It is a primitive biliary cirrhosis without antimitochondrial antibodies. This disease is reported at adult women in 90 % of cases. We report a 59 years old man case.</p><p><strong>Observation: </strong>The patient was admitted for a diffuse melanodermy, a physical asthenia and a slimming which preceded 4 months by a pruritus with cholestatic jaundice. Biology showed a normal rate of transaminase. Alkaline phosphatases and direct bilirubine were high. Hepatitis (B and C) and HIV serologies were negative. Abdominal ultrasound showed a homogeneous liver. There were no dilations of the bile ducts. The patient had a unilateral left leg oedema and stasing ulcer. Examinations revealed an abdominal pulsate mass. Echography and T scan showed a right common iliac artery aneurysm and compression of the left iliac vein. Surgery was resected aneurism and replaced vascular prosthesis. Lung involvement can affect functional or vital prognosis. We report 7 cases.</p><p><strong>Cases: </strong>We collected data from 60 years old mean patients. Pulmonary localisation was diagnosed after 19 years of evolution of the auto-immune disease. Discovering circumstances were dyspnea and chronic cough. The radiographic and scannographic signs were diffuse because of long diagnosis delay. The underlying auto immune disease was scleroderma, rheumatoid arthritis, Sjögren's syndrome, ankylosing spondylitis, Sharp's syndrome, and multiple autoimmune syndrome. {"title":"[Frequency of hematological and immunological manifestations of lupus in Dakar].","authors":"F S D Ndiaye, S Ndongo, S Fall, Y Lèye, A Dioum, M M Ka, T Moréira-Diop","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Introduction: </strong>The systemic erythematosus lupus (SEL) or lupic disease is a systemic auto-immun pathology characterized primarily by the presence of antibodies directed against native antibodies anti-DNA. The discovery circumstances of diagnosis are variable and polymorphic. The hematologic signs and the immunological disorders constitute the diagnosis criteria of the lupic disease.</p><p><strong>Methodology: </strong>It is a multicentric and retrospective study undertaken study from January 1, 1997 to September 30, 2006. Patients were followed up in the Internal Medicine of Dakar. We have studied the hematological and immunological aspects of the SEL and have evaluated the prevalence of lupic disease.</p><p><strong>Results: </strong>142 patients with lupic disease were included with 125 women and 17 men; the ratio--sex was 0.13. The mean age was 34 years with extremes of 6 and 72 years. Our patients had hematological and immunological manifestations in 32.4% and 76.8% of the cases. The immunological tests showed the presence of antinuclear antibodies in 97.9% of the cases, native antibody anti-DNA in 45.7% of the cases, the anti-ECT in 8.95% (with the anti-RNP in 78.3 % of the cases, anti-Sm in 56.5% and of anti-SSA in 87% of the cases). The roots showed an antiparsitic activity. The purpose of this work is to determine through bioassay on Rhabditis pseudoelongata the activity of the roots of Annona senegalensis and characterise if any substances responsible for this activity.</p><p><strong>Material and methods: </strong>The vegetable material consists of roots of Annona senegalensis collected in Casamance at 1999. The tests of activity are carried out on Rhabditis pseudoelongata, stock L. Lamy, Institut Pasteur, Paris. Powder of roots (1070 g) was extracted with methanol. From this rough extract, we carried out two extractions with the dichloromethane of which one after passages in alkaline and acid medium. Various extracts were subjected to a chromatography on column of silica gel or from alumina gel; some compounds were isolated by high performance liquid chromatography . The identification of compounds was deed using spectrometric techniques (mass, Ultraviolet, Infrared) and nuclear magnetic resonance. The tests of activity related to three extracts and seven molecules isolated from the roots. The percentage of dead worm is determined after two hours of contact with demineralised water containing the sample to be tested.</p><p><strong>Results: </strong>The neutral extract chloromethylenic showed an important activity on Rhabditis pseudoelongata. Five acetogenines were insulated starting from this extract chloromethylenic (gigantetronenine, squamocine, glaucanisine, glaucanetine, goniothalamicine) and two alkaloids starting from the alkaline extract chloromethylenic (liriodenine and norolivéroline). The tests on Rhabditis pseudoelongata of these compounds showed an activity of the squamocine with a IC50 of 0.1 microg/ml and IC90 0.3 microg/ ml more powerful than that of the levamisole, reference substance (IC50 of 0.8 microg/ml and IC90 13 microg/ml).</p><p><strong>Conclusion: </strong>The study of the biological activity of the roots of Annona senegalensis made it possible to determine the anthelminthic activity of the extract chloromethylenic from where the squamocine was isolated. It allows the cleaning solutions to reach the totality of the canal and to control any infection of the canal system. To achieve this goal is not without difficulty with the steel instruments, because of their rigidity. Recently, the Nickel Titanium instruments were introduced in Endodontics, to allow a setting form technically simplified and faster The aim of this study was to evaluate the duration of the canal shaping with the Pro Taper system.</p><p><strong>Materials and methods: </strong>This exploratory study was carried out in Conservative Odontology and Endodontics service (OCE) of the department of Odontology of Faculty of Medicine, Pharmacy and Odonto-Stomatology of the University CheikhAnta Diop of Dakar in 2006. Sixty nine root canals (50 teeth) were shaped with ProTaper.</p><p><strong>Results: </strong>The results showed that an average time of 17.35 minutes was necessary for the shaping of the long canals against 15.38 minutes for the middle canals and 14.29 minutes for the short canals. The average duration of canal shaping of the one-root teeth is 13.76 minutes and 17.77 minutes for the plural root teeth.</p><p><strong>Conclusion: </strong>Taking into account the necessity of maintaining the teeth on the arcade during long year; include that we have now to respect imperatively the biological principles during the canal shaping in a restricted duration to realize a dense and hermetic canal obturation of the canal system in the same meeting. We report and analyse the clinical and angiographical characteristics of macular degeneration in our patients older than 50 years.</p><p><strong>Material and methods: </strong>In this prospective study, we recruited during 13 months all patients presenting ARMD signs from our consultation department. Various clinic and angiography parameters were collected. The follow up varied between 6 and 20 months.</p><p><strong>Results: </strong>The prevalence of this pathology was of 6.5 per 1000. Sex ratio was 0.36 and mean age 61.6 years (53 to 80 years). No familial history of ARMD was found. Three patients were more exposed to sun because of their profession, 9 had high blood pressure, 6 were hyperopes and 1 was smoking. A drop of visual acuity was noted in all cases but no metamorphopsies. Signs founded were miliar drusen (20 eyes), serous drusen (4 eyes), association of those two kinds (4 eyes) and retinal pigmented epithelium abnormalities (PE) (8 eyes) alone or associated with drusen. These signs were significant of age-related maculopathy (ARM). The fluorescein angiography confirmed the type of lesions and showed no signs of neovascularization. An aggravation of the pigment migration was noted in one case after 20 months.</p><p><strong>Conclusion: </strong>Age related macular degeneration doesn't seem frequent in our population of patients and shows essentially under its initial form of ARM even though facilitating factors seemed identical, excepted tobacco addiction, sun exposition and systemic hypertension. The objective of this study was to draw attention of health workers in Fann hospital as part of the prevention of hospital-acquired infections.</p><p><strong>Materials and methods: </strong>This study was carried out prospectively from April 26h to May 25th. An anonymous questionnaire was administered by 20 formed investigators to health workers, along with an inventory of available resources for hand washing in the study site allowing to collect the data by interview.</p><p><strong>Results: </strong>A total of 256 health workers were investigated. The mean age was 35.3 +/- 9.4 years [range = 20-71] with a sex ratio of 0.62. As for the education level, the secondary and university predominated. The concept of Manu carrying was ignored by 59.3% of the personnel. This proportion was higher among people with low education level. Possibility of resident and transitional floras in the hand was ignored by most of the investigated personnel. The hand washing technique to be applied while putting vesicle probe was ignored by 59% of the personnel, and 34% declared using hand towel to dry hands. Half of the personnel ignored that bread soap was not recommended. The availability of hydro alcoholic solutions was variable according to the hospital wards. Aetiologies are dominated by stroke. The aim of this study was to describe the clinical, epidemiological and evolutionary aspects of the vascular aphasias.</p><p><strong>Materials and methods: </strong>We conducted from August 2003 to May 2005 a descriptive cross-sectional study at the Neurology department in Dakar. This study concerned all patients admitted at the Neurology department for stroke confirmed by the cerebral tom densitometry. All patients were subjected to an examination of the language allowing to confirm the diagnosis of aphasia and to determine the type. The follow-up was monthly during one year.</p><p><strong>Results: </strong>55 cases of aphasia were reported on 170 cases of stroke (frequency: 32.35%). Our patients were all right-handed. Mean age was 56.8 (28 to 86 years) with a sex-ratio of 0.61.76.36% of the patients could neither read nor to write. Only two made higher studies. The nature of stroke was ischemic in 73.7% and hemorrhagic in 26.3%. The aphasias with expressive language impairment were observed in 96.4% against 3.6% of the cases for aphasias with comprehensive language impairment. After one year of evolution, a regression of the disorders was observed only in 9 cases, and the regression was partial in 25 cases. The evolution of the aphasia was correlated with that of the motor deficit. Age, low educational level, ischemic stroke constitute factors of bad prognosis.</p><p><strong>Conclusion: </strong>Vascular aphasias are frequent and of reserved prognosis. However, ingestion of excessive fluoride during tooth development can cause structural changes in tooth enamel named fluorosis. At Gandiaye a city situated in the Senegalese endemic fluorosis area, the main water supply provided by a unique drilling with highly fluoridated water has broken down in 1996. Since then, the drinking water comes from wells which have poor levels of fluorides. The aims of this study were to evaluate the prevalence and severity of dental fluorosis and tooth decays in children born and reared continuously at Gandiaye after the stoppage of the drills and who were drinking water well.</p><p><strong>Methods: </strong>Water samples were collected from two wells and analyzed using a spectrometer and a specific fluoride electrode. The prevalence and severity of dental fluorosis was evaluated according to Dean's method, and the caries experience was measured using the DMF teeth index in 150 children aged from 6 to 8 years.</p><p><strong>Results: </strong>The fluoride levels in the water well were comprised between 0.03 ppm and 0.09 ppm according to the method used. The prevalence of dental fluorosis was 39.33% with the predominance of the very low to low fluorosis forms. The tooth decay prevalence was 48.66% and the mean DMF tooth was 0.98.
