Mean age of these patients was 69,9 years (54-91 years). Mean rate Total PSA was 134,8 ng/ml (6-2500 ng/ml). Repartition according Gleason score has revealed that only 35% of tumors were well differentiated. The occurring of digestive haemorrhage in post operative followings evokes spontaneously a stress ulcer. We report an observation of a child who presented on fourth day delay after operation an ulcer of the anterior duodenal bulbar face and a haemorrhage of the posterior bulbar face. A 7-year-old girl with no particular pathological antecedent was admitted for abdominal pain, bile vomiting and constipation evolving since 6 days. Clinical examination revealed a general state thickening, an infectious syndrome, a meteoric and general abdominal sensitivity. The abdominal radiography without preparation showed a pneumoperitoneum. The surgical exploration discovered a perforated ulcer on the bulbar anterior face. A simple closure associated with omental patch was performed. Four days after operation, she presented an abundant digestive haemorrhage with shock. The resuscitation did not improve the patient's general state. The upper digestive endoscopy revealed a haemorrhage of the posterior bulbar face. An adrenalin injection stopped the bleeding. The treatment by neutron pump inhibitors and an eradicating treatment of Helicobacter pylori permitted the healing of the ulcers. The occurring of digestive haemorrhage in the followings of surgical intervention for perforated ulcer involves an upper digestive endoscopy. {"title":"[Complications of transrectal prostate biopsy].","authors":"B Diao, F A Kaboré, B Fall, Ondo C Ze, A Odzébé, H Berthé, M Ali, P Banderembako, M Bâ, B A Diagne","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objectives: </strong>The aim of this study was to report the transrectal prostate biopsy complications, to identify the factors involved in their occurring and to insist on their prevention.</p><p><strong>Patients and methodology: </strong>It is a prospective study interested on patients managed in the urological and andrological service in Aristide le Dantec Hospital in Dakar for prostate tumour from February 2002 to March 2004 and in whom the indication of prostate biopsy was performed. All the patients fulfilling the including criteria were taken in care according to a same protocol that was a transrectal sextant biopsy This act was performed under preventive antibiotherapy based on fluoroquinolones started one day before and going on three days after biopsy associated to a rectal preparation performed the same day of biopsy At the end of the biopsy the patient was managed externally during three days after biopsy Following parameters were studied: age of patients, the histopathological results and the post operative complications.</p><p><strong>Results: </strong>Two hundred and four (204) biopsies were performed. The median age of the patients was 69.1 years with the extremes of 42 years and 93 years. The global rate of complications was 13.5% with a majority of bleeding. There no major complication registered. Rectal and urethral bleeding was over in 24 hours. The complete urine retention was under the form of a unique happening complication. These prostheses are made of alloys in which cobalt occupies a significant proportion. Being placed in an electrolytic (saliva) and biological (bacteria) medium, they are prone to degradation by phenomena of corrosion. In order to study in vitro the electrochemical behavior of the alloy cobalt-chromium, we subjected this metal part to anodic polarization in a synthetic salivary solution.</p><p><strong>Material and method: </strong>The electrochemical study was carried out at the biophysics laboratory of Casablanca medicine faculty. In order to describe complications of sickle cell disease, authors reported frequency and course of anemic manifestations.</p><p><strong>Method: </strong>This is a descriptive study based on retrospective analysis of data about 338 patients with sickle cell disease collected in the Service d'hematologie Clinique of Yopougon Teaching Hospital over a period of 11 years (March 1994 to September 2005).</p><p><strong>Results: </strong>Mean age of our patients was 21.34 years, ranging from 7 months and 62 years.Majority of patients (68.93%) are aged 15 years or more. Male patients are predominant, with a sex-ratio of 1.36 and most of our patients (98.82%) are from low social condition. Anemic complications were the most occurring complications in our patients with a frequency of 18.78%. Acute anemic complications are the most frequently noticed (87.87%), among which acute crises of deglobulization are mainly present (94.27%). Chronical anemic complications are noticed in 23.67%of our patients and consist mainly of gall bladder lithiasis (20.12%). Death occurred in 10.35% of our patients and was due to anemic complications in 42.86% of cases.</p><p><strong>Comments: </strong>The predominance of acute anemic complications may be due to the comorbidity observed in most of our major sickle cell disease patients. It may turn a chronical haemolytic anemia in acute hemolysis which is a major complication.</p><p><strong>Conclusion: </strong>Sickle cell disease has become nowadays a disease of little letality. Cortico-resistant nephrotic syndrome is the main manifestation. We report epidemiological, clinical and pathological aspects of FSGS in Dakar. We report the results of a retrospective study about focal segmental glomerulosclerosis (FSGS) identified from 258 kidney biopsies performed in the medical clinic 1 of A. Le Dantec hospital from January 1993 to December 2003. FSG is found in 134 cases (52%), membranous glomerulonephritis in 32 cases (12,4%), minimal change disease in 20 cases (7.7%). Ninety eigths files were exploitable. FSGS has male gender predominance with a sex ratio of 3. Median age of patients is 28 years (15 and 79 years). Symptomatology is dominated by oedema in 86 cases (87,7%), hypertension in 12 cases (12.2%), hematuria in 5 cases (5.1%), nephrotic proteinuria in 65 cases (66,3%) and no nephrotic proteinuria in 33 cases (33.6%), renal failure in 25 cases (25%)and leucocyturia in 18 cases (18%). FSGS involving more than 50% of glomeruli is encountered in 41 cases (42%), severe interstitial fibrosis is associated in 26 cases. Different pathological aspects are: classical FSGS in 88 cases (88.7%), FSGS \" collapsing\" in 7 cases (7.1%), FSG \"tip-lesion\" in one case, FSGS associated to membranous glomerulosclerosis in 2 cases and to diabetic glomerulosclerosis in one case. FSGS is primitive in 88 cases (89,8%) and secondary in 10 cases (10.2%). FSGS is the most common primitive glomerulopathy in Dakar. Nephrotic syndrome is the main manifestation of this disease. Its measurement is made either remotely , or after stimulation of pentagastrin secretion which the answer is amplified at the carrier of CMT. The aim of this study was to estimate a chimioluminescent method by comparing it with an immunoradiological method, manual, used as reference. Correlation study was done.</p><p><strong>Materials and methods: </strong>Two hundred and sixty three serums (263) were tested among which 64 resulting of healthy subjects and 199 resulting of patients affected) by medullary thyroid carcinoma. Statistical analysis of results was made by a study of correlation with the software OriginLab version 7.0. The manual technique used as reference method is radioimmunological (Elsa hCT, international Cisbio, Gif on Yvette, France). It was compared with a chimioluminescent technique (Nichols Advantage, Nichols Institute Diagnostics, CA, the USA).</p><p><strong>Results: </strong>The coefficients of correlation obtained between both tests were: r = 0.76 (exactness study), r = 0.91 (after stimulation), r = 0.95 and 0.79 (staged samples), r = 0.99 (M TC patients).</p><p><strong>Conclusions: </strong>Both techniques correlate strictly and significantly. One can define it as a level of blood pressure (140/90 mmHg) beyond which the cardiovascular risk is important and requires a therapeutic action. One of the etiopathogenic assumptions selected is a disordered state of the relation pressure natriuresis wich is the major mechanism of control of long-term arterial pressure. The diagnostic means and of monitoring were the subject of consensus on the level of scientific societies what is not the case of the therapeutic strategy . In more of lifestyle change, the patients with renal diseases generally require an association of antihypertensive drugs whose the thiazide diuretics ones must have a leader place. The infectious complications are common in sickle cell anaemia, which reaches 1% of the population in Senegal. The objective of this study is to assess periodontal conditions in young Senegalese sickle cell anaemics.</p><p><strong>Patients and methods: </strong>A sample of 82 subjects aged between 15 and 34 years with mean at 25.2 years +/- 4.6 was made up, including 35 homozygous and 47 non homozygous. Plaque index, gingival index and papillary bleeding index, tooth mobility and clinical attachment loss were assessed. Partial correlation between periodontal indexes and haemoglobin and hematocrit controlling for plaque index was performed.</p><p><strong>Results: </strong>No significant statistical differences were found for periodontal indexes and clinical attachment loss between the two groups, even if homozygous show higher values. Periodontitis is less frequent in homozygous and odd ratio show protective effect of sickle cell anaemia (OR = 0.381, IC at 95% = [0.130; 1.1 18]). Tooth mobility is significantly increased in homozygous with mean at 1.0 +/- 0.8 versus 0.5 +/- 0.4 in control group (p = 0.000). Partial correlation show significant negative association between haematocrit and papillary bleeding index in homozygous (p = 0.045).</p><p><strong>Conclusions: </strong>The hypothesis that sickle cell anaemia can affect periodontal conditions and worsen periodontal diseases is to be considered even if it has not been proved as risk factor.
