Dutta, Sunil Seghal","doi":"10.26502/jppch.74050158","DOIUrl":"https://doi.org/10.26502/jppch.74050158","url":null,"abstract":"The last National Family Health Survey, 2019–2021, suggests suboptimal infant feeding practices and an increasing burden of micronutrient deficiencies across the country’s states. The research has well established an association of child nutrition with eating habits and health outcomes in later life. The latest National Family Health Survey-5 data state that only about 4 of every 10 infants are introduced solid foods timely i.e., introduced to complementary feeding as per the recommended age. It is equally worrisome to note that only 11.1% of all children in India in the age-group of 6–23 months were fed the minimum acceptable diet with a direct correlation of around 67.1% of children under the age of 5 years suffering from anemia. These findings highlight that incorrect timing of initiation of complementary foods, early or delayed, and inadequate nutrition are a few critical factors behind the prevalence of nutrient deficiencies. In this regard, a group of experts from the field of pediatric nutrition from different parts of India and Nepal gathered and acknowledged that in addition to lack of knowledge on age-appropriate nutrition to infants, cultural beliefs, traditions, myths, and lack of awareness about nutrient enhancers and inhibitors are some of the common challenges in meeting age-appropriate nutrition. The expert group also recognized that supplementation and fortification co-exist in the clinical practice where supplementation is offered on a case-to-case basis to address the therapeutic needs of micronutrient deficiencies. In contrast, fortification should be advised as part of a balanced diet to reduce the risk of micronutrient deficiencies. However, it was unanimously agreed that scientific research and clinical practice have established that fortification or the use of Fortified Infant Cereals (FIC) to have better compliance and suggests this as a strategy to be used as a preventive, long-term sustainable mass approach to partner with homemade feeds for meeting the nutritional needs during the critical period of 6–23 months of age. The present position paper captures the opinion and suggestions of a group of clinicians regarding the pervasive concerns and challenges of complementary feeding in a diverse country like India with varied cultural and eating preferences. During the 20-week intervention, participants completed a weekly online intervention module and joined individual online health coaching. Data were collected at baseline (<17 weeks gestation), 24-27 weeks gestation (T2), and 35-37 weeks gestation (T3). Lifestyle behaviors included dietary intake (caloric, fat, added sugar, fruit, and vegetable) and physical activity (PA). Psychosocial factors were autonomous motivation, self-efficacy, executive functions, and consideration of future consequences (CFC). Affect comprised stress and emotional control. Two-sample <i>t</i>-tests and Cohen's <i>d</i> effect sizes were used to compare between group mean differences in the change from baseline to T2 and T3.</p><p><strong>Results: </strong>At T2, intervention positively influenced fruit intake (<i>d</i> = 0.47), autonomous motivation for healthy eating (<i>d</i> = 0.36), self-efficacy for healthy eating (<i>d</i> = 0.25) and PA (<i>d</i> = 0.24), executive functions (behavior regulation, d = -0.21; metacognition, <i>d</i> = -0.69), and emotional control (<i>d</i> = 0.79). The aim of the study was to provide information about appropriate physical rehabilitation protocol using ICF model for the child with AMC. Case Description: The reported case for the study was 13 years old girl born with suffering from AMC. During pregnancy period mother had history of malnutrition and prolong labour pain was present and child had deformity at different joints. Child was not able to sit properly except modified cross sitting, stand or walk due to deformities. Patient had no history of operation and serial plaster casts for this deformed condition. She was residential students and attended the inclusive school where he participated in a regular special education program. He took routine based individual physiotherapy, occupational therapy and hippotherapy intervention. The Child and her mother gave consent and agreed to participate in this observational case report study for 12 months. Purposive sampling technique was applied considering the inclusion and exclusion criteria. The research was carried out between July 2021 and June 2022. Measurement tools were used ICF model, prone positioning time, passive ROM and muscle strength. This article enlightens rehabilitation treatment strategies for child with AMC. Conclusion: Physical therapy of AMC children ought to be multi-focused, continual and holistic. Early way to deal with physiotherapist limits the complexity following AMC. AN is highly associated with comorbid psychiatric disorders such as depress-sion, which makes treatment difficult. Pharmacolo-gical options are limited because selective serotonin re-uptake inhibitors (SSRIs) are less effective in underweight individuals. Electroconvulsive therapy (ECT) is an important treatment option in these patients. Is this caution justified? Case presentation: We describe a 21-year-old woman with restrictive AN and depression, with suicidality. Trials of psychotherapy for depression and various antidepressants were unsuccessful and her condition deteriorated. She was admitted to hospital several times because of the precarious state of her health due to the eating disorder and serious suicide attempts. ECT was started because of the severe suicidality and her restrictive eating pattern. There were no complications, and the eating disorder and depression improved relatively soon after the start of treatment. ECT provided a breakthrough in the treatment of an unresponsive, life-threatening situation, such that follow-up therapy could be started. The eating disorder regressed several months after ECT. Conclusions: Case reports have their limitations and there is no unequivocal evidence that ECT is effective for AN with comorbid depression. Yet the literature suggests that ECT should be considered in a life-threatening situation or when treatment possibilities are exhausted. AN and underweight are not contraindications for ECT, although medical screening before treatment is highly recommended. The long-term effectiveness of ECT has yet to be established.
