{"title":"[Somatosensory evoked potentials in the acute phase of focal cerebral ischemia].","authors":"S Vohánka, P Strnad","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The authors made 38 examinations of the somatosensory evoked potentials of the median nerve (SEP) in 28 patients in the acute stage of focal cerebral ischaemia (20 men, 8 women mean age 62 years; range 28-81). According to the clinical and CT picture the patients were divided into two groups: affections in the carotid vessels (22 patients) and in the vertebrobasilar vessels (6 patients). The investigated parameters comprised the so-called central conduction time (interval N13-N19) and amplitude N19/P22. The highest incidence of abnormalities was in the group of vertebrobasilar ischaemias (83%), while in the group of ischamias in the carotid vessels it amounted only to 50%. In both the central conduction time as well as amplitude N19/P22 were effected. In the group of focal ischaemias the authors did not find a close correlation between the carotid circulation and the sensory deficit, but recorded a correlation between the affection of SEP and the resultant condition: in patients without SEP abnormalities complete restoration of the condition predominated (63%), while in the group with pathological findings during the SEP examination complete restoration was recorded only in 9%. Repeated (dynamic) evaluations and the relationship between the ischaemic focus and the somatosensory pathway are important.</p>","PeriodicalId":75690,"journal":{"name":"Ceskoslovenska neurologie a neurochirurgie","volume":"54 2","pages":"102-6"},"PeriodicalIF":0.0,"publicationDate":"1991-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"13200341","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The reflexes were evoked during relaxation, slight or intense contraction of the masseters with the mouth open or closed. By percussion of the chin from above extension reflexes were evoked and by percussion of the chin from below contracting reflexes. The response was recorded by means of an electromyographic apparatus. The short-term stretch responses were evoked from the relaxed and slightly contracted masseters with the mouth open or closed. During intense contraction of the masseters the short-latency reflex stretch response was not evoked. In that case at first a short-latency inhibition of the basal activity of the masseter was observed followed by a medium and late-latency excitation response. With the masseters relaxed a long-latency stretch response was observed only with the mouth open. Percussion of the chin from beneath with slight contraction of the masseter caused initial inhibition of its basal activity followed then by a short-latency excitation response. Comparison of the surgical findings with the X-ray findings revealed that on periradiculography the conclusion was in complete agreement in 31 patients, in another 27 partial agreement (a total of 73.3% patients examined in this way), in those examined by CT there was complete agreement in 16 and partial agreement in 11 (i.e. 71.1% on this sub-group). The shortest interval after operation was one year, the longest 20 years. In all instances a hypodense focus of varying extent was revealed, in four patients it gave the impression of a pseudocyst. In one patient the author observed a pseudocyst with an expansive effect on the lateral ventricle. Puncture produced clear, colourless fluid without signs of inflammatory changes. Extensive pathological lesions were found in particular in patients where recovery was complicated by a relapse. It was surprising that quite extensive pathological changes were found also in patients who were treated by so-called minimal surgical intervention (puncture with drainage).
