(2) Informal retrospective analysis of approximately 200 children who have been allowed to resume sports 3 days after uncomplicated EA.</p><p><strong>Results: </strong>(1) Thirteen COs responded to the survey. The total estimated EA cases performed were 8617 over 10 years. Four patients were known to have bled mildly; none required reoperation. Eleven of 13 COs supported discharge home 2 hours after uncomplicated EA if other standard discharge criteria were met. Seven of 13 COs supported a return to school the next day if the child had returned to his or her usual state of health; however, 2 COs suggested 7 to 10 days at home. Four of 13 COs supported a return to sports 3 days after uncomplicated EA. (2) None of the 200 children who had been allowed to resume sports 3 days after uncomplicated EA have had any known bleeding. At least 10 toddlers have actually engaged in unplanned strenuous exercise within 4 to 6 hours of EA without adverse effect.</p><p><strong>Conclusions: </strong>Considerable variation exists in Canada in terms of current practice standards regarding return to home, school, and sports after uncomplicated EA. Survey data suggest that, after uncomplicated EA, it is safe for healthy children to return home after 2 hours and to school the next day. Its coordination with respiration is therefore crucial to avoid aspiration and apnea. The aim of this study was to determine the coordination between non-nutritive swallowing (NNS) and phases of the respiratory cycle, including the assessment of the effect of states of alertness in adult sheep. Six animals were surgically instrumented under general anesthesia to record electroencephalography, electro-oculography (state of alertness), diaphragmatic electromyography (EMG), nasal flow (respiration), esophageal EMG, and the thyroarytenoid muscle (NNS). Our results revealed that (1) the highest NNS frequency is observed in rapid eye movement (REM) sleep, especially for bursts of NNS; (2) while NNS begins mainly during inspiration in all states of alertness, REM sleep, however, is responsible for an increase in the proportion of NNS beginning in expiration; (3) the link between inspiration and NNS is not affected by rumination. In conclusion, the link between NNS and inspiration in adult sheep is similar to that of lambs but contrary to previous reports on NNS induced by water in humans. In this article, we present an introductory analysis of our clinical experience with the CO(2) laser at the QEII Health Sciences Centre in Halifax, Nova Scotia. Outcomes from a cohort of 36 patients with glottic cancer staged as Cis-T2 (7 Cis, 17 T1, 12 T2) who underwent endoscopic resection using a CO(2) laser between January 2002 and June 2005 were studied retrospectively. The mean follow-up was 16.2 months (range 0-41 months). At the time of the study, no patient had died of laryngeal disease, one patient had died of another disease, and one patient was lost to follow-up. There had been three recurrences in the cohort: two local recurrences and one recurrence in a regional lymph node. After salvage, all patients with recurrences were disease free at the time of the study. There were three postoperative complications in the cohort: one postoperative myocardial infarction, one case of respiratory distress postextubation, and one case of subcutaneous emphysema. The average time for the procedure was 0.97 hours (range 0.25-2.75 hours). The average postoperative length of stay was 1.2 days (range 0-12 days). Of 35 cases with follow-up, 60% had no reported problems with voice in their last visit and 11% reported consistent hoarseness or weakness. {"title":"Reducing the incidence of revision rhinoplasty.","authors":"Christopher Thomson,&nbsp;Martyn Mendelsohn","doi":"10.2310/7070.2007.0012","DOIUrl":"https://doi.org/10.2310/7070.2007.0012","url":null,"abstract":"<p><strong>Objectives: </strong>To evaluate reasons for revision rhinoplasty in a tertiary care setting to help reduce the incidence of revision rhinoplasty.</p><p><strong>Methods: </strong>Retrospective review of 184 consecutive revision rhinoplasty cases performed by a single surgeon, evaluating the major reasons for patients seeking revision rhinoplasty surgery.</p><p><strong>Results: </strong>The senior author performed 539 rhinoplasty cases during the period January 2001 to June 2003. 184 were revision cases. Within this group 56 were the author's own revisions and 128 had undergone primary surgery by \"other surgeons.\" Major revision indications were airway in 109, crookedness in 70, residual hump in 31 and irregularity in 25 cases. Less common problems included inadequate reduction, tip asymmetry, tip bossae, saddle deformity and change of mind. The incidence of airway restriction and crookedness in the author group was significantly less than in the other surgeon group (p<.0001 and p = .0002 respectively). Other indications did not differ significantly between the groups.</p><p><strong>Conclusion: </strong>The pattern of problems requiring revision rhinoplasty is changing as the improved skills of the surgeon are countered by the increased demands of the patient. The high incidence of nasal obstruction following rhinoplasty reminds us that attention to the airway should not be compromised in the focus on cosmetic outcome. Crookedness of the nose has become a notable complaint by a discriminating public. Given its tendency to recurrence and potential for malignancy, complete removal of the papilloma remains the treatment of choice. Since 1994, we have routinely employed an extended endoscopic approach for the resection of inverted papillomas. We present our methods and outcomes.</p><p><strong>Method: </strong>The study was conducted in an academic tertiary care hospital-based practice. It is a retrospective review of patients treated by a single surgeon. IP was diagnosed by biopsy before surgery. With computed tomography and magnetic resonance imaging, we attempt to identify the sites of origin and extent of IP. Cases with previous medial maxillectomy or invasive disease are treated via the open approach. Otherwise, the extended endoscopic approach is used. The tumour is debulked, and its attachment points are identified. Endoscopic medial maxillectomy is then performed. If maxillary sinus involvement in its anterior, inferior, superior, or lateral portion is suspected, a Caldwell-Luc approach is performed to allow for mucosal excision and complete removal of the anterior lateral nasal wall. When the lamina papyracea or ceiling of the ethmoid or sphenoid sinus is involved, the bony wall is resected. The frontal recess can be approached via a Lynch incision or an endoscopic transorbital approach.</p><p><strong>Results: </strong>Twenty-two patients were referred for IP. Nineteen patients were treated via the extended endoscopic approach. The average follow-up period was 23 (3-66) months. The study was structured as an investigation of voice and resonance characteristics of Croatian children with and without sensorineural hearing loss, using sustained phonation of the vowel /a/ which was recorded using a high-quality tape recorder carried out by two voice clinicians. The samples were digitized and analyzed for frequency and spectral characteristics by EZVoice and Bruel & Kjaer Real-time Frequency Analyzer and high quality sound level meter (mouth-to-microphone distance = 30 cm). Differences were observed in perturbation measures; F0 variability; vocal intensity. Spectral deviations were also observed. Discussion focuses on application of these findings by Croatian speech and hearing specialists with the hearing impaired population. Results indicated the following: measures of jitter were significantly elevated in the hearing loss group as compared to the normal controls. A similar result was observed for measures of shimmer. Lack of voice professional's awareness of importance for making pleasant voice quality of hearing-impaired individuals was the initial idea of this study.
