Endoesophageal pH determinations using Heidelberg telemetring capsule.","authors":"T Banciu,&nbsp;E Sorian","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Gastroesophageal reflux (GER) was studied in 86 chronic ethanol consumers, by determination of endoesophageal pH using a Heidelberg telemetring capsule. All the subjects were subjected to radiologic and endoscopic explorations in some cases completed by esophageal mucosa biopsy. In 49 subjects (56.4%) GER was present in various degrees according to the calculated score; medium and high intensity reflux in 37 cases (44%). GER was symptomatic in 62% of the cases and 30 patients (35.7%) presented esophagitis with columnar epithelium metaplasia in 3 cases. The intensity of reflux and the presence of esophagitis were found to be correlated with the degree of alcoholic \"impregnation\". The peculiarities of anatomical branching pattern, of the intimal thickening, muscle columns development, of age-related changes, in the intima/media thickness ratio, as well as the segmental characteristics in susceptibility to plaque development and in the plaque pattern are presented. {"title":"Comparative study of insulin antibodies in diabetic patients with primary insulin-dependence (type I) and secondary insulin-dependence (type II).","authors":"C Ionescu-Tîrgovişte,&nbsp;Z Mirodon,&nbsp;D Cheţa,&nbsp;E Sântu,&nbsp;M Simionescu,&nbsp;A Nicolau,&nbsp;I Mincu","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Insulin antibodies (% binding) were determined by RIA method in 404 insulin-treated diabetic patients divided into two groups: (A) primary insulin-dependent patients (Type I diabetes): 300 cases, 170 M, 130 F, mean age +/- SD 29.2 +/- 7.5 yrs, disease and insulin treatment duration 7.7 +/- 6 yrs: (B) Type II diabetic patients needing insulin (secondary insulin-dependence): 104 cases, 47 M, 57 F, aged 53.4 +/- 9.2 yrs, duration of diabetes 13.1 +/- 8.3 yrs, and of insulin treatment 3.1 +/- 2.1 yrs. Both groups of patients were with the same types of insulin preparations. In 297 cases, all belonging to group (A), fasting C-peptide was also determined. It was found that R-IFN in doses of 20,000 IU/g has practically similar results as G-IFN in doses of 10,000 IU/g therefore half the dose of the former product. Thus both preparations reduced significantly the mean duration of a herpetic dermatitis attack and both proved more efficient on smaller lesions (less than 2 sq.cm) than on larger ones (over 2 sq.cm). Likewise with both preparations the reduction of the healing period was more marked when administered in the first 2-3 days after onset. It was also found that the arrest of new vesicle appearance after two days of treatment is a reliable clinical criterion for the estimation of IFN capacity to block effectively viral multiplication in the host's epithelial cells. All the patients studied, except patient with cancer of the lung, showed a tendency toward a low count of such cells significant in patients with precancerous states of the larynx, cancer of larynx, cancer of the larynx after radiotherapy and Hodgkin's disease and nonsignificant in cancer of the stomach, cancer of the large intestine and plasma cell myeloma. In patients with cancer of the lung the count of lymphocytes with intact beta-glucuronidase-positive lysosomes was significantly increased as compared with that in healthy subjects. Different forms of cardiac involvement could be detected in 51% of the patients with PSS and in 18.5% of those with PM. The most frequent cardiac dysfunctions were disturbances of rhythm and conduction probably due to the coronary vascular changes in PSS and to processes of fibrosis and degenerescence of the specific myocardial fibers in PM. Myocardial lesions expressed by myocarditis and cardiomyopathies are not infrequent and have important prognostic implications. In the group of patients studied the valvular cardiopathies and clinically evident pericardites were rare. Cardiac involvement clearly proved more frequent in PSS - in which the pathogenic mechanism is mainly vascular - than in PM.
