{"title":"COVID-19 Vaccine Uptake: Prevalence, Health Facility Enablers and Barriers among Adult Tuberculosis Patients across Nairobi County Clinics, Kenya.","authors":"Waqo Boru, George Makalliwa, Caroline Musita","doi":"10.4236/jtr.2025.131002","DOIUrl":"https://doi.org/10.4236/jtr.2025.131002","url":null,"abstract":"<p><p>Patients with tuberculosis (TB) are at increased risk of developing severe forms of novel coronavirus 2019 (COVID-19), and COVID-19 coinfection aggravates active TB progression through immunosuppression. Despite the availability of COVID-19 vaccines to prevent and reduce COVID-19 transmission, COVID-19 vaccine uptake levels remain low worldwide. In Kenya, an African country with high TB prevalence and TB-HIV coinfection, 47% of the population had received the COVID-19 vaccine by 2023. This study determined health-system enablers and barriers to the COVID-19 vaccine among adult TB patients across Nairobi County TB clinics in Kenya. An analytical cross-sectional study was used. Three hundred eighty-eight TB patients from six TB clinics across six sub-counties in Nairobi were recruited. The participants completed the study questionnaire. After confounding for age, employment status perceived COVID-19 susceptibility, and perceived COVID-19 seriousness, health system enablers of COVID-19 vaccine uptake were consistent and positive messaging on COVID-19 (adjusted odds ratio (aOR) = 4.498; 95% CI: 1.953 - 10.36, p < 0.001), the feeling that the vaccination had significant social benefits was (aOR = 2.632; 95% CI: 1.108 - 6.257, p = 0.028), and enough public awareness about the vaccine (aOR = 2.619; 95% CI: 1.099 - 6.239, p = 0.03). A significant barrier was vaccine preference (aOR = 0.387, 95% CI: 0.179 - 0.838, p = 0.016). This study confirmed health facility factors that enable and hinder COVID-19 vaccine uptake among TB patients. {"title":"Healthcare Worker-Related Factors Contributing to Tuberculosis Treatment Non-Adherence among Patients in Kisumu East Sub-County.","authors":"Marlyn Ochieng, Jackline Nyaberi, Susan Mambo, Charles Wafula","doi":"10.4236/jtr.2024.121002","DOIUrl":"https://doi.org/10.4236/jtr.2024.121002","url":null,"abstract":"<p><strong>Background: </strong>Treatment non-adherence poses significant risks to health outcomes and impedes the health system's efficiency, hence curtailing progress towards the end Tuberculosis (TB) strategy under SDG 3.3. Despite interventions to address TB treatment non-adherence, Kenya still reports high TB treatment non-adherence rates of 35% and consequently poor treatment outcome rates. Health Care Workers (HCWs) play a critical role in linking the population to health services, yet little is known of their influence on patients' TB treatment non-adherence in Kenya.</p><p><strong>Objective: </strong>To analyze HCW-related factors associated with TB treatment non-adherence among patients in Kisumu East Sub-County.</p><p><strong>Methods: </strong>Health facility-based analytical cross-sectional mixed-method study. A Semi-structured questionnaire on treatment adherence and patients' perceptions of HCWs during the clinic visit was administered to 102 consenting adult (out of a total census of 107 adults) drug-susceptible TB patients. 12 purposively selected HCWs by rank from 6 health facilities participated in Key Informant Interview sessions. Medication adherence was measured using the Morisky Medication Adherence Scale and then expressed as a dichotomous variable. Quantitative analysis utilized STATA version 15.1 while qualitative deductive thematic analysis was done using NVIVO version 14.</p><p><strong>Results: </strong>TB treatment non-adherence rate of 26% (CI: 18% - 36%) was recorded. Overall, patients who felt supported in dealing with the illness were 8 times more likely to adhere to treatment compared to those who were not (aOR = 7.947, 95% CI: 2.214 - 28.527, p = 0.001).
