{"title":"Effect of Dietary Supplementation with Lactobacillus Strains in Non-Dairy Matrices on Colonic Metabolic Activities: A Clinical Trial","authors":"","doi":"10.29011/2577-2228.100339","DOIUrl":"https://doi.org/10.29011/2577-2228.100339","url":null,"abstract":"Background: Gut microbiota has beneficial effects on intestinal mucosa. It enhances immunity and produces metabolites that influence general health. This study aimed to evaluate the effect of dietary intervention with multi-strain potential probiotic Lactobacilli on functional metabolic colonic activities in healthy adult males. Furthermore, the study introduces dates and biscuits as non-dairy matrices for administration of probiotic Lactobacilli. Methods: The crossover study included eleven adult male volunteers, who were assigned to three different interventions consecutively, with each intervention lasting for 21 days. The participants were given biscuits with probiotics, followed by biscuits with placebo, and lastly dates with probiotics. Each phase was followed by a month of washout period. The consumed biscuits and dates contained probiotics with a total daily dose of 1.68 x 108 and 3.3 x 108 colony forming units, respectively. Before and after each test period, the volunteers were clinically examined and stool samples were collected. The samples were examined for Lactobacillus count, short chain fatty acids, secretory immunoglobulin A, ammonia contents, and stool pH using standard techniques. Results: After daily consumption of probiotics, a significant increase in fecal secretory immunoglobulin A and short chain fatty acids were observed, with associated significant decrease of fecal ammonia contents. As a whole, colonic metabolic functions were significantly improved. Conclusion: Biscuits and dates proved to serve as efficient matrices for delivery of live probiotics. {"title":"Cardiac and Other Presentation and Clinical Outcomes of COVID-19 Pandemic among Different Ethnic and Religious Populations in the city of Jerusalem","authors":"","doi":"10.29011/2577-2228.100288","DOIUrl":"https://doi.org/10.29011/2577-2228.100288","url":null,"abstract":"Background: The COVID ‐ 19 pandemic is an ongoing global pandemic. Jerusalem with its 919,400 inhabitants has a wide variety of populations, of which 62% are Jews (36% ultra-orthodox; 64% non-ultraorthodox) and 38% Arabs which were largely affected by the pandemic. The aim of our study was to understand the different presentations, course and clinical outcomes in these different ethnical and cultural groups in Jerusalem in the COVID-19 pandemic. Methods: We performed a cohort study of all COVID-19 patients admitted between March 9 - July 16, 2020 to the two university medical centers in Jerusalem. Patients were divided according to their religion and ethnicity into 3 main groups: 1) Ultra-Orthodox Jews; 2) other (non-Ultra-Orthodox) Jews and 3) Arabs. Results: Six hundred and two patients comprised the study population. Of them the 361 (60%) were UltraOrthodox Jews; 166 (27.5%) non-Ultra-Orthodox Jews and 75 (12.5%) Arabs. The Arab patients were younger than the Ultra-Orthodox Jews and the non-Ultra-Orthodox Jews (51±18 year-old vs. 57±21 and 59±19, respectively, p<0.01), but suffered from signi�cantly more co-morbidities. Moreover, hemodynamic shock, ischemic ECG changes and pathological chest x-ray were all more frequent in the Ultra-Orthodox patients as compared the other groups of patients. Being an Ultra-Orthodox was independently associated with signi�cantly higher rate of Major Adverse Cardiovascular Events (MACE) [OR=1.96; 95% CI (1.03-3.71), p<0.05]. Age was the only independent risk factor associated with increased mortality rate [OR=1.10; 95% CI (1.07 - 1.13), p<0.001]. Conclusions: The COVID-19 �rst phase in Jerusalem, affected different ethnical and cultural groups differently, with the Ultra-Orthodox Jews mostly affected by admission rates, presenting symptoms clinical course and MACE (Acute coronary syndrome, shock, cerebrovascular event or venous thromboembolism). We studied one hundred participants aged between 18-24 years responded to the survey; the mean age of our patients was 21 years. There was a correlation between (BMI, Daily food quality, Lifestyle) and (Number of symptoms, Symptoms improvement, Duration of treatment), respectively. There is mounting evidence supporting the possibility of vertical transmission. Case: A pregnant woman with SARS-CoV-2 infection was admitted in a designated hospital in her 33th week of pregnancy and was discharged 10 days later. A male newborn was delivered vaginally in her 40th week. The newborn was negative for SARS-CoV-2 RNA and positive for SARS-CoV-2 Immunoglobulin G and was followed-up for 1 year without development retardation. Summary and Conclusion: Mother-to-fetus transmission seems to be rare for this virus. Especially during the third trimester of pregnancy, there is no effect on fetus’s growth and development. Nurses play an essential role as part of the community health care workforce; however, only a limited number of baccalaureate nursing students tend to choose a career in community health nursing after graduation. There is currently a gap in knowledge surrounding nursing students’ perception of a career in Community Health Nursing and the issues influencing their career choice upon graduation. Purpose: To explore issues that influence career choice in community health nursing from the perspective of baccalaureate nursing students. Research Methodology: The study was guided by a descriptive qualitative research approach. Individual semi-guided interviews and focus groups were conducted with eleven nursing students and a group of key stakeholders to share their thoughts on pursuing a career in community health nursing and the factors that enabled or hindered their decision making. Thematic analysis of the interview and focus group data generated relevant themes. Findings: Two major themes were represented in the study: 1) societal trends and expectations, and 2) issues influencing career choice in community health nursing. Discussion and Implications: The personal and contextual factors influencing the perceptions and attitudes of students towards pursuing community health nursing are discussed. Existing literature was integrated into the discussion of the many factors that both motivated and hindered baccalaureate nursing students from pursuing community health nursing. The underrepresentation of new graduates in community health nursing calls for directed efforts by community health nursing organizations and the university to improve the situation. Additional indirect costs also increase the economic burden of households creating barriers of access and adherence that can affect the healing of patients. Because TB affects the poorest segment of society the effects can be more serious for poor people. Indirect costs are poorly documented in Bolivia. We therefore conducted a study to determine the socioeconomic impact of TB on patients and their households. Methods: A cross-sectional survey was carried out on 2017 in the Department of Cochabamba, Bolivia, included all patients who were on TB treatment within the health network. The generic survey instrument proposed by WHO for tuberculosis patients cost surveys was used. The most important direct medical and direct non-medical payments were calculated in a disaggregated manner. The calculation of indirect costs was carried out through the results-based approach where the loss of income was calculated from the report of household income. Catastrophic cost was determined and multivariable logistic regression was run to assess which factors were independently associated with a total catastrophic cost experienced by a TB patient. Results: The indirect costs during an entire illness episode had a median of USD 768.1 corresponding to 63.4% of the total cost for all patients. In the case of multidrug-resistant TB (MDR TB) patients, the median cost was USD 6454.8 corresponding to 54.3% of their total costs. Direct non-medical costs had a median for all patients of USD 408.7 (33.7%), and of USD 5399.6 (45.4%) for MDR TB patients. The most important non-medical costs were those linked to directly observed treatment (DOT) with a median of USD 419.8 during intensive phase and of USD 108.6 during continuation phase for transport costs. Food and drinks costs had a median of USD 94.5 during intensive phase and of USD 54.8 during continuation phase; consumption of nutritional supplements had a median cost of USD 249.1 and additional food outside the regular diet had a median cost of USD 519 for all patients. Catastrophic costs were incurred by 43.9% of TB patients. These catastrophic costs were observed in 50% of the population aged over 55 years, in 60% of MDR-TB patients, and in 63% of poor patients. Using logistic regression, factors associated with a higher risk of catastrophic costs were to live in an urban province (OR = 1.82 95%CI 1.11 to 2.98, P = 0.002), to be older than 55 years (OR = 1.21 95%CI: 1.07 to 1.38, P = 0.005), and to have a low economic income (OR = 0.83 95%CI: 0.69 to 0.99, P = 0.049). Conclusions: Patients with TB treated in health services of Bolivia face a great financial burden. Indirect costs were higher than direct costs in a complete TB episode, especially in MDR-TB patients.
