Thirty-two patients could be investigated in a follow-up study. Eight recurrences were observed. The condition manifested itself in one case by a sudden perforation of the jejunum; in the other case by ileus of the terminal ileum. In both cases the disease process pursued a relentless course that ultimately led to the death of the patients. Elective surgery in patients with lymphangiectasia has been performed before. {"title":"The relationship between carcinoma of the rectum and hemorrhoids.","authors":"H G Gooszen","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The suspected positive correlation between rectal carcinoma and hemorrhoids was examined in a retrospective study. In patients older than 50 a significant positive correlation could be demonstrated between rectal carcinoma and hemorrhoids. In the group of hemorrhoid patients described it could not be shown that rectal carcinoma was more frequent than in the normal population. In patients older than 40 with hemorrhoids a rectoscopy is always recommended. {"title":"Curative and palliative surgical treatment of pancreaticoduodenal adenocarcinoma.","authors":"H F Hoitsma, S Meijer, G den Otter","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In the period 1965 to 1976 inclusive, 55 patients suffering from duodenopancreatic adenocarcinoma were surgically treated at the Free University Hospital. Twenty of these patients underwent a 'curative' Whipple pancreaticoduodenectomy for a resectable tumor. The difference in survival time makes a differentiation important between carcinomas occurring in the head of the pancreas and those of the peri-ampullary region. In our series, 11 of the former were found and 9 of the latter group. Of each group one patient died within 30 days of the operation. The average survival time of 9 patients, operated on for a tumor in the head of the pancreas, was 10.2 months, with a range from 2 to 23 months. One patient is still alive 23 months after the operation. The average survival time of 5 patients with a periampullary tumor was 22 months with a range from 6 to 52 months. Three patients are still alive, one more than 12 years and 2 more than 19 months after operation. Considering the bad results, and the fact that 30 percent of pancreatic carcinomas are multifocal, it would seem that the classical Whipple operation is a thing of the past. One can then make a plea for total pancreaticoduodenectomy or even for a regional pancreatectomy. Thirty-five patients underwent a palliative procedure, consisting of a biliary bypass combined in a number of cases with a gastroenterostomy. Five patients died within 30 days of the operation. {"title":"Surgical treatment of clavicular fractures with Kirschner wires: a comparative study.","authors":"P J Paffen, E W Jansen","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In the period January 1968 to January 1976, 1400 patients were treated for clavicular fractures. In 74 (5 percent) of cases co-adaptation osteosynthesis was carried out and the results are reported. Conservative treatment of clavicular fractures is to preferred. Surgical treatment should be performed only on strict indication. Percutaneous intramedullary Kirschner wire fixation, performed in an atraumatic manner and with proper postoperative treatment, gives good results for simple fractures. On pathological examination lymph node metastases were not found in both instances. There was a positive family history of the occurrence of carcinoma.
