{"title":"Ovarian metastatic melanoma. This observation relates the case of a 33 year-old woman who presented with a voluminous unilateral, histologically poorly differentiated, ovarian mass, and discusses the differential diagnosis. Immunohistochemical studies demonstrated strong positivity for S-100 protein, HMB-45 and vimentin. {"title":"[Sequences of human herpes virus type 8 and Epstein-Barr virus in AIDS-related primary central nervous system non-Hodgkin's lymphoma].","authors":"L Bélec, L Wingertsmann, F Chrétien, A S Mohamed, J Minarovits, F Gray","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Presence of human herpes virus type 8 (HHV8), detected by nested PCR, and expression of Epstein-Barr virus (EBV), as assessed by immunochemistry and in situ hybridization, were evaluated in 20 primary non-Hodgkin immunoblastic lymphomas (NHL) of the central nervous system (CNS) from patients who died from AIDS, and in 10 samples of cerebral tissues from patients who died from AIDS, without cerebral lymphoma or Kaposi's sarcoma, as controls. Six lymphomas (30%) contained HHV8 sequences, and 19 (95%) expressed EBV; detection of HHV8 was more frequent in patients with Kaposi's sarcoma, than in other subjects (4/6 versus 2/14). Three (30%) controls contained HHV8 sequences, whereas none expressed EBV. AIDS-related CNS NHL is therefore clearly associated with EBV expression, while the presence of HHV8 appears occasional, probably associated with a low tissue viral load. The most frequently encountered is cerebromeningeal cryptococcosis. We report 3 clinicopathological cases of rarer fungal infections of the central nervous system in AIDS patients due to Candida and Aspergillus genders. In most cases, a systemic candida infection or aspergillus pulmonary infection preceded the onset of cerebral granulomas or abscesses. These infections usually occurred at the terminal stage of the disease and were associated with other neuropathologies. Its sensitivity and specificity in staining reactive mesothelial cells in serous effusions was determined using the indirect immuno alkaline phosphatase anti-alkaline phosphatase technique. Smears prepared from 78 serous effusions (15 benign, 54 malignant, 9 atypical) were immunostained with GC12 and five other monoclonal antibodies: KL1 directed against cytokeratins, D33 against desmin, E29 against epithelial membrane antigen, Calam 27 against epithelial surface antigen and NEO 723 reactive with carcinoembryonic antigen. GC12 reacted positively with 80 to 100% mesothelial cells from the 15 benign effusions. Carcinoma cells were negative in 87% of malignant cases. Ovarian carcinomas were specially stained among positive cases. One malignant mesothelioma was positive and one was unreactive with GC12. Moreover, the reactivity of atypical cells in 9 serous effusions with GC12 was helpful to characterize benign and malignant effusions: the immunophenotype GC12-, Desmine-, Calam 27+, EMA+, ACE+ allowed detection of carcinoma cells in 2 cases.
