A pilot study was performed on 25 patients undergoing routine cataract surgery to assess the penetration of the drug into aqueous humor after oral administration. {"title":"Exfoliation syndrome.","authors":"A H Tarkkanen","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The exfoliation syndrome has now been recognised all over the world and even in such ethnic groups as the Skolt Lapps, Navajo Indians, Australian aborigines and all major tribes of the South African Bantu. The prevalence appears rather all major tribes of the South African Bantu. The prevalence appears rather similar in those countries where population-based surveys have been conducted. The exfoliation syndrome is age-dependent. In Caucasians the prevalence is low before the age of 60 years, increasing steadily to about 8 per cent in the group aged 80 to 90 years. However, the Bantus show a 6.4 per cent prevalence in the age group 30 to 39 years. In population-corrected surveys the females show a higher prevalence of the syndrome than do the males. Familial occurrence of exfoliation and population genetics have suggested an autosomal dominant mode of heredity. Suggestions of differences in prevalence of the exfoliation syndrome in different countries and even in different areas of the same country call for further population-based even in different areas of the same country call for further population-based surveys. Recognition of exfoliation is clinically of paramount importance as 20 percent of exfoliation patients show abnormalities of intraocular pressure. Poor pupillary dilatation, fragile zonular fibres and 2 per cent lens displacement may induce difficulties in extracapsular cataract surgery with posterior chamber lens implantation. The level of intraocular pressure will be determined by the rate of aqueous humour production and the facility of unconventional outflow. The facility of unconventional outflow has been shown to be increased in eyes with experimentally-induced hypotony from cyclodialysis, ciliochoroidal detachment, iridocyclitis, or retinal detachment. Aqueous humour production is reduced in eyes with hypotony during the acute phase following cyclodialysis, and in eyes with iridocyclitis or rhegmatogenous retinal detachment. Chronic cyclodialysis or ciliochoroidal detachment does not lead to reduced aqueous humour production, if unassociated with iridocyclitis. Detachment of the ciliary body in hypotony is often associated with, but does not appear to cause, reduced aqueous humour formation. Inputs from the frontal eye field and superior colliculus both project to the brainstem and appear to provide parallel and interacting inputs. The former is more concerned with voluntary shifts of gaze, whereas the latter mediates involuntary reflexive gaze shifts to suddenly appearing visual stimuli. The basal ganglia through the substantia nigra, pars reticularis has direct inputs to the colliculus and appears to have a gating role. {"title":"Ciliary surgery for glaucoma.","authors":"U Demeler","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Ciliary Body excision for the treatment of intractable glaucoma has been performed in 64 eyes (75 operations) between 1975 and 1985. The indications, intra-and postoperative complications and the results are presented. In the majority of the patients the indication for surgery was a secondary closed angle glaucoma with aphakia. All the eyes had been previously operated on at least three times, some having had as many as seven other antiglaucomatous procedures before the partial excision of the pars plicata of the ciliary body. The preoperative intraocular pressures pressures ranged from minimally 35 mm Hg to over 50 mm Hg. Intraoperative complications such as vitreous loss and vitreous haemorrhages from ciliary body vessels were reduced to a minimum by the use of a Fleiringa-ring, a paracentesis and an extensive cauterisation of the ciliary body tissue. The postoperative complication of scleral wound dehiscence was reduced by a double scleral wound closure. Localised damage to the endothelium was observed in two eyes following Nd:YAG laser iridotomy and in one eye following dye laser iridotomy. No endothelial changes were recognised after surgical peripheral iridectomy. In five cases venous stasis retinopathy was evident ophthalmoscopically. Fluorescein angiography was abnormal in 19 cases. The abnormalities found were microaneurysms (16 cases); slow arteriovenous transit time (8 cases); macular oedema (8 cases); leakage from the arterioles (3 cases); disc new vessels (4 cases). Morphological resolution of maculopathy was achieved in 88 per cent of cases. Exudative maculopathy responded well but had a high recurrence rate, whereas oedematous maculopathy had a moderate failure rate and few recurrences. Insulin or non-insulin dependency did not appear to influence the outcome.
