This study entitled Nationwide Evaluation of X-ray Trends (NEXT) can be used as a guide to determine if your x-ray department meets these criteria. Recommendations are made for the reduction of patient exposure in the event that the Entrance Skin Exposure Guides (ESEG) are exceeded. {"title":"Sonographic patterns of ectopic pregnancy - how do they help in making the diagnosis?","authors":"M C Hill, E Lampton, M B Isikoff, J N Diaconis","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The diagnosis of ectopic pregnancy is clinically difficult. To determine how the different sonographic patterns help in making the diagnosis of ectopic pregnancy, the ultrasound examinations of twenty-three patients were retrospectively reviewed. Four sonographic types of ectopic pregnancy were found: Type 1--Cystic adnexal mass with a fetal pole. Type 2--Cystic adnexal mass with no fetal pole. Type 3--Complex or solid adnexal mass. Type 4--Pelvis fluid with no mass. In Type 1, a specific ultrasound diagnosis of ectopic pregnancy could be made. In Types 2, 3 and 4 the sonographic findings although suggestive of an ectopic pregnancy were not specific. In these cases the diagnostic accuracy was increased if culdocentesis was performed when ultrasound demonstrated fluid in the pelvis. Unlike previous reports, uterine size, endometrial echoes and myometrial echoes were found to be of no value in making the diagnosis. The positive pregnancy test increased the diagnostic specificity of the ultrasound examination. {"title":"Medulloblastomas.","authors":"H Castro-Vita, O M Salazar, C Scarantino, P Rubin, M De Cova","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>\"Analysis of 29 cases evaluated at SMG, Rochester, N.Y.\" This study analyzed 29 patients with the diagnosis of medulloblastoma evaluated at Strong Memorial Hospital, Rochester, New York. Twenty-three patients received combined (surgery and radiotherapy) treatment. Six had surgery alone. Chemotherapy was never employed as a primary treatment, however, two cases were treated with drugs at the time of relapse. The patients having craniospinal irradiation had a better survival rate when compared with the group receiving cranial irradiation or surgery alone. The five year survival rate for the first group was 50% versus 20% for the second group and 0% for the third group. The main cause of failure was recurrence in the posterior fossa. Fifteen patients failed in this area representing 88% of the total failures. Seeding to the spinal cord was determined in 5 patients, four of whom did not have spinal cord irradiation. {"title":"Perforation in malignant disease of the stomach.","authors":"W F Rogers, J Halls","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Perforation of the stomach occurring as a complication of malignant disease is a rare event. Patients often present with a prior history suggestive of benign gastrointestinal disease followed by an abrupt episode of abdominal pain. Silent perforations are unusual, but do occur. The radiographic criteria of the ulcer crater suggest benign disease in the majority of cases an it is only when there is a definitive operation the differentiation from benign disease is difficult and multiple biopsies of the ulcer are necessary. The employment of ultrasonography in the evaluation of suspected pelvic masses is well established. A number of pathologic conditions can be responsible for a pelvic mass in female patients and consideration of the differential diagnosis of such masses has been well documented. The discrimination of a pathologic pelvic mass from an anatomic variation or unusual location of a normal structure can be, however, a potential source of confusion and erroneous interpretation. {"title":"\"Japanese needle\" transhepatic cholangiography. Experience in 98 cases and review of the literature.","authors":"C A Muhletaler, A J Gerlock","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Percutaneous transhepatic cholangiography (PTC) using the \"Japanese needle\" technique as described by Okuda et al. was performed in 98 patients. The overall success rate in cannulating bile ducts with this technique has been 91.8%. Nondilated bile ducts were successfully cannulated in 82.8% of the patients and nondilated bile ducts were successfully seen in 96.8% of the patients. Ten patients were found to have by PTC an obstructive jaundice without significant dilatation of the common bile duct or intrahepatic radicles. Exploratory laparotomy was avoided in 19 of 23 patients with nonobstructive jaundice as demonstrated by this technique. Major complications were present in 2 patients, no deaths were attributed to this procedure. The diagnosis requires endoscopic confirmation of the erosions.
