Methods: 53 cases of suspected silicosis complicated with pulmonary tuberculosis were \u0000selected in the time span ranging from February 2017 to May 2019. TB-IGRA test, \u0000PPD test, TB-DNA-PCR and TB-Ab detection were performed. The sensitivity, specificity, \u0000positive predictive value and negative predictive value were calculated. Results: Silicosis and pulmonary tuberculosis were diagnosed in \u000011 cases, with an incidence of 20.75%. The positive rates of TB-IGRA, PPD, TB-DNA-PCR \u0000and TB-Ab were 66.04%, 30.19%, 5.67% and 26.42%, respectively. The sensitivity was \u000090.91%, 81.82%, 27.27% and 54.55% respectively. The specificity was 42.86%, 80.95%, \u0000100% and 80.95% respectively. The positive predictive values were 28.57%, 50%, 100% \u0000and 42.86% respectively. The negative predictive values were 94.44%, 91.89%, 84% \u0000and 87.18%. The positive rate, sensitivity and negative predictive value of TB-IGRA \u0000were the highest, while the specificity of TB-DNA-PCR was the highest yet with low \u0000positive rate, sensitivity and positive predictive value. Liotta","doi":"10.4236/JTR.2021.92006","DOIUrl":"https://doi.org/10.4236/JTR.2021.92006","url":null,"abstract":"Background: In patients with HIV, conventional tests are of low sensitivity; therefore, a new diagnostic test with hydrogel nanoparticles with reactive blue dye is proposed, which allows the capture, conservation and concentration of ESAT-6 in urine. NIPAs are copolymers that capture low molecular weight proteins and protect against enzymatic degradation. Using an immunoassay, it is possible to detect and quantify ESAT-6 present in urine as a possible marker of active TB. Design/Methods: Study in Lima, Peru, HIV+ participants, ≥18 years with and without tuberculosis (TB). Smear microscopy, culture in solid medium and urine immunoassay were performed. The reference was the diagnosis of TB by radiological or clinical microbiological criteria (indication for TB treatment). There were 2 preanalytical processes: untreated and treated urine (centrifuged, heated), then incubation with NIPAm. After washing, elution, sonication, heat and centrifugation, the final eluate was obtained. This was placed on nitrocellulose membranes, which by means of fixation and incubation processes with anti-ESAT-6 and anti-IgG antibodies, revelation with C-DiGit® Blot Scanner and FluorChem R FR0001. Calibration curves were included in the membranes, density was measured using Image J software. ROC curves, sensitivity and specificity were obtained. Results: The result by groups was HIV+ patient: ROC: 0.75, cut-off point ≥24.06 ng/ml, sensitivity 76.32%, specificity 68.89%, patients ≤200 cells CD4 mm3/ml, ROC: 0.78, cutoff point ≥26.20 ng/ml, sensitivity 75.86%, specificity 71.88%, patients >200 CD4 mm3 cells/ml, ROC: 0.73, cutoff point ≥24.6 CD4 mm3/ml, sensitivity 73.68%, specificity 73.68%. However, when extra-pulmonary \u0000tuberculosis affects the cervico-facial \u0000region, the cervical lymph nodes are the most involved. Aim: Throughout this case report, we put emphasis on keeping in mind of the possible \u0000tuberculous infection in a chronic lesion of mandible in children. Case \u0000presentation: In this article, we present a case of a 2-year-old \u0000child who was complaining of swelling of the right angle of the mandible. The \u0000computed tomography showed an osteolytic \u0000lesion suggesting tuberculous osteomyelitis but didn’t rule out a bony \u0000malignant neoplasm of the mandible. The histological examination of biopsy of \u0000the lesion proved the tuberculous osteomyelitis \u0000of the mandible. The child has received anti-tubercular therapy for 9 months, and she marked a good recovery. These drugs are notorious for inflicting serious adverse drug reactions \u0000(ADRs), which many studies have shown causes a wide range of economic and \u0000health problems including death. Aim: The \u0000study examined the prevalence of ADRs, associated risk factors, socio-demographic \u0000association and outcomes among patients treated for MDR-TB at a comprehensive \u0000tuberculosis treatment center in Nigeria. Method: The \u0000study was conducted at the Government Chest Hospital, Jericho, Ibadan. We \u0000applied a retrospective assessment of patient treatment data and ADRs reports stored at the study \u0000site from March 2013 and February 2016. Subsequently, a prospective study of \u0000ADRs was conducted on patients admitted into the same hospital. Causality \u0000relationship between the drugs and the reported ADRs was determined with a specially validated tool. The outcomes assessed include recovery from the ADRs, death and permanent deafness from the ADRs. Extracted data were analyzed using SPSS \u0000version 22.0. Risk Ratio was calculated for the influence of risk factors for \u0000adverse drug reactions. Logistic regression was performed to test for the \u0000strength of relationships between risk factors and incidence of ADRs among \u0000patients. Result: Almost all the participants in this \u0000study reported adverse drug reaction [99% (118/119)]. However, ototoxicity was \u0000the most prevalent ADR (35.3%), followed by electrolyte imbalance (12.6%), gastrointestinal track (10.1%) and psychiatric disorders (10.1%). Being older \u0000than 35 years and HIV negative or having a healthy BMI were not significant \u0000risk factors for developing ADRs. Duration \u0000of ADR above one month was significantly associated with the outcome of ADR. Conclusion: Ototoxicity, electrolyte imbalance, psychiatric disorders and gastrointestinal \u0000tract problems were the most frequently reported ADRs. Despite recent improvements in TB case notification, estimates from the National Prevalence survey suggest that there is still a significant pool of missed TB cases in the state. Although active TB case finding interventions are needed at community level, information on local TB transmission hotspots is lacking. The objective of this study was to map the geo-spatial location of all TB cases detected in the state in 2019. Findings from this secondary data analysis will help to target interventions appropriately with a view to achieving better program efficiency. Method: A de-identified dataset containing descriptive physical addresses of registered TB cases in 2019 was developed. The dataset was then deconstructed and restructured using Structured Query Language in a relational data base environment. The validated dataset was geocoded using ArcGIS server geocode service and validated using python geocoding toolbox, and Google geocoding API. The resultant geocoded dataset was subjected to geo-spatial analysis and the magnitude-per-unit area of the TB cases was calculated using the Kernel Density function. TB case notification rates were also calculated and Choropleth maps were plotted to portray the TB burden as contained in the dataset. Results: Five local government areas LGAs (Onitsha North, Onitsha South, Idemili North, Nnewi North, Ogbaru) had spots with “Extremely high” burden with two LGAs (Onitsha North and South) accounting for the largest spots. Eight LGAs had spots with “Very high” TB burden. Also, 24 hotspots across the state had “High” TB burden and two LGAs (Orumba North, Orumba South) had only “Low” TB burden areas. According to the World Health Organization data, our country, Mali has detected only half of the 10,385 cases of tuberculosis expected for 2014 for a population of 17,309,000 inhabitants. The objective of this present work was to describe the different clinical aspects and the epidemiology of tuberculosis at Hospital Sominé Dolo in Mopti. We performed a retro-prospective and descriptive of tuberculosis cases diagnosed in our department of medicine between May 2016 and August 2018. A total of 96 tuberculosis cases were recorded, i.e. 4.6% and 1.0% for hospitalizations and consultations patterns, respectively. The median of age was 41 with extremes from 5 to 80 years. The age group [31 - 40 years] was the most affected with 20.8%. Men and women were affected in identical proportions, i.e. 50%. Pulmonary locations were the most frequent with 55.2%. Pleural tuberculosis was far the most frequent of the extra-pulmonary forms with 24.0% followed by the peritoneal and bone localization with 6.3% each. The majority of patients were followed on an outpatient basis, i.e. 90.6%. The disease lethality was 7.3%. Unfortunately, \u0000resistance has been reported to a vast majority of currently use drugs. The \u0000objective of this study was to search for new therapies from \u0000plant products for the sustainable management of tuberculosis and S. aureus infections. Materials and Methods: Crude \u0000extracts were obtained by sequential maceration of dry powdered plant material \u0000in three solvents; hexane, dichloromethane and methanol. Phytochemical \u0000screening to identify active constituents in the crude extracts was done by \u0000conventional methods. The antimycobacterial and antimicrobial activity on M. tuberculosis control strain \u0000H37Ra and a clinical isolate of S. \u0000aureus respectively, was evaluated at eight different \u0000concentrations using the microplatealamar blue assay. Cytotoxicity of the active \u0000extracts was evaluated on monkey kidney epithelial cells and assessed using the \u0000MTT/formazan assay. Results: Twelve crude extracts \u0000were obtained, the hexane extract of Sanseviera liberica rhizomes (P3rH) showed antimycobacterial \u0000activity with a minimum inhibitory concentration (MIC) of 1 mg/mL. The hexane \u0000and methanol extracts of Emilia \u0000coccinea, P2H and P2M respectively, showed antimicrobial activity \u0000with MICs of 500 μg/mL and 1000 μg/mL respectively. P2M and P2H had selectivity \u0000indices of respectively, 0.1046 and 0.2336. Conclusion: This study validates the use of S. liberica and E. coccinea for the traditional management of \u0000tuberculosis and S. aureus \u0000infections, respectively. At the same time, tuberculosis (TB) has been identified as \u0000the leading cause of death amongst all infectious diseases globally. Routine \u0000screening of clients visiting health facilities can help to prevent the spread \u0000of these diseases. Aim: To assess \u0000the relationship between the practice of \u0000facility-based routine tuberculosis screening and routine screening for \u0000COVID-19. Methodology: Using a Snowball technique, a \u0000cross-sectional online survey was carried out during the national lockdown from \u00005 July to 5 August 2020. The target population for this survey was health care \u0000workers from the different health facilities across Nigeria. An online semi-structured questionnaire was used to \u0000interview healthcare workers to identify their knowledge, attitudes, and practices (KAP) towards COVID-19 and the practice of routine TB screening. Descriptive analysis, \u0000analysis of variance (ANOVA), and Pearson’s Chi-square test was used for \u0000statistical comparative analysis. Results: This shows \u0000that 53.9% of healthcare workers did not practice routine TB screening while \u000046.9% did not practice routine COVID-19 screening. Respondents who practiced \u0000routine TB screening were found to be more likely to practice routine COVID-19 \u0000screening (p 0.001). Healthcare workers in primary healthcare centers were more likely \u0000to carry out routine screening for both diseases (p 0.001) \u0000and among these, Community Healthcare \u0000Workers were more likely to carry out routine screening for both diseases than \u0000other cadres (p 0.001). In developed countries, \u0000incidences are minimal and however, complicated manifestations of tuberculosis are \u0000becoming a trend within the prison system. This case \u0000report illustrates a patient within the United States correctional system who demonstrated a rare \u0000infection of tuberculosis despite absent pulmonary findings. Our Patient was a 39-year-old \u0000inmate presenting with fatigue, night sweats, \u0000weight loss, and progressive dyspnea. Pulmonary imaging and cultures of sputum were negative for \u0000tuberculosis. Further imaging and laboratory workups demonstrated extensive \u0000spinal infections along with intracranial lesions that were positive for \u0000tuberculosis. Neurosurgery declined operative medicine due to advancement of \u0000the disease and appropriate antibiotic therapy was immediately initiated. \u0000Tuberculosis continues to exist in developed countries with an increased emergence of \u0000complicated cases. Furthermore, incarceration \u0000dramatically increases an individual’s risk of worse infections. The clinical \u0000impact involves promoting awareness for a need to identify and prevent \u0000continued spread of disseminated infections.
