The tumour penetrated through the lateral wall of the sinus into the intracranium. It was manifested by pain in the left half of the face, Horner's syndrome and paresis of the abducent nerve. The diagnosis was based on the case-history, clinical picture, computed tomography and biopsy. A non-differentiated carcinoma was involved. {"title":"[Treatment of precanceroses and carcinoma of the larynx].","authors":"J Tocík","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The problem of diagnosis and treatment of precanceroses and carcinome of the larynx is still one of the foremost interests of ENT specialists. In 1973 to 1982 82 patients with precanceroses of the larynx and 174 patients with carcinoma of the larynx were detected and treated. In the treatment of precanceroses of the larynx the laryngomicro-surgical procedure proved useful. In the third stage of Reinke's oedema a satisfactory functional effect was achieved in 100%, in solitary papilloma and papillomatosis in 88%, in leukoplakia and pachyderma in 76% and in chronic hyperplastic in 67%. Least relapses were recorded in the third stage of Reinke's oedema (0%) and in chronic hyperplastic laryngitis (23%). In the treatment of carcinoma of the larynx combined surgical and radiation therapy is preferable and was used in 55.4% patients; in 36.9% patients only irradiation was used and in 7.7% patients only surgical treatment. Within the framework of surgical and combined treatment 52.8% patients were subjected to laryngectomy and 47.2% to various types of partial resections of the larynx. By combined treatment five-year survival was achieved in 67.5% patients; in stage I and II of carcinoma in as many as 87.5% and in stage III and IV only in 47.5%. The Eustachian tube may be obstructed by a tumour or its orifice compressed, sometimes the tumour penetrates into the middle ear. The authors discuss the position which may develop in infiltration of the semicanalis m. tensoris tympani and how functional disorders of this muscle may be manifested. {"title":"[Stapedectomy and a piston in otosclerosis].","authors":"B Kuniak, J Klacanský, M Profant","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>At the Otolaryngological Clinic, Faculty Hospital Bratislava since 1971 to 1987 673 microsurgical operations on the stapes were made in patients with otosclerosis, incl. 73 using the method of stapedotomy and interposition of a teflon piston. The method was very successful. Hearing improved in 91.7% of the patients. In no instance marked deterioration of hearing or deafness developed. The surgical operation is associated with a smaller risk of damage to the inner ear. Patients tolerate stapedotomy well and the results are better than after stapedoplasties according to Schuknecht. The decibel gain is more even and at higher frequencies more marked than after stapedectomy. {"title":"[Pharyngo-gastric anastomosis in the treatment of cancer of the pharyngo-esophageal region].","authors":"I Hybásek, T Suchý, K Kabelác, K Beranová","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The authors submit their own experience with pharyngogastric anastomosis in patients where on account of circumferential cancer of the oesophagus and hypopharynx laryngopharyngooesophagectomy had to be performed. For the anastomosis a tubular flap of the greater curvature of the stomach was used which was pulled through the posterior mediastinum and sutured with the mucosa and musculature of the residual pharynx and the preserved upper half of the epiglottis. The operation is pretentious and requires satisfactory cardiopulmonary function of the patient. A disadvantage of the operation is substantial loss of gastric function, compensated by the patients by a therapeutic diet and medicinal substitution of the gastric chemism. {"title":"[Use of the balloon catheter in dilatation of corrosive stenosis of the esophagus].","authors":"M Pokorný","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>During the period between October 1985 and the end of March 1988 from a total number of 269 dilatations in 29 patients with benign stenoses of the oesophagus of varying origin and in one patient with malignant stenosis 109 operations were made in 5 patients with corrosive stenoses. Three of these five patients suffered from double stenosis. For dilatation balloon catheters of Cook Co. were used. {"title":"[Early paralysis of the recurrent nerve in bronchogenic carcinoma].","authors":"K Podhola, I Průcha, S Tichý","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In 1983-1987 34 patients with paresis of the recurrent nerve were examined. In 4 patients with left-sided paresis bronchogenic carcinoma was revealed, i. e. in 12% of all examined subjects. In relation to the group of patients with left-sided paresis bronchogenic carcinoma was detected in every fifth patient. As no other symptoms than hoarseness are present in paresis of the recurrent nerve and the results of examinations are frequently negative, the diagnosis is difficult. The authors summarize their experience and draw attention to systematic examination and possible new examination methods which must be indicated.
