Therefore, the research project \"TEL-O-Rena\" aimed to develop a multimodal, accessible, telemedical rehabilitation aftercare service for this target group. The scientific evidence regarding the effectiveness of such interventions is currently limited.</p><p><strong>Material and methods: </strong>Literature reviews, expert interviews, application observations, and surveys of people with visual impairment were conducted in the fields of visual rehabilitation, movement therapy, psychology and pedagogics. Based on this, corresponding therapy curricula are created.</p><p><strong>Results: </strong>Tools were identified in the respective subfields and compiled into assessments that systematically capture the specific needs of participants at the end of inpatient ophthalmological rehabilitation. Based on these findings, telemedical therapy modules were developed, including optometric exercises to compensate for visually induced strain symptoms, motor training to improve balance and cervical motor control and a psychological intervention to reduce severe fatigue. These modules were adapted to the specific needs of people with visual impairment under inclusive pedagogical considerations.</p><p><strong>Conclusion: </strong>A basic set of accessible telemedical therapy modules has been developed. This is particularly true for the older population, who are most commonly affected by AMD.</p><p><strong>Objectives: </strong>Analyzing the awareness of an existing AMD diagnosis in the older population. We evaluated AMD self-reports of participants of the population-based AugUR study aged ≥ 70 years in and around Regensburg using the records of treating ophthalmologists (BAA).</p><p><strong>Materials and methods: </strong>AMD self-reports of the AugUR participants were collected at study inclusion using interview-based questionnaires. An AMD diagnosis documented by the BAA was recorded by the BAA using an online questionnaire. Consensus or dissent of the available information was determined by creating a timeline for the date of study inclusion, the date of the first documented AMD diagnosis by the BAA and the date of the last visit to the BAA.</p><p><strong>Results: </strong>Self-report and BAA records on AMD were available for 1473 AugUR participants. Consensus was reached for 1270 individuals (86%). Of the 262 individuals with an existing BAA AMD diagnosis prior to study inclusion, 166 (63%) reported that they did not have AMD. In 137 of these 166 \"underreporters\", early stages of AMD were documented by the BAA.</p><p><strong>Conclusion: </strong>Our results suggest that 63% of the older population with a BAA-AMD diagnosis are unaware of their AMD diagnosis. Most of these diagnoses represent early stages of AMD, which may not be communicated to patients as \"AMD\". It also represents the most common cause of IOL explantations. An increasing incidence of this complication has already been described in the literature since 1995. The aim of this study is to observe the development of the incidence of IOL dislocation within our department over the last 10 years and to identify and evaluate possible risk factors.</p><p><strong>Materials and methods: </strong>In this retrospective monocentric analysis, a total of 578 eyes of 544 patients, who had undergone surgery for an IOL dislocation within the last 10 years, were included. The presence of myopia, pseudoexfoliation, prior vitrectomy or intravitreal injection (IVI), traumatic events and use of topical prostaglandin therapy were analyzed as possible risk factors.</p><p><strong>Results: </strong>Since 2017, there has been an increase in the incidence of IOL dislocations (R<sup>2</sup> = 0.45). On average, the dislocations occurred 11.7 years after cataract surgery, at an age of 75.6 ± 11 years. Myopia was present in 62% of the patients, of which 40.6% had high myopia. Pseudoexfoliation was documented in 38% of cases. No clear correlations were found for the further analyzed factors, such as prior prostaglandin therapy, IVI or trauma.</p><p><strong>Conclusion: </strong>Due to the increasing number of cataract surgeries, a further increase in the incidence of IOL dislocations is to be expected in the future. Similar to acanthamoeba keratitis, it is often initially misdiagnosed with the slit lamp. For more than 20 years an increase in the incidence of fungal keratitis has been observed, also in Germany possibly due to the increased use of soft contact lenses and climate change. This article provides an overview of the typical clinical signs and symptoms of keratomycosis and presents the latest findings in the field of diagnostics as well as drug and, in particular microsurgical treatment.
