{"title":"Clinical anatomy of the m. flexor carpi radialis tendon sheath.","authors":"H M Schmidt","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>At the transitional zone from the forearm to the hand the insertion tendon of the m.flexor carpi radialis (FCR) glides on a fibrous and fatty cushion, which is connected dorsally with the joint capsule of the radiocarpal articulation. The tendon distally crosses the palmar side of the scaphoid tubercle and enters the dorsally curved rim of the trapezoid tubercle. At the level of the wrist joint the narrow tendon sheath begins, which extends to the insertion at the metacarpus. Immediately after entering the gliding tunnel the tendon branches off radially as a rule with an accessory fibre strand 8 mm in width to the scaphoid, trapezium and the joint capsule between these two bones. The insertion tendon regularly is attached to the palmar and radial surfaces of the second and third metacarpal bones. The wall of the osteofibrous gliding tunnel can be prominent following trauma, inflammation or arthrosis deformans in the trapezio-scaphoideal joint and may irritate the tendon (tendovaginosis stenosans). Against resistance forces pain will occur in the wrist joint during palmar flexion. The typical point of tenderness is situated at the entering of the tendon in the thenar region. In a total of 13 different hind limb muscles this tracer was applied in a slow-release gel. Motoneurons innervating a particular hind limb muscle were clustered in longitudinally arranged motor pools. Motor pools of different muscles did show considerable overlap both in the rostrocaudal and transverse plane. But, the various motor pools clearly show a somatotopic organization of motoneurons even in such a condensed lumbar spinal cord as in Xenopus laevis. Motoneurons innervating more distally positioned muscles are generally found in more caudal segments, while proximal muscles (with the exception of the m. adductor magnus) are supplied by motoneurons more or less throughout the lumbar enlargement. Flexor muscles usually are innervated by motoneurons situated ventrolaterally in the ventral horn, extensor muscles by dorsomedially found motoneurons. This pattern is particularly apparent for proximal (thigh) muscles, less so for more distal (shank and foot) muscles. Attention is given to the terms established in the officially canonized Nomina Anatomica (NA) of the IANC as well as to the widely used terms in clinical literature. This terminology has been reviewed in the light of underlying kinematic concepts developed by investigators of tarsal joint motions as found in the original papers and in a number of widely used handbooks. The 47 animals were divided into 5 groups: group 1: controls, group 2: cleft made in the bony palate, using a dental burr (phi 3 mm), group 3: cleft as in group 2, oral mucosa stitched, group 4: cleft made without perforating the nasal mucosa; oral mucosa stitched (\"Veau\"), group 5: cleft made as in group 4; before stitching the oral mucosa pieces of autologous rib bone were introduced into the cleft (\"Schuchardt\"). At the ages of 4, 5, 6, 8, 12, 18 and 24 weeks, one or two animals per group were sacrificed. The palatal region was microscopically studied using 10 mu paraffin sections stained according to Herovici. It was found that it is difficult to make a reproducible cleft on the histological level. A large interindividual variation in local damage due to the surgical procedure was observed; this might contribute to the variation in growth results of the facial skeleton reported in literature. Signs of excessive bone resorption were found. The presence of bone in places where sutural tissue should be present, observed in some growing animals, will contribute to reduction of palatal width, medial collapse of the palate and asymmetry of the facial skeleton. The description is primarily concerned with ligaments, membranes and fibrocartilages which were so far incompletely analysed. Since computers became available this study became a new dimension in that it became feasible to apply advanced mathematical multivariate methods which make use of morphometric data. However, looking back on what has been achieved so far with these methods, it must be admitted that the results obtained are often unsatisfactory. This has led to a certain lack of acceptance of these methods. In the present paper it is argued that very useful results may be obtained by applying more sophisticated multivariate methods which are specifically designed for the anthropological problems at issue. Three examples are given. The first deals with the controversy between \"Creationism\" on the one hand and \"Evolutionism\" on the other. Our results strongly support the Evolutionists' point of view. The second example deals with the reconstruction of human phylogeny. An investigation is discussed which has led to a startling new hypothesis concerning the evolution of man. The last example concerns a preliminary investigation of trends in human sexual dimorphism. Injection of latex, plastoid as well as radiopaque substance (40% barium sulfate) were used to follow the distribution of the renal vein through the kidney tissue. The renal vein of the camel is unique as it gives off vv. interlobares I which drain the ventral half of the kidney, in addition to large cranial and caudal stem vessels from which vv. interlobares I of the dorsal surface branch. The volume of the calcified zone expressed as a percentage of the total cartilage was not only constant for one joint, but also in all the joints of a single individual. Nevertheless, the variation between subjects ranges from 6.9 to 8.6%. In two cases both sides of the body were investigated. As was the case in an earlier investigation on the femoral head, the bilateral distribution of the thickness was the same. Since the thickness of the total cartilage varies with the local distribution of loading in the joint, it follows that the thickness of the calcified layer also depends upon mechanical factors. Five subjects is too few to allow correlation with age or sex to be either refuted or confirmed.
