We present \u0000our initial experience with a structured framework for discussion, SKIPS (setting, knowledge, invitation, \u0000perception and strategy), as a modification to a previously reported tool for breaking bad news, SPIKES. \u0000Our new framework emphasizes patient beliefs and wishes in the setting of a discussion requiring ethical \u0000considerations or treatment decisions. {"title":"Early Breastfeeding Practices Promote Effective Parenting: An Analytical Study in Countries exposed to Conflict in the Middle East","authors":"A. Al-Jawaldeh, A. The data included: early \u0000initiation of breastfeeding [EIBF] in the first hour and the first day, offering prelacteals, exclusive \u0000breastfeeding [EBF] and continued breastfeeding [CBF]. Parenting practices included exposure to \u0000psychological aggression and physical punishment and attitudes to physical punishment for rearing in \u0000children 1-14 years. Childcare indices included child left alone, with care of a child less than 10 years and \u0000with inadequate caregiver. Correlative studies and descriptive statistical analysis were done. \u0000Results: EBI within the first hour of birth was significantly correlated with non-violent discipline [r0.5] \u0000P 0.05. EBF inversely correlated with psychological [r-0.5] \u0000and minor physical punishment [r-0.4] and being left alone in the past week [r-0.5] at P<0.05. Longer \u0000duration of breastfeeding for 12 and 24 months correlated inversely with severe physical punishment [r-0.6 \u0000at P<0.01 and r-0.5 at P<0.05 respectively]. CBF for two years was also negatively correlated with being \u0000left alone [r0.5 at P>0.05]. Variations between countries in parenting styles and adequacy of childcare \u0000related to breastfeeding patterns. \u0000Conclusions: Breastfeeding is protective against violent parenting styles and enhances adequacy of \u0000childcare. {"title":"Neuroleptic Cardiomyopathy and MyocardialHibernation","authors":"V. Volkov","doi":"10.32474/acr.2018.01.000113","DOIUrl":"https://doi.org/10.32474/acr.2018.01.000113","url":null,"abstract":"Aim: It is to summarize and in a new way to interpret the data, which have been earlier received, in order to prove the development of the hibernation of myocardium (HM) in cases of the neuroleptic cardiomyopathy (NCMP). Methods and results: Morphometric methods of research were used which meet modern requirements of the evidence-based medicine. The studied micro morphometric parameters describe the condition of three structural components of myocardium (vasculature, intercellular matrix, and parenchyma). Such parameters as zone of pericapillary diffusion (ZPD), Kernogan index (KI), SPR, RIE were calculated. Karyometry and cytometry of cardiomyocytes (CMCs) were performed, the specific volumes of hypertrophied CMCs (SVHC), of atrophied ones (SVAC), and – by the method of polarization microscopy – the specific volume of dystrophic ones (SVDC) were determined. When NCMP is absent, the changes, which have an acute character, develop in the case of SCD and NMS and reflect the statistically significant shifts of the respective quantitative parameters. When NCMP is present, the only parameter, which is significantly changed in connection with SCD and NMS, is SVDC. Conclusion: NCMP causes irreversible damages in myocardium, it leads to its are activity, practically almost fully prevents it from reacting to any influences, particularly to the coming of SCD and/or development of NMS. (Schwartz 2006). For many centuries it was not unveil the mystery of the origin of the illness. Gout is the unique pathology that belongs to the human race. When uric acid is deposited in the articulate tissue produce an intense inflammation, basic element in the development of gout. Interesting is the evidence of ultra sonographic signs of monosodium urate crystalline articulate deposits in 25% of clinically asymptomatic hyperuricaemic subjects (more than 8mg/ dL) [1], and approximately 9% of the joints without clinical signs of flogosis [2]. Large epidemiological studies have now demonstrated that gout is an independent risk factor for incident coronary heart disease, [3-6] heart failure, [7] stroke [8] peripheral artery disease [9] and death cardiovascular [10,11]. But, several meta-analyses have concluded that hyperuricaemia is an independent risk factor for coronary heart disease [12,13] and also, for the development of hypertension [14,15]. The standard diagnostic goal remains the identification of the typical birefringence of crystals of uric acid under polarized light microscope in the synovial fluid and in the aspirated material from the tophi [16,17]. At the same time, despite the active development of digitization processes of both the Russian and world economy, there is no single normative and/or doctrinal concept of cryptocurrency, both in economic science and in jurisprudence. We believe that this problem is of particular relevance in connection with the need to determine the state-power strategy of a public entity with regard to the development of Russian financial and tax legislation, considering current features of the financial system development in the digital age. In this regard, we consider the assessment of the “cryptocurrency” phenomenon taking into account the specifics of the Russian tax-legal regulation as one of the categories of taxation law, which is related to such fundamental notions as “object taxation” and “tax base”. Also, the development of the current concept of cryptocurrency significantly affects the most important notions of tax law-fiscal sovereignty, tax residency, legal personality, taxpayers and others.
