{"title":"What do families, staff, and stakeholders think about children's dream fulfilment?","authors":"Jayne Galinsky","doi":"10.3109/01460862.2014.946544","DOIUrl":"https://doi.org/10.3109/01460862.2014.946544","url":null,"abstract":"<p><strong>Background: </strong>This article reports the results of a service evaluation designed to assess children's dream fulfillment charity, Dreams Come True.</p><p><strong>Aims and objectives: </strong>With the objective to gain the opinions of those who have used and are involved with the charity, parents (n=10), staff (n=10), and key informants (n=18) were asked what the charity does most and least well.</p><p><strong>Method: </strong>Focus group meetings using the nominal group technique were held for parents and staff and an email survey was sent to key informants.</p><p><strong>Results: </strong>Conclusions, in the form of voted responses to the question of what the charity does most and least well are presented.</p><p><strong>Conclusions: </strong>Popular themes arising from each group were the benefit of dreams, the importance of empathy, staff communication, and fundraising. A total of 300 children (ages 18 months to 16 years), whose abused mothers sought safe shelter or a protection order for the first time, were studied. Data revealed internalizing behaviors, such as depression and externalizing behaviors, such as bullying decreased 4 months after mothers obtained help. Children's scores from the shelter group indicated more dysfunction. There are many elements influencing the effect of health promotion services. One is the extent of parental participation. However, research in this area is limited. Focus group data were content analyzed. Six themes were conceptualized. They are Equity, Equality, Evolution, Engagement, Consultation, and Continuity. We concluded that a strategic approach in partnership with families is required for health promotion activities for children. The study questions were: What diabetes treatment is being delivered? Who is providing the treatment? Where is the treatment given?</p><p><strong>Methods: </strong>A cross sectional, descriptive approach was used to collect data from parents (66) of children with type 1 diabetes attending an Australian primary school (kindergarten-Year 2). An online self-administered questionnaire was designed in Survey Monkey and was available via a dedicated Facebook page. Data were analysed using statistical analysis (SPSSv21).</p><p><strong>Results: </strong>Blood glucose testing was occurring for all children, with 49% of children self testing. 77% of children were receiving an insulin bolus or injection at school. 34% was provided by the child and 53% of insulin was given via pump. Teachers, parents and teacher's aides also provided insulin at school. There was a statistically significant association between the number of children receiving insulin at school and the insulin delivery device, χ(2 )= 16.75, df = 1, p ≤ 0.000). Children using insulin pump therapy were more likely (97%) to receive insulin at school than children who used injections (55%). Children who were able to self-administer insulin were more likely to receive insulin (93%) at school than children who were unable to self-administer insulin (65%) (χ(2 )= 7.38, df = 1, p = 0.007) 81% of children received diabetes treatment in the classroom, with the remainder in the school administration office.</p><p><strong>Conclusion: </strong>Insulin administration across Australian primary schools was inconsistent. Not all children were receiving the recommended insulin treatment. Suitable preparation can decrease the pain and fear of needles in hospitals; however, few have examined how such preparation affects healthy children. This study examined whether learning with needles decreases fear of needles and changes motivation to get vaccinations in school-age children and the possible association between fear of needles and motivation toward vaccinations.</p><p><strong>Methods: </strong>This study included children participating in the \"Let's Be Doctors\" event, which was held in 4 child centers in Tsukuba city, Ibaraki, Japan. In this event, children learned about injections and how a vaccine works, and injected a vaccine (water) into skin (sponge) using a real syringe and imitation needle. Data were collected just before and after the event by anonymous self-assessment questionnaires that used a 4-point Likert scale to assess fear of needles, motivation to get vaccinations, recommendation of vaccinations, and fear toward doctors among the children. Answers were divided into two categories for statistical analysis.</p><p><strong>Results: </strong>In total, 194 children participated in the event and 191 children answered the questionnaire (response rate 98.5%). We analyzed 180 subjects, comprising 79 boys (43.9%) and 94 girls (52.2%), mean age of 8.1 ± 1.0 years. The number of children reporting a fear of needles decreased from 69 (38.3%) before the event to 51 (28.3%) after the event, and those unwilling to get vaccinations decreased from 48 (26.7%) to 27 (15.0%). Children who reported fear of needles before the event were more unwilling to get vaccinations than those with no fear of needles (36 [52.2%] vs. 12 [10.8%]), while after the event the number of needle-fearing children unwilling to get vaccinations decreased to 19 (27.5%).</p><p><strong>Conclusions: </strong>Children's fear of needles and unwillingness to get vaccinations were decreased after experiencing a learning event with needles. Without researcher-clinician collaboration, the clinical relevance of any new instrument cannot be ensured (Murray et al., 2013). We appreciate the work by Leonard et al. (2013) that explored the predictive ability of an abbreviated pressure ulcer risk prediction tool. Specifically, the researchers sought to determine whether using a modified version of the Glamorgan Pressure Ulcer Risk Assessment Scale (mGS) would alter the pressure ulcer risk identification when compared to the original Glamorgan Pressure Ulcer Risk Assessment Scale (GS) (Willock et al., 2009). Prospective data were collected from a convenience sample of critically ill pediatric patients (n1⁄4 133) only. However, the GS was designed for use with the general pediatric inpatient ward population. The mGS contains only two items (mobility and equipment) whereas the GS contains nine items (mobility, equipment, significant anemia, persistent pyrexia, poor peripheral perfusion, inadequate nutrition, low serum albumin, weight, and incontinence). Using a 2-item instrument would be less burdensome for busy clinicians than using a 9-item instrument. While parsimony is a desirable characteristic of any instrument used in the clinical setting, we think the authors have now thrown the baby out with the bathwater. It makes sense, given how the GS and now the mGS are scored, that reduced mobility and/or devices pressing or rubbing on the skin, will place a patient at risk for an immobility-related or devicerelated pressure ulcer. But, do we really need a tool to tell us this? Do we need to differentiate the level of risk from ‘‘at risk,’’ ‘‘at high risk,’’ and ‘‘at very high risk’’? It is highly probable that a large percentage of students in each class in Irish schools have asthma and are required to manage symptoms during the out-of-home period. Young people with chronic illnesses such as asthma find themselves labelled and marginalized due to a lack of awareness of others about their condition.</p><p><strong>Aims and objectives: </strong>Young people who feel supported by their peers have been shown to have higher levels of self-efficacy and fewer exacerbations of their asthma symptoms.</p><p><strong>Methods: </strong>An on-line asthma e-learning program was developed to provide support to teenagers who have asthma, and help them inform their classmates and friends about asthma. A quasi-experimental approach was used to pilot the educational intervention.</p><p><strong>Results: </strong>The results of the pilot demonstrate that the program is effective. Teenagers who have asthma found the e-learning program to be informative and relevant to them. However, available studies provide conflicting evidence on what kind of flush/lock solution should be used to increase the life of PVCs in children.</p><p><strong>Objectives: </strong>To describe the clinical behavior of nurses working in Italian pediatric units with regards to PVCs flushing and locking practices.</p><p><strong>Methods: </strong>Cross-sectional study. Nurses were invited to participate using the network of the Italian Society of Pediatric Nursing Science. Those participating completed an online questionnaire available on a website established for this specific purpose. Results: 405 questionnaires were completed.</p><p><strong>Results: </strong>The majority of nurses reported using Normal saline solution (NS) to flush 22 gauge PVCs: 77.6% in children up to 6 months of age, 74.7% in children 6 months to 2 years, and 74.6% in children over 2 years. Nurses tend to use heparin solutions (HS) more frequently when a smaller gauge PVC is used (24 instead of 22) and when access is less frequent. The use of HS for PVC lock is more common in onco-hematology units (54.5% in children over 6 months with 24 gauge PVC), pediatric surgery units (35%), and in short-stay units (55.6%), whereas NS is used more frequently in Intensive care units (9.4%) and neonatology units (12.2%).</p><p><strong>Conclusion: </strong>Although the majority of respondents use NS, we found a high variability in practices among Italian nurses.
