This is illustrated by two succesful cases. {"title":"Surgical treatment of the bleeding peptic ulcer.","authors":"H L Kalsbeek","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Sixty-six patients were subjected to operation for a bleeding ulcer, 14 of them died. There was little difference between the results of truncal vagotomy and resection, in contrast to reports in the literature that vagotomy gave better results especially in duodenal ulcer. Emphasis is laid on the importance of locating the hemorrhage preoperatively. {"title":"Perforating rectal injuries.","authors":"H B Oeseburg","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The symptomatology, diagnosis and treatment of perforating rectal injuries are discussed. An analysis is presented of the data concerning 24 patients with intra- and extraperitoneal penetration of the rectum. Factors that contribute to the continuing high mortality rate of rectal perforation are its rarity, the highly infectious character of faecal contamination of the abdominal cavity or perirectal tissue, and the fact that examination often reveals little or no external trauma. It was found that 2 and 6 strains, respectively, were sensitive to discs containing 100 mug sulphafurazole and 300 mug polymyxin B, respectively, 41 strains were sensitive to the combination of discs containing 100 mug sulphafurazole and 50 mug colistin, respectively, and 38 strains were sensitive to the combination of discs containing 100 mug sulphafurazole and 300 mug polymyxin B, respectively. The tray method proved that 2 and 5 strains were sensitive to 25 and 200 mug sulphafurazole, respectively, and that 1, 2 and 3 strains were sensitive to 25, 50 and 100 mug polymyxin B, respectively. Also 41 strains were sensitive to the combination of 100 mug sulphafurazole and 25 mug colistin and 38 strains were sensitive to the combination of 25 mug sulphafurazole and 6.3 mug polymyxin B. There was good agreement between the results of the disc and of the tray studies. The ASPS is a relatively rare malignant tumor of the soft parts that grows slowly and shows a predilection for the extremities; it occurs most often in young women. The tumor is deceptive in that the clinical impression is benign. So far, the only method of treatment is ample surgical excision. The tumor appears to be scarcely radio-sensitive and primary cytostatic therapy gives very little response. Histogenetically, the tumor is related to the neuro-endocrine paraganglion system. {"title":"Necrotic colitis in the presence of normal vascularization of the colon.","authors":"H W Deleu","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Necrotic colitis in the absence of organic obstruction of the mesenteric vessels is an extreme and fulminant form of ischemic colitis. This calamity with its high mortality rate usually occurs in patients with pre-existent cardiopathy. It is rare for necrotic colitis to occur as a complication of hypotension, hypovolemia or sepsis. A report is presented of a case of total gangrene of the colon in a patient with hemorrhagic shock. Certain concepts are presented concerning the etiology of non-occlusive intestinal infarction. Rigid fixation with osteosynthesis is the treatment of choice.
