To some, low-flow rates are advocated as safe, environmentally friendly, and cost-effective. Others remain adherent to the practice of higher-flow rates due to the concerns of Compound A and renal injury. In this episode of the Depth of Anesthesia podcast, we explore the claims that low-flow (< 2 L/min) administration of sevoflurane produces Compound A, and that Compound A exposure leads to renal injury. We also investigate the origins of the claims in addition to conflicting recommendations by manufacturers and regulatory agencies. According to China Stroke Statistics 2019, ischemic stroke accounted for 81.8% of all stroke cases [2]. The most effective medical treatments for acute ischemic stroke (AIS) are to offer intravenous thrombolysis (within 4.5 hours) and mechanical thrombectomy (within 6 hours) during the ultra-early period of time following the onset [3]. However, these two treatments have a narrow effective time window which greatly limits their clinical use [4]. The stroke registry data from 132 urban hospitals indicated that the rate of thrombolysis for AIS in China remained around 2.4%, which was significantly lower than that of about 20% in other countries [5]. As reducing prehospital delay for stroke victims in China is very critical, Dr. Jing Zhao and Dr. Renyu Liu proposed the Stroke 1-2-0 strategy for rapid stroke recognition and response [6]. Since then, this strategy was modified to Stroke 1-1-2, which is a universal stroke educational program without language barriers [7]. Here are listed the efforts that the special task force accomplished in 2020.","PeriodicalId":75245,"journal":{"name":"Translational perioperative and pain medicine","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-06-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"48726578","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Diagnosis is based on clinical features and may be confirmed by visualization of negatively birefringent monosodium urate crystals under polarized light microscopy. In cases of gout, polarized light microscopy can reveal negatively birefringent monosodium urate crystals. Though gout is common in joints few cases have been described elsewhere in the body. This paper describes a male patient at his early 80s who developed acute tophaceous pulmonary gout. The literature on this subject is limited to a few case reports, and the exact mechanism is still unknown. This case, to our knowledge, reports the first case of persistent hiccups following an interlaminar lumbar epidural steroid injection of non-Particulate type. It was performed for the treatment of chronic pain due to lumbar radiculopathy, in a male patient in his early 30s. As the etiopathology and associated patient-related factors of this bothersome complication are not fully understood, and the number of interventional pain procedures is on the rise, anesthesiologists and pain physicians must anticipate this complication regardless of the age, site, and composition of epidural injection. Such potential complications should be included in the relevant informed consent.","PeriodicalId":75245,"journal":{"name":"Translational perioperative and pain medicine","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-06-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"43802686","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

An increasing percentage of proceduralists are not surgeons and non-operating room nursing and sup port staff are often employed, adding another layer of separation from conventional practice. In most clinical scenarios, anesthesia personnel are still required to be present to ensure the safety of sedation, immediate re suscitation for unexpected events, and overall patient well-being. Anesthesia providers are expected to pro vide comprehensive services for complex or critically ill patients in these locations, all within the constraints of normal working hours. Abstract As the world’s growing population ages, diseases associated with the elderly, such as atrial fibrillation (AF), have increased in prevalence. These patients require specialized anesthetic care when presenting for surgical procedures, as they often have additional cardiovascular comorbidities and are frequently anticoagulated. Innovative techniques prom ise to mitigate thromboembolic risks associated with AF or even provide a cure. However, many are highly technical and associated with unique and potentially serious compli cations. Increasingly, comfort with providing anesthetics in nontraditional locations and use of advanced transesoph ageal echocardiography are required of anesthesiologists to safely and competently provide care during these novel procedures. The majority of left atrial appendage occlusion devices are percutaneously delivered and require intensive fluoroscopy or transesophageal echocardiography both for device placement and evaluation of possible side effects, such as leaking around seal devices, pericardial effusion, or tamponade. Catheter-based ablations that seek to both map and eliminate arrhythmogenic foci via different energy sources are associated with numerous side effects, includ ing cardiac tamponade, hemo- and/or pneumothorax, vas cular access-related complications, stroke, phrenic nerve injury, pulmonary vein stenosis, atrioesophageal fistula, and death. Due to its proximity to the pulmonary veins, mild esophageal ulceration occurs frequently with energy de -livery. Patients who develop atrioesophageal fistula often present in extreme circumstances and require a multidis ciplinary approach on an emergent basis before operative management. The modern anesthesiologists, therefore, must be familiar with these procedures to care for these pa tients and mitigate damage from unintended side effects.","PeriodicalId":75245,"journal":{"name":"Translational perioperative and pain medicine","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-06-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"48792396","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
