Many DSD conditions are rare and insufficiently known in adult medicine, highlighting the need for a structured transition pathway.This article summarizes recommendations derived from clinical guidelines, the Berlin Transition Program, and practical clinical experience. Typical case scenarios (e.g., hypospadias, congenital adrenal hyperplasia (CAH), complete androgen insensitivity syndrome (CAIS), gonadal dysgenesis) are used to illustrate key challenges, therapeutic objectives, and competencies needed for effective transitional care.Findings demonstrate that successful transition depends on clear communication, interdisciplinary collaboration, continuity of care, and the promotion of patient autonomy. Key aspects include the management of hormonal issues, evaluation of structural genital variations, tumor risk assessment, and psychosocial support. Common barriers include limited acceptance of the diagnosis, lack of structural resources, and insufficient expertise within adult healthcare services.A personalized transition plan-supported by medical summaries, imaging reports, and structured patient education-enhances the quality of care and fosters empowerment and participation. This review summarizes recent advances in testicular germ cell tumors (TGCTs) and penile cancer, with a focus on emerging biomarkers and targeted therapeutic strategies. Conventional serum tumor markers such as AFP, β-HCG, and LDH remain important but are limited by suboptimal sensitivity and specificity. Novel biomarkers, particularly microRNA miR-371a-3p, have demonstrated substantially higher diagnostic accuracy and may improve risk stratification, detection of disease recurrence, and treatment decision-making in TGCTs. In addition, the analysis of circulating tumor DNA (ctDNA) has emerged as a promising liquid biopsy procedure. In penile squamous cell carcinoma, molecular classification based on human papillomavirus (HPV) status and comprehensive genomic profiling are becoming increasingly relevant for prognosis and therapeutic selection. The identification of actionable molecular alterations has facilitated the development of immunotherapy, anti-EGFR strategies, and antibody-drug conjugates. Early clinical data also suggest that ctDNA may provide valuable information for treatment monitoring and early relapse detection. Despite these promising findings, further prospective studies and standardized methodologies are required before many of these approaches can be incorporated into routine clinical practice. This approval was based on the phase III NIAGARA trial, which demonstrated that the combination led to a significant improvement in event-free survival compared with neoadjuvant chemotherapy alone. Furthermore, the rate of complete pathological responses was higher with the addition of perioperative durvalumab to neoadjuvant chemotherapy. This benefit was also reflected in overall survival, which was significantly prolonged in the durvalumab combination arm. Importantly, the combination did not compromise the completion rate of cystectomy or increase surgical complication rates. This article aims to present the perioperative durvalumab-based treatment regimen in the context of the available evidence and to critically discuss its clinical benefit and limitations. A particular focus is placed on defining appropriate criteria for patient selection and characterizing the safety profile. In this context, therapy-related grade 3-4 adverse events reported in the NIAGARA trial are specifically considered, including anemia (14% in the durvalumab arm), neutropenia (14%), urinary tract infections (14%), and decreased neutrophil counts (7%). The potential role of circulating tumor DNA (ctDNA) as a prognostic biomarker is also addressed, without questioning the indication for radical cystectomy or adjuvant therapy. Over the last 15 years, however, infection rates have declined substantially. This improvement is attributable to technological advances in implant design, particularly infection-retardant coatings, improved perioperative prophylactic concepts, and optimized patient selection based on identified risk factors. Modern surgical strategies such as the no-touch technique, tailored antibiotic and antifungal prophylactic regimens, and antiseptic irrigation protocols during revision procedures have contributed substantially to the reduction of postoperative infections. At the same time, the management of manifest implant infections has evolved: emergency explantations are now largely reserved for severe systemic infections, whereas targeted antimicrobial therapies followed by salvage surgery are increasingly being adopted. Its objectives are continuous documentation, scientific evaluation, and quality assurance of treatment for patients with urothelial carcinoma at all stages in office-based outpatient care. This registry captures the cross-sectoral quality of care for urothelial carcinoma of the bladder and upper urinary tract in Germany. Currently, data from 1531 of 2024 enrolled patients are available for analysis in UroNAT.Results: In 69% of patients, presentation was driven by a symptom, predominantly gross hematuria or microscopic hematuria. Tumors were located in the urinary bladder in 94.5% of cases, and in the ureter or renal pelvis in 2% each. A solitary tumor was observed in 65% of patients, 2-7 tumors in 33%, and more than 8 tumors in 2%. Histologically, non-invasive tumor stages predominated (57% Ta, 20% T1), while 23% presented with T2-T4 disease. High-grade (G3) tumors accounted for 50% of cases, G2 for 26%, and G1 tumors for 24%; pure carcinoma in situ (CiS) was present in 3.9%. Approximately 90% of patients underwent TURB or re-TURB; cystectomy was performed in 5.1%, and procedures on the upper urinary tract or radiotherapy were carried out in fewer than 5%.Conclusion: This nationwide registry allows all uro-oncological care providers from urology, oncology, and hospitals to participate via their outpatient clinics.
