{"title":"Althesin (alphadione, CT 1341) a 'new' induction agent for anesthesia.","authors":"W T Cornet, D T Popescu","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Althesin (alphadione) is a hypnotic agent introduced in the U.K. a few years ago by the firm of Glaxo. It is mixture of two steroids: alphaxalone 9 mg per ml and alphadolone 3 mg per ml, in solution in cremophore EL. After a concise review of the history of steroid anesthetics, the pharmacology of the new product is described. In a personal investigation, Althesin was used to induce anesthesia in 100 patients. The dose equivalent with thiopentone was determined first. It was found to amount to 52 microliter per kg corresponding to 3 mg per kg of thiopentone. A separate form was completed for each individual patient, to record the side effects, if any. The pulse and blood pressure were checked once a minute after the induction (for 5 minutes). In 8 patients the following parameters were recorded: plethysmogram, ECG, central venous pressure, pulse rhythm arterial pressure, stroke volume and cardiac output. Compared with thiopentone, Althesin is not significantly worse, but neither does it present any advantages where the circulatory parameters determined are concerned. {"title":"Lobular carcinoma of the breast.","authors":"J J Doeven","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Lobular carcinoma of the breast, both the in situ and the infiltrating type, is discussed on the basis of three observations. Mammography may be useful, although if often leads to the diagnosis of mastopathy. In cases of in situ lobular carcinoma, ablation of the breast constitutes an adequate therapy. The adequacy of subcutaneous removal of the mammary parenchyma is doubtful. In infiltrating lobular carcinoma, radical mastectomy by Halsted's or Patey's method is the treatment of choice. Special attention is given to the surgical technique, the electrode and the measurement of the threshold value. Advanced age and moderately severe cardiorespiratory disorders do not preclude good results from this operation. Following detailed laparoscopic inspection of the abdomen, the appendix is pulled out with a laparoscopic forceps through a small incision in the right lower abdomen; it is then skeletized. Following ligation with catgut and nylon, the appendix is severed using the thermocautery. The stump is subjected to careful after-coagulation or treated with iodine, and repositioned. {"title":"Urinary hydroxyproline as a measure in fracture healing.","authors":"J de Boer, J P Jaspers, C G de Haas","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The determination of any pattern in urinary total OH-proline following bone fractures has been the purpose of this investigation. This was carried out in a group of 11 patients with major fractures, a group of 8 patients with minor fractures and a control group of 10 healthy laboratory technicians. Indeed, some patterns could be detected. In patients with major fractures a considerable elevation of urinary total OH-proline was found which was correlated to the size and number of fractures and the healing tendency of the fracture. Normal to only slightly elevated urinary total OH-proline values during the course of the healing process of major fractures could perhaps indicate delayed union, or even malunion.
