Three healthy male subjects underwent endurance and incremental lifting tests. Cardiorespiratory parameters (HR, VO2) were monitored with a portable telemetric oxygen uptake analyzer, mechanical parameters were calculated from a lift dynamometer. Rating of perceived exertion was given on Borg's CR-10 scale. Individual fitness profiles for lifting tasks were obtained from the individual relationships between oxygen consumption and heart rate at different lifting frequencies. Physiological responses to repetitive lifting were matched with subjective perceptions. The relationship between perception of exertion and time was described by power functions, Y = a.Xn, in which 0 < n < 1. A single-variable statistical regression for power functions was performed to obtain the individual \"iso-perception\" curves. The iso-perception curve corresponding to a \"moderate effort\" (CR3) identified the individual tolerable threshold for prolonged lifting tasks. {"title":"Functional correlates in the rehabilitation of occupational low back pain.","authors":"P Capodaglio,&nbsp;J Nilsson","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The aim of this preliminary report was to identify physiological parameters related to functional improvement in industrial workers with occupational low back pain (LBP) undergoing an intensive rehabilitation program. Four able-bodied and four LBP industrial workers were evaluated before and after a 4-week rehabilitation program. Dynamic and static strength tests, namely sustained isometric contractions, were performed in conjunction with surface electromyographic (EMG) measurements. Significant pre-rehabilitation differences were found between the two groups. Measured forces changed significantly in LBP subjects immediately after rehabilitation. No significative changes were found in the control subjects after the rehabilitation program. {"title":"[Clinico-statistical investigation about cement dermatitis in Italy].","authors":"L Soleo,&nbsp;C Foti,&nbsp;G Pesola,&nbsp;G Lasorsa,&nbsp;G Candilio,&nbsp;M Marconi,&nbsp;M Nesti,&nbsp;M Passerini,&nbsp;G A Vena,&nbsp;G Angelini","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>To define the prevalence of cement dermatitis (allergic contact dermatitis-ACD-and irritant contact dermatitis-ICD), two retrospective studies have been carried out in Italy: the first one based on the analysis of dermatitis cases ascribable to cement as defined by INAIL (Italian Institute for Industrial Accidents Insurance) from 1984 to 1992; the second one by surveying cement dermatitis cases in workers in the building industry aged between 16 and 70 carried out by Istituto di Clinica Dermatologica dell'Università di Bari from 1988 to 1994. The survey on INAIL data showed that in the years take into account 5,290 dermatitis cases included in item 41 of the occupational diseases table have been defined. About 80% of these cases have been observed in workers working as bricklayers and floor-layers. Therefore, given the remarkable exposure to cement in these professions, the prevalence of cement dermatitis in Italy has been estimated to 6 cases/province/year, even though it has not been possible recognize the clinical form of cement dermatitis. Moreover, the study showed that disabling cutaneous after-effects eligible for compensation have been observed in 30% of the cases defined by INAIL. The allergologic study carried out by Clinica Dermatologica has not only defined the incidence of contact dermatitis (ACD and ICD), in building workers, but it has also extrapoled dermatitis cases due to cement. As a whole, in the years taken into account, 166 occupational or mixed ACD cases and 77 occupational or mixed ICD have been diagnosed. The incidence of contact dermatitis ascribable to cement has equalled 79% among ACD and 88% among ICD, with a ratio of about 2 to 1 in favour of allergic forms. Among the chemicals tested, potassium bichromate showed the highest frequency of cases positive to patch tests. The skin site the most affected by cement dermatitis is hand, followed by upper limbs, lower limbs and feet. We considered a group of 44 Urban Police officers (36 males, 8 females), age 39.7 +/- 9.1, whose principal job requirement is a good energetic and motor availability, for a fitness evaluation through a submaximal treadmill test, with subsequent steps of 6 minutes. During the test, physiological variables (VO2, VE, QR through a metabograph, Hr trough an Ec-monitor and Pa through a manual sphygmomanometer) and subjective evaluations of fatigue and dyspnea were monitored. Studying the individual variables trend it was possible to identify the critical metabolic level that was easily tolerated by each individual. The prescription of such cushions is not a simple task in that the physical and clinical needs of each individual patient must be taken into account. The prevention of pressure ulcers is essential for both the physical and psychological wellbeing of the patient and avoids any slowing of the pace of rehabilitation programmes. The protection of the cutaneous and subcutaneous tissue on the areas of bone prominence is extremely important for those patients who pass much or all of the day seated in a wheelchair. The results confirm that the overall cases of pulmonary diseases are greatly reduced by a correct improvement in the working environment. The diagnosis of swallowing disorders was based on videofluorography (VFG); rehabilitation treatment on Logemann's technique. All pts exhibited a variety of disturbances usually occurring in combination rather than as isolated disorders. The most frequent problem was a delayed swallowing reflex which was found in all pts of the three groups. This fact may reflect the importance of cortical as well as brainstem input to the triggering of the swallowing reflex. Nevertheless some important differences appeared in the groups: the great majority of cranial trauma pts exhibited a severe reduced tongue control while a reduced laryngeal closure was present only in brainstem CVA pts. Moreover, several epidemiological studies on populations exposed to inorganic As by ingestion indicate an increased risk for cancer at other sites and, particularly, for bladder cancer. We describe the case of a petrol chemical worker, who died of metastasized bladder cancer at the age of 52, after being employed for over 30 years in a hydrogen production unit. Analysis of the technological cycle and biological monitoring data revealed an excessive, prolonged exposure to arsenic trioxide (As2O3) vapours and fumes; a solution of this compound was utilized to absorb the CO2 produced by oxidation of the synthesis gas. Careful anamnesis indicated a prolonged contact between the carcinogen and the bladder mucosa, due to the presence of severe urethral stenosis with chronic urinary obstruction. It also appears likely that synergism between As exposure and smoking (5-10 cigarettes per day until 46 years) occurred.
