There are few publications on the environmental factors. The objective of this review is to present the role of psychosocial adversity in the aetiology and course of ADHD.</p></div><div><h3>Methods</h3><p>A search was carried out in the following databases: PubMed, ScienceDirect, SciELO, ClinicalKey, EMBASE, Lilacs, OVID, APA and PsycNET. English and Spanish were selected without being limited by type of study or year of publication. Finally, a qualitative synthesis was conducted.</p></div><div><h3>Results</h3><p>ADHD development could be related to exposure to adverse factors in the family, school or social environment. It has been proposed as an explanatory mechanism that adversity interacts with genetic variants and leads to neurobiological changes. There may also be a gene-environment correlation whereby individual hereditary characteristics increase the risk of exposure to adversity, and indirectly increase the probability of developing ADHD. Research on psychosocial adversity represents a big challenge, not only due to the complexity of its construct, but also to the effect of subjective perception of a given event.</p></div><div><h3>Conclusions</h3><p>ADHD aetiology is complex and involves the interaction of both genetic and environmental factors, in which these factors correlate and cause the disorder. For this reason, we will try to reflect this fact, which it seems necessary to remember even though it is well known, since it can be overlooked in emergency situations in the hospital setting, with the consequences derived from an incomplete diagnosis and with the potential life-threatening risk for the patient.</p></div><div><h3>Case presentation</h3><p>A 13-year-old female adolescent, who presented an acute clinical picture suggestive of dissociative disorder. Although doctors are reluctant to seek professional help when suffering from a mental disorder, specialised services developed specifically to treat their mental health problems have reported promising results. The purpose of this article is to describe the design and implementation of the Professional Wellbeing Programme (</span><em>Programa de Bienestar Profesional</em>) of the Uruguayan Medical Council (<em>Colegio Médico del Uruguay</em>). The context, inputs, activities and some of the outputs are described according to a case study design. The main milestones in the implementation of the programme are also outlined, as well as the enabling elements, obstacles and main achievements. Emphasis will be placed on the importance of international collaboration to share experiences and models, how to design the care process to promote doctors’ access to psychiatric and psychological care, the need for them to be flexible and dynamic in adapting to new and changing circumstances, such as the COVID-19 pandemic, and to work in parallel with the medical regulatory bodies. During 2019, in the month of July, clozapine was changed to </span></span></span>amisulpride, in September he developed fever, muscle stiffness, </span>stupor<span><span>, diaphoresis<span> and tachypnea over a two-week period; paraclinical tests showed elevated </span></span>creatine phosphokinase<span><span> (CPK) and leukocytosis<span>, so NMS was considered. The antipsychotic was withdrawn and he was treated with bromocriptine and </span></span>biperiden<span>, with a good response. Ten days after discharge<span>, he began treatment with olanzapine, which generated a similar episode to the one described in December, with subsequent management and resolution.</span></span></span></span></p></div><div><h3>Discussion</h3><p>The diagnosis is based on the use of drugs that alter dopamine levels, plus altered state of consciousness<span>, fever, autonomic instability and paraclinical tests showing leukocytosis and elevated CPK. Differential diagnoses must be ruled out. Early diagnosis generally leads to total remission, although some patients will suffer complications, long-term sequelae or recurrences. The recurrence in this case derived from the early reintroduction of the neuroleptic after the first episode. Treatment should be individualised according to severity to avoid mortality.</span></p></div><div><h3>Conclusions</h3><p>Atypical antipsychotics are rarely suspected of generating NMS. One approach of data modelling that reconciles multidimensionality and the existence of a general factor is the bifactor analysis. We used unrestricted exploratory-confirmatory bifactor modelling to validate the Brazilian version of the IIP-PD-47.</p></div><div><h3>Methods</h3><p>The sample consisted of 1,091 subjects aged 18-64 years who answered the IIP-PD-47 and a collateral measure of pathological traits, the Dimensional Clinical Personality Inventory 2 (IDCP-2).</p></div><div><h3>Results</h3><p>After testing many candidate models, our data were best represented by a bifactor model with one general factor and five specific uncorrelated factors. Nevertheless, a closer inspection of the discriminant validity of each IIP-PD-47 factor revealed strong support for the general factor and a factor capturing aggressive behaviours, but less support for the additional four specific factors.</p></div><div><h3>Conclusions</h3><p>The theoretical and practical implications of these findings are discussed, and some recommendations are offered about the need for controlling response styles when assessing PD traits via self-report inventories. The scale was administered to 321 health science professionals (78 males and 243 females), whose ages ranged from 22 to 64 years (38.12 ± 9.61).</p></div><div><h3>Results</h3><p>Aiken’s V-coefficient values were statistically significant. An exploratory factor analysis was performed which indicated the existence of a single factor, and confirmatory factor analysis (CFA) confirmed a satisfactory six-factor model. Their main characteristic is excessive concern about body weight and shape, which causes great discomfort and physical problems and leads to a decrease in quality of life and alterations in the patient’s functionality social environment. The objective of this study is to describe the emotional and behavioural symptoms of adolescents who consult a specialised ED programme in the city of Bogota.</p></div><div><h3>Methods</h3><p>Observational, descriptive, cross-sectional study, for which patients between 11 and 19 years old with an ED diagnosis were recruited.</p></div><div><h3>Results</h3><p>Forty patients with an ED diagnosis were included, of which 92% were female. The mean age of the patients was 16.6<!--> <!-->±<!--> <span><span>1.9 years; 57% of patients live in a two-parent home and 30% in a single-parent home; 72% of the sample had excellent academic performance; 50% were moderately ill; 60% received pharmacological management with SSRIs; 65% of patients met clinical criteria for anxiety disorder, 30% for depressive disorder; 22.5% had aggression problems; 17.5% criminal behaviour; 72.5% of the sample met clinical criteria for internalising symptoms and 42.5% for externalising symptoms, the majority being patients with a diagnosis of </span>bulimia nervosa.</span></p></div><div><h3>Conclusions</h3><p>Patients with bulimia nervosa obtained higher scores in the different emotional and behavioural symptoms than those with other eating disorders. Rodríguez","doi":"10.1016/j.rcpeng.2020.12.006","DOIUrl":"10.1016/j.rcpeng.2020.12.006","url":null,"abstract":"<div><h3>Objective</h3><p>To establish the factors associated with burnout syndrome in dental specialists working in the city of Bucaramanga and its metropolitan area.</p></div><div><h3>Methods</h3><p><span><span>A cross-sectional study was conducted in which the validated version of the Maslach Burnout Inventory-Human Services Survey (MBI-HSS) questionnaire was applied to 117 dental specialists. The variables analysed were the scores obtained in the three dimensions of the instrument - sociodemographic and job-related characteristics, professional environment, and habits. Spearman's correlation coefficient and the Mann Whitney or Kruskal-Wallis U test were used in the bivariate analysis, and a </span>logistic regression<span> was performed in the multivariate analysis. A value of p < 0.05 was considered statistically significant. Each participant accepted their inclusion in the study after the </span></span>Informed Consent process was done.</p></div><div><h3>Results</h3><p>The average age was 44.0 ± 7.8 years, and 57.3% were women. It was observed that 3.4%, 4.3% and 4.3% of respondents scored highly in emotional exhaustion, depersonalisation and lack of personal achievement, respectively. Cigarette smoking presented a direct association with the dimensions of depersonalisation (p = 0.031) and lack of personal achievement (p = 0.025). On the other hand, having completed the postgraduate degree 10 years or more ago showed a negative association in these two dimensions (p = 0.049 and p = 0.045, respectively).</p></div><div><h3>Conclusions</h3><p>The results suggest that burnout syndrome is not a frequent problem in dental specialists who work in Bucaramanga and its metropolitan area. With this clinical case, we intend to explore the difficulties in the pharmacological approach of delusional jealousy disorder and to summarise the most recent findings in the treatment of this condition.</span></p></div><div><h3>Methods</h3><p>Case report.</p></div><div><h3>Case presentation</h3><p>A 76-year-old man involuntarily admitted to a psychiatric ward due to threats of physical aggression to his wife in the context of irreducible ideas of her infidelity. Initially, we observed an improvement of symptomatology<span> with risperidone<span> and its long-acting injectable formulation, but the emergence of hypotensive side effects required the off-label use of paliperidone palmitate 50 mg/ml.</span></span></p></div><div><h3>Conclusions</h3><p>Few studies, mainly case reports, look at the specific treatment of delusional jealousy.
