal force was significantly affected by the implant prostheses. In the intermediate missing cases, the proportion of the implant-side occlusal force of the residual natural teeth relative to the total occlusal force was significantly affected by the implant prostheses. The proportion of the occlusal area was also significantly affected. In free-end missing cases, implant prostheses significantly increased the occlusal force and area, which resulted in the proper occlusal distribution. In intermediate missing cases, an implant prosthesis may only improve the same-side occlusal loading of the natural teeth.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"24 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2016-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"84750497","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The aim of the present systematic review was to assess the scientific literature concerning the implants placement in antiresorptive agents-users and the related risk of implants failure and ONJ development. English papers published from January 2003 until December 2014 were identified on Medline database. Titles and abstracts retrieved form electronic search were screened separately by two examiners; thus, original studies dealing with dental implants placement during or before bone antiresorptive agents therapy, and the relative risk of implant failure or development of osteonecrosis were evaluated. Due to the heterogeneity of the included studies and the high risk of bias, there is no evidence of the safe use of oral antiresorptive agents prior or after dental implant surgery. Indeed, implant failure and ONJ development can occur and represent a devastating side effect that should be considered during the treatment. Within the limitation of the present systematic review, high quality studies are needed to provide an adequate level of evidence regarding the safety of dento-alveolar surgery during or before bone resorption inhibition therapy, and the increase predisposition to osteonecrosis of the jaw (ONJ) development. Therefore, antiresorptive agents therapy should be considered a risk factor until further evidence is prospectively obtained.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"30 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2016-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"84132343","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The practices of all implant dentists have been impacted by Dr Misch’s teachings, research, and/or writings. Many individuals and organizations have recently honored Dr Misch, and he certainly is on everyone’s short list of those who have contributed significantly to the field of Implant Dentistry. Journal of Oral Implantology (JOI) would like to salute Dr Misch for the volumes of literature he has contributed to our profession. Dr Misch has authored three editions of Contemporary Implant Dentistry, which has been translated into 9 languages including Japanese, Spanish, Portuguese, Turkish, Italian, and Korean. Contemporary Implant Dentistry is the most popular book in dentistry. He has also written the well-received text Dental Implant Prosthetics. He has published over 250 articles dating from March 1979 to February 2015. Of these, 124 articles have been published journals indexed by PubMed. Dr Misch has 20 articles published in JOI. My favorite JOI publication of Dr Misch’s is ‘‘Workshop Guidelines on Immediate Loading in Implant Dentistry’’. This manuscript is the report of the ‘‘The Immediate Function Consensus Conference’’ chaired by Dr Misch on November 7, 2003, as part of the 52nd annual meeting of the American Academy of Implant Dentsitry. This conference and publication have been instrumental in progressing dentistry from the then standard 2-stage surgical protocol established by Branemark et al to the now frequently performed 1-stage or non-submerged implant protocol. This is just one of the many significant advances that Dr Misch has contributed to our profession. Not only has Dr Misch made significant clinical advancements, but also has supported his innovations with research and given the necessary efforts to publish his work in peerreviewed journals. What is astounding is that as busy as he is, he still finds the time to document his work. His work far exceeds that of one lifetime. Because of his efforts, patients and clinicians not only benefit today, but will benefit for generations to come. JOI congratulates and offers sincere appreciation to Dr Carl E. Misch for all he has done and is doing for Implant Dentistry. Dr Misch, thank you for choosing JOI for 20 of your publications.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"259 1","pages":"625"},"PeriodicalIF":0.0,"publicationDate":"2015-12-10","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"79607979","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
