{"title":"[Awareness of the political abuse of psychiatry in relation to psychiatry and the law].","authors":"Z Kalvach","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The political abuse of psychiatry in the former USSR was enabled by: 1. a disproportionate pointing up of the medical criteria of mental disorder to the detriment of the social ones, above all by circumventing the sociological labelling process; 2. lack of control of involuntary psychiatric hospitalisation by the law; 3. an obsolete definition of psychiatry. The author draws attention to the fact that the sample of patients was little aware of the fact of mandatory medical secret and thus unable to make use of advantages that a significant source of escape of information is medical documentation in the widest sense of the word. Conversely, the trend of social changes will, no doubt, lead to extension and more detailed information of the patient on the nature and prognosis of his disease, as it is the case in advanced countries, whereby the patient will be entitled to decide to whom and to what extent these data on his person will be provided. The apparatus makes it possible to take an EEG reading and to evaluate it automatically. Moreover, the paroxysm were evaluated clinically and visually from EEG tracings. The authors proved a significant disagreement as regards evaluation of the length of electroconvulsions assessed by the three methods of evaluation (clinical, automatic EEG analyzer and visual EEG evaluation). They identified moreover in the group, using the automatic EEG analyzer, 11.7% abortive convulsions, by visual EEG evaluation 10.3%, while according to clinical evaluation the rate was as high as 59.2%. Conversely on clinical evaluation no convulsion appeared to be protracted, while according to EEG this was the case in 9.6% (automatic analyzer) and in 10.3% (visual evaluation). The basic change is the duty of the court to investigate every admission of patients without their consent and to take a decision on the justification of admission within seven days. In practice this new legal regulation is most frequently the concern of psychiatrists. When these legal norms are applied in the care of the mentally sick, some problems may arise. {"title":"[Methods of mental health assessment in the adult population: psychometric characteristics of the K and L questionnaire].","authors":"A Koudelková","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The method was originally conceived as a tool for assessment of mental health of the normal population, not burdened seriously by psychopathological factors, attending the surgeries of doctors engaged in primary health care. The psychometric properties of this tool guarantee, however, that the method can be used also in other areas of psychological care. The questionnaire has two separate versions for the adult population in productive age (K) and for the older population, i.e. retirement age (L). It was tested in large numbers of subjects--1644 men and 2235 women. Factor analysis identified 19 effective items based on their variability, inner consistence and discriminating capacity. Univariance analysis revealed differences between groups of subjects as regards sex and age. The factor matrix supports the hypothesis of a one-dimensional interpretation of both types of data. This conclusion is supported also by values expressing the homogenity of scales. Indices of mental health were further validised with regard to the general adaptability of the personality. The author describes three stages of implementation as regards admission of a person to a psychiatric in-patient department, i.e. prerequisites for involuntary psychiatric hospitalization, the stage of the intervention and the stage of admission to and detention in a psychiatric in-patient department. Attention is drawn to the fact that in the legal regulations are some inaccuracies and that practical implementation of involuntary hospitalization of the mentally sick is associated with some difficulties.
