{"title":"The semiology of migration","authors":"José Antonio Garciandía Imaz , Isabella Garciandía Rozo","doi":"10.1016/j.rcpeng.2021.05.010","DOIUrl":"10.1016/j.rcpeng.2021.05.010","url":null,"abstract":"<div><h3>Introduction</h3><p>The phenomenon of migration generates a series of experiences in the human being that are translated into emotions, feelings, adaptation processes, grief and psychopathological processes, and even pathological expressions, represented by clinical pictures of different kinds.</p></div><div><h3>Objective</h3><p>The purpose of this article is to carry out a conceptual and clinical reflection on the semiology around the concept and experience of migration, in order to illustrate the complexity that it entails as a human phenomenon.</p></div><div><h3>Methods</h3><p>A reduced narrative review, circumscribed and restricted to the semiological, psychopathological and clinical aspects of migration.</p></div><div><h3>Discussion</h3><p>The separation, ruptures and losses that derive from migration do not go unnoticed by the individual. They are inscribed in his/her corporality as physiological injuries that affect his/her life and as symbolic injuries that affect his/her existence.</p></div><div><h3>Conclusions</h3><p>Migration supposes a rupture of the totality of being. The objective of this study is to know the suicide mortality rates and their distribution between the years 2017 and 2019.</p></div><div><h3>Methods</h3><p>The national death registries of the Ministry of Health of Peru were analysed, calculating the regional death rates from suicides adjusted for age and gender using the standardisation recommended by the World Health Organization.</p></div><div><h3>Results</h3><p>A total of 1666 cases of suicide were identified (69.3% males); the age group with the highest frequency was that of 20–29 years (27.8%); the mean age at suicide was higher in males (37.49<!--> <!-->±<!--> <!-->18.96 vs. 27.86<!--> <!-->±<!--> <!-->15.42; <em>P</em> <!--><<!--> <!-->.001). Hanging was the most common suicide method among both males (58.87%) and females (48.14%). For males, hanging was followed by poisoning (22.6%) and firearms (4.59%); for females, by poisoning (38.75%) and firearms (0.59%). The suicide rate increased from 2017 (1.44/100,000 inhabitants) to 2019 (1.95). The highest rates were identified in the departments of Arequipa, Moquegua and Tacna.</p></div><div><h3>Conclusions</h3><p>In recent years, there has been an increase in the number of suicide cases and the rates by department, with the highest number of cases reported in males. Males tend to use more violent suicide methods. Of these, 68.5% were women, and the mean age was 21.4 years. Depressive symptoms were found in 74% of the participants, anxiety symptoms in 57% and distress symptoms in 65%. The variables associated with the development of symptoms of moderate-severe depression, anxiety and distress were: not having family economic stability, being in the first years of medical training, being female, and fearing that their medical training would be delayed and impaired.</p></div><div><h3>Conclusions</h3><p>In a sample of medical students surveyed during the COVID-19 pandemic, mental health problems were common. All these patients were admitted to the psychiatric inpatient unit of a Spanish hospital.</p></div><div><h3>Results</h3><p>Despite the different diagnoses, the patients included presented different types of delusional misidentification syndromes, both hyperidentification and hypoidentification. Antipsychotic<span> treatment was not very effective against these delusional misidentification syndromes.</span></p></div><div><h3>Conclusions</h3><p>The coexistence of several delusional misidentification syndromes indicates that the aetiopathogenesis of the different types is similar. Students ages ranged from 12 to 18 years old (mean, 15.09<!--> <!-->±<!--> <!-->1.82). The participants completed three screening tools: a socio-demographic questionnaire, a screening instrument to collect information related to the frequency of use of some substances, such as tobacco and cannabis, and a self-report inventory to assess various personality and psychopathology domains. A series of t-tests, ANOVA and linear regression analyses were conducted.</p></div><div><h3>Results</h3><p>Physical aggression (t<!--> <!-->=<!--> <!-->7.74; <em>p</em> <<!--> <!-->0.01), cognitive depression (t<!--> <!-->=<!--> <!-->5.03; <em>p</em> <<!--> <!-->0.01), affective depression (t<!--> <!-->=<!--> <!-->8.24; <em>p</em> <<!--> <!-->0.01), affective instability (t<!--> <!-->=<!--> <!-->3.46; <em>p</em> <<!--> <!-->0.01), few social relationships (t<!--> <!-->=<!--> <!-->3.36; <em>p</em> <<!--> <!-->0.01), self-harm (t<!--> <!-->=<!--> <!-->3.45; <em>p</em><<!--> <!-->.01), cannabis and tranquilizer use (t<!--> <!-->=<!--> <!-->2.83; <em>p</em> <<!--> <!-->0.05; and t<!--> <!-->=<!--> <!-->2.37; <em>p</em> <<!--> <!-->0.05) had a significant independent relationship with suicidal behaviour. Fast and valid instruments are required for this population group. The objective is to establish the construct validity and reliability of the Generalised Anxiety Disorder (GAD-7) scale in Colombian doctors during the COVID-19 lockdown.</p></div><div><h3>Methods</h3><p>E-health study, in which cross-sectional data were collected online (n = 1030) from 610 COVID doctors and 420 non-COVID doctors, during the Colombian lockdown, between 20 April and 10 August 2020. Each subject was contacted, and they confirmed their participation, identity and professional role.</p></div><div><h3>Results</h3><p>A single factor factorial structure was found, made up of the 7 items of the instrument, which managed to explain 70% of the variance. The goodness of fit indices (RMSEA = 0.080; CFI = 0.995; SRMR = 0.053; p < 0.001) showed an “acceptable” unidimensionality and adequate factor loadings in each item of the GAD-7, >0.070. Finally, the internal consistency of the instrument was good, with a Cronbach’s alpha of 0.920 (95%IC, 8.80–9.71).</p></div><div><h3>Conclusions</h3><p>The GAD-7 is an instrument that presents adequate indicators of validity and reliability. As such, suicide is considered a public health problem, and prior suicide attempt is one of the risk factors associated with completed suicide. Despite the strategies implemented and the studies carried out, in Colombia suicide figures are on the rise, more markedly in the economically active population.</span></p></div><div><h3>Objective</h3><p>To identify the sociodemographic, family, personal, economic and religious factors associated with suicide attempt in patients of productive age (18–62 years old) in a mental health institution in Bogota, Colombia.</p></div><div><h3>Methods</h3><p>An analytical prevalence study was conducted at the Nuestra Señora de la Paz mental health clinic in Bogota. To explore the relationship between the factors described and suicide attempt, a review of 350 medical records of the selected population was carried out.</p></div><div><h3>Results</h3><p>In total, 37.7% of the sample presented a suicide attempt. Associations were found between the suicide attempt and higher education than primary school (PR<!--> <!-->=<!--> <!-->0.47 [0.23−0.97]), no economic income (PR<!--> <!-->=<!--> <!-->1.72 [1.13−2.61]), no partner (PR<!--> <!-->=<!--> <!-->2.10 [1.33−3.32]), alcohol consumption (<em>P</em> <!-->=<!--> <!-->.045), hallucinogen use (PR<!--> <!-->=<!--> <!-->2.39 [0.97−3.43]) and the presence of personality disorder (PR<!--> <!-->=<!--> <!-->1.93 [1.11−3.34]).</p></div><div><h3>Conclusions</h3><p>The results of the study are similar to those previously described in other studies around the world. In Colombia, the general population and healthcare professionals (even some psychiatrists) seem to have mostly negative opinions towards electroconvulsive therapy treatment, but maybe this could be reconsidered if more information is provided; therefore, the aim of the present study was to evaluate the changes in memory and the severity of the symptoms in a group of patients with severe depression before and after electroconvulsive therapy.</p></div><div><h3>Methods</h3><p>Twenty-three patients ranging in age from 23 to 70 years from the electroconvulsive therapy service at the San Juan de Dios Clinic (Manizales, Colombia) were recruited in order to assess the effect of electroconvulsive therapy on memory in patients with severe depression. Depressive symptoms and memory were assessed with the Hamilton Depression Scale (HAMD) and Rey Auditory Verbal Learning Test (RAVLT), respectively. The assessment was administered to participants before the initial treatment of electroconvulsive therapy series (0-1 day) and 2 days after their last treatment.</p></div><div><h3>Results</h3><p>Electroconvulsive therapy resulted in significant improvement in the rating of depression. There were no significant differences in the five learning trials, delayed recall, learning and forgetting scores from pre-treatment to post-treatment. Significant pre-treatment/post-treatment differences were found in the delayed recognition trial.</p></div><div><h3>Conclusions</h3><p>Pre- and post- electroconvulsive therapy cognitive assessment is a feasible and useful procedure. In general, memory performance does not worsen after electroconvulsive therapy in patients with depression.
