{"title":"Machine perfusion versus cold storage for preservation of kidneys before transplantation.","authors":"M J Slooff, J van der Wijk, B G Rijkmans, G Kootstra","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Machine preservation was introduced into the Groningen Transplant Group in December 1972. Thirty five of the 90 kidneys transplanted in the following period of four years were preserved on a machine. The other kidneys were preserved by the so-called cold storage method. The advantages and disadvantages of both methods are outlined. Evidence is found in the literature and in this study that cold storage is sufficient for preservation-times up to 24 hours. When the preservation time exceeds this period, then machine preservation may be an advantage. Kidneys damaged by ischaemia were found to have a higher percentage of immediate function when preserved by the machine compared to those preserved by cold storage. There was no significant difference in one-year graft survival for the kidneys preserved by either method. Machine preservation helps to select between viable and non-viable kidneys from non-heartbeating donors. This is especially important in view of the lack of kidneys for transplantation. Even a temporary artificial anus is superfluous. In none of the six patients thus treated was any complication observed, all are normally continent. A review of the literature reveals that only three papers have been published dealing with selective embolization for controlling hemorrhage from the kidney. {"title":"Stomal dysfunction following partial gastrectomy.","authors":"R T Bijleveld","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The case is reported of a man aged 57 years in whom after partial gastrectomy, gastric emptying remained impaired for a long period of time. The disorder was caused by stomal dyfunction. After 74 days' conservative therapy, mostly on an outpatient basis, passage through the stomach recurred spontaneously. Successful conservative therapy of such a long duration has not previously been reported in the literature. Protracted obstruction of the stoma after partial gastrectomy is rare. {"title":"Comparative follow-up study of conservative and surgical treatment of femoral shaft fractures in children.","authors":"J W Schonk","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The guiding principles of treatment of femoral shaft fractures in children are discussed. Up to the age of six years a Bryant or Weber bandage was used. In older children surgical treatment was preferred. The indications are outlined. Between 1959 and 1974 100 children with femoral shaft fractures were treated in the Maasziekenhuis, 21 percent received surgical treatment. In 92 cases follow-up was possible. For detection of changes X-ray examination is preferred. Significant deformities were not found. An increase in length by more than one centimeter of the fractured femur was seen in 22 percent of cases after conservative treatment, in 53.8 percent of cases after treatment with a medullary pin and in 16.6 percent after treatment with an AO-pressure plate.
