Our results indicate increased renal production of PGE2 in nephrotic patients. This contributes to the maintenance of renal function, probably by counteracting an activated renin-angiotensin system and could be accompanied by a simultaneous and deleterious enhancement of the degree of proteinuria. Nevertheless, the participation of angiotensin II in this last even cannot be excluded.</p>","PeriodicalId":76354,"journal":{"name":"Proceedings of the European Dialysis and Transplant Association. Intravenous antibiotic pulses plus oral AHA achieved urine sterilisation in all. Then oral chemotherapy plus AHA was given for a mean period of 10.8 +/- 5.4 months. In four patients, the urine remained sterile, but in all the patients urinary pH remained below 6.4 and urinary NH4+ below 40 mg/dl. The patients are divided into 33 non-diabetic and 32 diabetic and the latter are subdivided into blind and sighted. Cumulative actuarial survival on CAPD calculated at five years was highest for blind diabetics, intermediate for non-diabetic and lowest for sighted diabetics. Blind diabetics had the lowest rate of drop-out from CAPD. A rise in multimeric IgA-IC occurred in three out of seven controls with a peak at two to four hours after the meal, being cleared thereafter. The amount of multimeric IC present at fasting in four of six patients diminished at two to four hours after food challenge reaching a new peak around six hours. In both controls and patients, IC containing antibodies against diet antigens (e.g. ovalbumin) paralleled those of multimeric IgA-IC. In controls the specific polymeric IgA-IC presented a maximal peak with distribution similar to multimeric IgA-IC, but with a faster disappearance from the circulation. By contrast, polymeric IgA-IC remained elevated 24 hours after food ingestion in most patients. These results suggest that a defect in the hepatic clearance of circulating polymeric IgA-IC exists in patients with IgA nephropathy.</p>","PeriodicalId":76354,"journal":{"name":"Proceedings of the European Dialysis and Transplant Association. In the diffusion phase of sequential therapy, a decrease in mean arterial pressure (MAP) and total peripheral resistance (TPR) was noted despite no change in circulatory plasma volume (CPV) and CO. In the ultrafiltration (UF) phase, MAP recovered with an increase in TPR despite the decrease in CPV and CO. Muscle vascular resistance increased in UF phase but not in the diffusion phase. The results suggest that the decrease in CPV through UF resulted in CO decrease, but that the compensation mechanism for the decrease is impaired by diffusion.</p>","PeriodicalId":76354,"journal":{"name":"Proceedings of the European Dialysis and Transplant Association. In 12 out of 24 cases ultrasonically guided fine-needle aspiration biopsy of the tumours was performed in order to obtain a cytological specimen. Thirteen specimens were biopsies, the remaining came from autopsies. The presence of refractile particles was found in 55 per cent of biopsies and 100 per cent of autopsies. Its presence was not constantly associated with morphological lesions of the liver. The stone-formers excreted significantly less polyanionic macromolecules in their urine than did the normals, the difference being attributable to the lower excretions of glycosaminoglycans (GAGS), ribonucleic acid (RNA) and Tamm-Horsfall mucoprotein (THM), all of which are precipitable with alcian blue. The relative potencies of the various inhibitors measured under 'whole urine equivalent' conditions using a batch crystallisation system, showed that the order of inhibitory activity towards CaOx crystal agglomeration was RNA greater than GAGS greater than THM greater than pyrophosphate (PPi). This paralleled the order of ability of these inhibitors to produce a high negative zeta potential on the surface of CaOx crystals.</p>","PeriodicalId":76354,"journal":{"name":"Proceedings of the European Dialysis and Transplant Association.
