{"title":"[Pathohistologic studies of serial sections of squamous cell cancer of the oral cavity and oropharynx following cytostatic induction therapy with bleomycin and cisplatin].","authors":"R Bettinger, E Meyer-Breiting, A Klima","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>43 patients with advanced squamous cell carcinoma of the oral cavity and oropharynx were treated preoperatively by a combined chemotherapy with bleomycin and cisplatinum. After surgery histological serial sections of resection specimens were examined. In no case did the tumour tissues show a complete response. After induction chemotherapy 35 patients had histologically partial response - shrinkage by more than 50%. In these cases histological evaluation revealed tumour areas with pseudocysts containing on the one hand necrotic material and showing keratinisation on the other. A tumour was also found histologically under regrown epithelium in 10 cases where complete tumour remission was postulated clinically. Eight patients showed histologically a minor response - shrinkage below 50% with only little necrotic areas. Exophytic tumours responded better to chemotherapy than endophytic tumours. In the literature different degrees of the disease are described which range from simple hoarseness to dangerous states of extreme shortage of air. {"title":"[Morphologic changes in the epithelium of myocutaneous pectoralis major islands flaps. A light and electron optic study].","authors":"A Beigel, C Schubert, H Rudert","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>On a long-term basis, the epithelium of pectoralis major flaps incorporated in the mouth and the oropharynx is macroscopically and microscopically very similar to the sourrounding mucosa. Light microscope and electro microscopic investigations show that the type of keratinization of the epithelium of this flap has changed from orthokeratosis to parakeratosis which is typical for the mucosa of the mouth. {"title":"[Echo laryngography. A contribution to the method of ultrasonic diagnosis of the larynx].","authors":"G Böhme","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>This article describes a method for sonographic investigation of the larynx, the suggested term for which is \"echo laryngography\". It is more purposeful if M-mode is employed as well as B-mode for the assessment of laryngeal function and performance. Intra- und extralaryngeal sound windows are described and a discussion concerning their access by means of a systematic transversal section from caudal to cranial and coronary and sagittal sections in the vertical plane will ensue. The varying degrees of ossification of the laryngeal cartilage and the air-filled parts of the larynx make sonography more difficult. The transverse and vertical epiglottis function test which we developed extends the range for sonography of the larynx. {"title":"[Primary larynx carcinoid. Only 20 cases of laryngeal carcinoid tumours have been reported. Since histological diagnosis is difficult, this unusual neoplasm was often misdiagnosed as an undifferentiated carcinoma. The case of a 72-year old man is reported, who was admitted to hospital after suffering from hoarseness and left-sided otalgia for 4 weeks. Indirect laryngoscopy showed a tumour at the left ary region. The tumour was removed endoscopically. The light and electron microscopic characteristics and the results of the histochemical examinations are reported. In case of a superinfection with pathogenic microorganisms the degree of mucositits can make an interruption of cancer therapy imperative. It may also cause severe complications such as fulminating fatal infection, local spreading to the oesophagus and even mediastinitis as well as disseminated fungal infections. We investigated the effect of the systematic local combined application of H202, povidone iodine, nystatine and dexpanthenol solution for the prophylaxis and therapy of mucositis induced by radiochemotherapy. During 16 months of application of our scheme symptoms of mucositis appeared only to a minor extent. {"title":"[Results of simultaneous radio-polychemotherapy in advanced inoperable cancer of the head and neck].","authors":"T P Wustrow, T G Wendt, A Schalhorn","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Sixty untreated patients with advanced carcinoma of the head and neck (stages III = 11 and IV = 49) were treated simultaneously with three cycles of polychemotherapy and radiation. Chemotherapy consisted of cisplatinum (DDP) 60 mg/m2 after prehydration with saline and mannitol, 5-fluorouracil (5-FU) 350 mg/m2 and folinic acid (FA) 50 mg/m2 on day 2 as a bolus and a continuous infusion of 5-FU 350 mg/m2/24 h and folinic acid (FA) 100 mg/m2/24 h from day 2-5. Concomitantly, accelerated hyperfractionated radiation was administered from day 3-11. Two fractions per day with 1.8 Gy each were given, 13 fractions in 9 days. This cycle was repeated two times on day 22 and 44 with an interval without treatment from day 16-21 and 34-43. Total radiation dose was 70.2 Gy in 51 days. Acute toxicities (WHO grade II and III) consisted mainly of leucopenia (75%), thrombopenia (15%), weight loss (mean 5.8 +/- 3.7%) and mucositis (66%). Grade IV was never reached. Except for 3 patients, who died during treatment due to fatal tumor bleeding or carotid rupture, all were able to finish the treatment with reduction in chemotherapy in only 95% (DDP) and 98% (5-FU) with no changes in the radiation protocol. Evaluation of tumor response at 3 months after end of treatment showed 68% complete and 32% partial responses. 5 patients developed distant metastases. Survival with local control after 12 months was 80.8% and 71.3% after 24 months. 1 and 2 years disease-free survival was 70.8% and 62.1%. Total survival irrespective of cause of death was 77.9% and 57.2% after 1 and 2 years. {"title":"[The angioarchitecture of the septal mucosa. Experimental studies of the rabbit].","authors":"G Grevers, U Heinzmann","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Histological and transmission-electron microscopical studies alone cannot answer all questions regarding the microcirculation of the nasal mucosa. In the present study additional morphological techniques were used to demonstrate the course of the different parts of this peculiar vascular network. Using the \"injection technique\", the course of every single vessel of the nasal septum could be shown, but the character or size of these vascular structures was not easy to identify. Scanning electron microscopic investigations were able to verify vascular junctions of all sizes as well as intraluminal aspects of different vessels and the relations between all parts of this endonasal circulatory system. Lower magnifications also showed the tissue arrangement in the nasal septum. In higher magnifications vessels of arterial or venous character could be identified by distinct morphological features. Additional studies were carried out using vascular corrosion casts. On studying these structures, the entire course of the different parts of the endonasal vascular system could be observed. In addition, the arrangement of the different capillary networks of the nasal system, mentioned by several earlier investigators, could be demonstrated without doubt. This technique also allowed to distinguish between arteries and veins by the appearance of their endothelial cell nuclei. Summing up our findings it can be pointed out that all techniques used in this study are useful to understand the complicated angioarchitecture of the nasal mucosa.
